  • Lymphoepithelial Cyst of the thyroid coexistent with amyloid goiter

    Abstract:

    A case of amyloid goiter in a 73-year-old man with systemic amyloidosis complicating long lasting rheumatoid arthritis is reported. The thyroid gland was massively infiltrated with amyloid and contained foci of fat tissue and epithelial solid cell nests. Incidentally, a Cyst lined by squamous epithelium, with small amount of lymphocytes in the subepithelial stroma was found. This Cyst fulfilled the criteria for the diagnosis of a lymphoepithelial Cyst presumably of Branchiogenic origin. The herein described case of simultaneous occurrence of Branchiogenic Cyst and amyloid goiter represents, to our best knowledge, a unique coexistence of these two entities. The possible relationship between these lesions and their differential diagnosis is discussed.

    Abstract:

    Abstract A case of a Cystic metastasis of squamous cell carcinoma of palatine tonsil in a 54-year-old male is reported, clinically presenting as a Branchiogenic Cyst. The grade of differentiation of the squamous cell epithelium lining the Cyst was very variable: in some areas it was extremely well differentiated, thus resembling common benign branchial cleft Cyst; transition to dysplastic areas with features of carcinoma in situ was visible and finally, in one small focus, invasion of the epithelial structures into the lymphoid tissue was observed. Despite its metastatic nature, the tumor fulfilled histological criteria of a so called malignant Branchiogenic Cyst (Branchiogenic carcinoma). The authors discuss the existence of primary malignant Branchiogenic Cyst and the criteria necessary for its diagnosis.

  • Diagnosis and treatment of branchial cleft Cyst

    Abstract:

    Purpose:To study the diagnosis and treatment of Branchiogenic Cyst. Methods:Fifty nine cases of branchial cleft Cyst treated in Cancer Hospital of Shanghai Medical University from 1990 to 1999 were analysed. Results:The branchial cleft Cysts more frequently arose from the second branchial pouches and also from the first pouches. The operation was more difficult when the neck mass was bigger and the history was longer. All our cases were completely excised and had pathological proof. Conclusions:The branchila cleft Cyst is a rare disease. The FNA aspiration histopathologic examination, B ultrasonography and CT can help the diagnosis of the nech mass which is clinically suspected as Branchiogenic Cyst. The optimal treatment is the early diagnosis an complete excision.

    Abstract:

    Introduction: The existence of malignant Branchiogenic Cyst has been controversial since it was first described. Most authors believe that it is actually a head and neck primary tumor Cystic metastasis. Objective: Discuss the existence of malignant Branchiogenic Cyst. Case Report: A patient with a Cystic mass in front of the sternocleidomastoideus muscle superior part, which was removed as a malignant Branchiogenic Cyst. However, the anatomopathological examination suggested it was an unknown primary tumor. The patient has been followed-up with periodic examinations without evidences of a primary tumor. Conclusion: Probably we will never know whether it was a malignant Branchiogenic Cyst or an unknown primary metastasis, due to diagnostic criteria overlapping
