We present two more causes of prenatal anaemia leading to Blueberry Muffin lesions: chronic fetomaternal haemorrhage and severe intracranial bleeding.

Langerhans cell histiocytosis presenting as Blueberry Muffin Baby.

The clinical lesions most commonly result from intrauterine infections, such as rubella and cytomegalovirus, and less commonly with malignancy and hematologic disorders. Langerhans cell histiocytosis is a clonal proliferation of dendritic histiocytes in the skin. This has very rarely been associated with a Blueberry Muffin presentation. We report the case of a newborn with typical lesions of cutaneous hematopoiesis and lytic bone lesions related to Langerhans cell histiocytosis. At birth, approximately 40 2 mm to 5 mm purpuric, nonblanching macules were scattered on the trunk, extremities, and soles of our patient. Laboratory studies were unremarkable and cultures were negative. Skin biopsy showed a dermal proliferation of histiocytes staining positive for S100 and Cd1a. Pediatric bone surveys, chest radiographs, and computed tomography scans of the head were normal. Six months later, the skin lesions had resolved, but radiographs revealed lytic bone lesions of the right tibia, right ilium, and left pubic ramus, consistent with skeletal Langerhans cell histiocytosis.

Disseminated juvenile xanthogranulomatosis in a newborn resulting in liver transplantation.

Although most patients have solitary lesions, some have multiple cutaneous lesions, associated visceral lesions, or both. Although juvenile xanthogranulomas are usually confined to the skin; visceral involvement does occur in a small subset of patients and can be life threatening. We describe a newborn girl initially described as a "Blueberry Muffin Baby" with multiple juvenile xanthogranulomas of the skin and liver that were complicated by progressive cholestasis and portal hypertension necessitating a liver transplantation.

Fetal hypoxia secondary to severe maternal anemia as a causative link between Blueberry Muffin Baby and erythroblastosis: a case report

Congenital infections of the TORCH complex (toxoplasmosis, other, rubella, cytomegalovirus and herpes) and hematological disorders have been classically associated with this neonatal dermatological manifestation. We report for the first time an unusual presentation of Blueberry Muffin lesions in a neonate born from a mother affected by severe anemia during pregnancy. Case presentation A male, white Caucasian, neonate showed a cutaneous rash at birth, suggestive of “Blueberry Muffin”-like lesions. These cutaneous lesions were associated with marked elevation of the circulating nucleated red blood cells, and with ultrasound findings of peculiar brain ischemic porencephalic lesions. The clinical features of spontaneous disappearance and the association with marked erythroblastosis strongly suggest that these dermatological findings may be the consequence of an extramedullary hematopoiesis unexpectedly evoked by the intrauterine chronic exposure to hypoxia caused by severe maternal anemia. Conclusions In conclusion, fetal hypoxia secondary to severe maternal anemia may play a causative and unreported role in the development of neonatal Blueberry Muffin lesions.
