Practical utilization of Cardiac Computed Tomography for the success in complex coronary intervention

Because the conventional angiographic analysis is limited by the inability to visualize the plaque information and the occluded segment, Cardiac Computed Tomography has evolved as an adjunct to invasive angiography to better characterize coronary lesions to improve success rates of PCI. Adding to routine image reconstructions by coronary Computed Tomography angiography, the thin-slab maximum intensity projection method, which is a handy reconstruction technique on an ordinary workstation, could provide easy-to-understand images to reveal the anatomical characteristics and the lumen and plaque information simultaneously, and then assist to build an in-depth strategy for PCI. Especially in the treatment of chronic total occlusion lesion, these informations have big advantages in the visualization of the morphologies of entry and exit, the occluded segment and the distribution of calcium compared to invasive coronary angiography. Despite of the additional radiation exposure, contrast use and cost for Cardiac Computed Tomography, the precise analysis of lesion characteristics would consequently improve the procedural success and prevent the complication in complex PCI.

Comprehensive assessment of takotsubo cardiomyopathy by Cardiac Computed Tomography

Although Cardiac CT was originally conceived for the assessment of the coronary artery tree, it has evolved to embrace several non-coronary applications. These include myocardial assessment, resulting in myocardial characterization comparable with that of CMR and Cardiac CT for late enhancement imaging. In this report, we describe a patient with takotsubo cardiomyopathy who underwent a comprehensive assessment using Cardiac CT. This technique enabled to identify the condition of the coronary arteries and to evaluate the four-dimensional left ventricular function and myocardial late iodine enhancement immediately. The information obtained was similar to that provided by CMR; however, Cardiac CT is more practical than CMR in emergency settings. Comprehensive assessment by Cardiac CT can be beneficial for a noninvasive evaluation of patients with suspected takotsubo cardiomyopathy.

Applications of Cardiac Computed Tomography in the Cardio-Oncology Population

At this time, there are limited guidelines for how to best manage this vulnerable cohort. Questions regarding timing of screening, choice of imaging modality and risk reduction strategies—especially in those patients with known atherosclerotic disease—remain to be elucidated. Over a decade of case series, retrospective studies and clinical trials have shed light on the evolving role of Cardiac Computed Tomography (CT) in this population, of which there is a relative paucity of data regarding its potential utility in the specific cardio-oncology population. Focusing on ability of Cardiac CT to evaluate multiple Cardiac and vascular structures, provide diagnostic and prognostic information, as well as assist interventional and surgical colleagues in surgical/percutaneous valve replacement and revascularization strategies is the premise for this review.

BudoffAbstract:Single coronary artery syndrome is usually an asymptomatic condition in most patients; thus most patients are diagnosed when noninvasive imaging (Cardiac Computed Tomography angiography, Cardiac magnetic resonance) or invasive coronary angiography is done for evaluation for other Cardiac conditions. With advances using cardiovascular imaging in the evaluation and management of single coronary artery syndrome; Cardiac Computed Tomography Angiography (CCTA) has emerged as a very essential and leading imaging modality. CCTA has the best resolution (spatial and temporal) among the imaging modalities available for analyzing the cardiovascular system for the diagnosis and management of single coronary artery syndrome. We are presenting cases of patients with single coronary artery syndrome with 3 different courses (anterior, septal, inter-arterial), illustrating how CCTA gives a detailed anatomic/structural evaluation of the origin and course of the coronary arteries, and other Cardiac structures. Thus in patients diagnosed with single coronary artery syndrome, CCTA can provide vital information on the not only the course, but the associated narrowing of coronary vessels. © 2011 Wiley-Liss, Inc.

BudoffAbstract:Introduction: Coronary Arteriovenous Malformations (AVM) can lead to various cardiovascular events. The severity of clinical manifestation depends on the degree of the malformation. The significance of major arterial shunt is that they can deprive the myocardium of the necessary amount of blood, leading to myocardial ischemia, and potentially tachycardia, left ventricular dilatation and heart failure secondary to increased stroke volume and Cardiac output. Cardiac Computed Tomography (CCT) has proven to be a good non-invasive diagnostic tool in the detection of coronary Arteriovenous Malformations. Case Presentation: We include 4 patients, who presented with various symptoms in relation to the coronary Arteriovenous Malformations. Diagnostic confirmation was made non-invasively using multi-row detector Cardiac Computed Tomography (MDCT). Conclusion: Coronary Arteriovenous Malformation is a rare congenital disease which can produce symptoms of varying severity depending on the size of the malformation. CCT angiography can help in non-invasively diagnosing the malformation along with revealing the anatomic details, which can be used for surgical planning, or for better medical management of the patients.
