In this study, a biodegradable Graft made of composite nanofibers is introduced. A composite nanofiber core-shell structure was prepared by a combination of conjugate electrospinning and freeze-dry technology. The core fiber was poly(l-lactide-co-caprolactone) (PLCL)-based and the core fibers were coated with heparin/silk gel, which acted as a shell layer. This special structure in which the core layer was made of synthetic materials and the shell layer was made of natural materials took advantage of these two different materials. The core PLCL nanofibers provided mechanical support during vascular reconstruction, and the shell heparin/silk gel layer enhanced the biocompatibility of the Grafts. Moreover, the release of heparin in the early stage after transplantation could regulate the microenvironment and inhibit the proliferation of intima. All of the Graft materials were biodegradable and safe biomaterials, and the degradation of the Graft provided space for the growth of regenerated tissue in the late stage of transplantation. Animal experiments showed that the Graft remained unobstructed for more than eight months in vivo. In addition, the regenerated vascular tissue provided a similar function to that of autogenous vascular tissue when the Graft was highly degraded. Thus, the proposed method produced a Graft that could maintain long-term patency in vivo and remodel vascular tissue successfully.

the using of y Blood Vessel Graft anastomosis in coronary artery bypass surgery
Chinese journal of experimental surgery, 2006Co-Authors: W U ZhiyongAbstract:To review the clinical results of descending thoracicaorta "Y" Blood Vessel Graft anastomosis. Methods 16 patients whose ascending aorta were calcificated underwent coronary bypass surgery with MIDCAB. Age range was from 51 to 75 years with a mean of 60.5±10.5 years. Patients suffered from multi-Vessel disease. We used many minimally invasive techniques including "Y" Blood Vessel Graft anastomosis, anastomosis of Blood Vessel Graft to descending aorta, minimally invasive direct and thoracoscope assist, etc. Results All patients were survived. The mean duration of intubation was 8.7± 6.5 hours. The average ICU stay were 3.1±0.5 days. All patients were not Blood transfused. All survived patients had no symptoms of angina during the short-term follow-up( 1 to 17 months). Conclusion Descending thoracic aorta "Y" Blood Vessel Graft anastomosis was a new procedure for minimally invasive direct coronary artery bypass. The earlier outcome was satisfacted.

Chinese Journal of Thoracic and Cardiovaescular Surgery, 2008Co-Authors: Zhiwei Wang, W U Hongbing, H U XiaopingAbstract:Objective To investigate the clinical results of minimally invasive coronary artery bypass Grafting with "Y"Blood Vessel Graft anastomosis from descending thoracic aorata to coronary artery.Methods There were 66 patients with coronary heart disease.including 49 males and 17 females.Their age ranged from 46 to 82 years with a mean age of(63.8±9.0)years.Multi-Vessel disease was found in all patients bi coronary angiography,double ones in 15,three ones in 38,and four ones in 13 Left ventrcular ejection fraction was from 0.35-0.62with a mean of 0.53±0.08.Their angina pectoris was classified by the Ganda Cardiovascular Society(CCS),class Ⅱ in 15,class Ⅲ in 36,class Ⅳ in 15.Several minimally invasive techniques were used including"Y"Blood Vessel(saphenous vein) Graft anastomosis from descending thoracic aorta to coronary artery,small incision on the left thorax and dissection of interal mammary artery by thoracscope,off-pump cornary artery bypass Grafting,etc.The meaning of"Y" Blood Vessel Graft anastomosis is that one main Vessel Graft supplied Blood to myocardium from decending thoracic aorta whose anastomotic stoma is 5 cm diaphragm.by end to side anastomoses,mother veasel spplied Blood to myocardium from the main Vessel Graft by end to side anastomoses.The Grafts wer measured by Medi-Stim Butterfly Flowmetre for transient flow quality.Results All patients were sur vivid.The mean duration of ventilation was (5.6±3.2)hous.The average ICU stays were(37.3±7.6)hous.The average chest grainage were(190±75)ml.The average Blood transfusion was(150±50)ml.The Qm was(27±7)ml/min,PI was 3.5±1.4.Insuf was(7±5)%.All survived patients were followed up(12 to 48 months)bi UCG and ECG,which showed they had no symptoms of anginactoris,their activity amount and cardiac function were improve.Conclusion The"Y"Blood Vessel Graft anastomosis from descending thoracic aorta to coronary artery with minimally invasive incision on the left thorax,dissection of internal mammary artery by thoracoscope and off-pump coronary artery bypass Grafting was a new procedure for coronary artery Grafting.This new procedure can prevent ascending aorta which wire extended anda calcificnted from being distroyed,so it can prevent emlolism of cerebral artery and coronary artery which come from ascending aorta with extence,atherosclerossi and calcification.It is satisfactory,efficient,minimally invasive.The short-term outcome was satisfactory and the long-term outcom will be confirmed in the future. a case of deep femoral artery aneurysm

Because of the growth of the mass and the worsening of his symptoms, the patient visited our hospital. Multidetector computed tomography revealed a large deep femoral arterial (DFA) aneurysm. Surgical intervention was planned because of the large size of the aneurysm, the high risk of perforation and the worsening symptoms. Aneurysmectomy and revascularization of the distal DFA with an artificial Blood Vessel Graft were performed. DFA aneurysms are extremely rare. These aneurysms have a high rate of rupture, and surgery plays an important role in their treatment. However, standard methods have not yet been established because of their rarity of DFA aneurysm. We describe a case of DFA aneurysm in a patient who was successfully treated with aneurysmectomy and revascularization with an artificial Blood Vessel Graft.
