    Abstract:

    Patients with COPD need to cope with a disabling disease, which leads to Health status impairment. To investigate the long term change of Health status in subjects with mild to moderate airflow obstruction and to compare this to subjects without airflow obstruction, with and without a smoking history. Second, to investigate the factors potentially associated to rapid Health status decline in our total cohort. Two hundred and one subjects were included. Generic [Short form 36 Health survey (SF36) and EuroQol - 5 dimensions (EQ-5D)] and disease specific [Clinical COPD questionnaire (CCQ) and COPD Assessment Test (Cat)] Health status questionnaires were regularly repeated over a six years period. Other functional outcomes comprised measures of lung function, physical fitness, physical activity and emotional state. On average, Health status decline did not differ between groups with the exception of the EQ-5D index, which deteriorated faster in subjects with airflow obstruction compared to the never smoking control group [− 0.018(0.008) versus 0.00006(0.003), p = 0.03]. Subjects presenting at least one exacerbation had faster rate of deterioration measured with Cat [0.91(0.21) versus − 0.26(0.25), p

    Abstract:

    Background Health Related Quality of Life (HRQoL) is a relevant variable in the evaluation of Health outcomes. Questionnaires based on Classical Test Theory typically require a large number of items to evaluate HRQoL. Computer Adaptive Testing (Cat) can be used to reduce tests length while maintaining and, in some cases, improving accuracy. This study aimed at validating a Cat based on Item Response Theory (IRT) for evaluation of generic HRQoL: the Cat-Health instrument. Methods Cross-sectional study of subjects aged over 18 attending Primary Care Centres for any reason. Cat-Health was administered along with the SF-12 Health Survey. Age, gender and a checklist of chronic conditions were also collected. Cat-Health was evaluated considering: 1) feasibility: completion time and test length; 2) content range coverage, Item Exposure Rate (IER) and test precision; and 3) construct validity: differences in the Cat-Health scores according to clinical variables and correlations between both questionnaires. Results 396 subjects answered Cat-Health and SF-12, 67.2% females, mean age (SD) 48.6 (17.7) years. 36.9% did not report any chronic condition. Median completion time for Cat-Health was 81 seconds (IQ range = 59-118) and it increased with age (p < 0.001). The median number of items administered was 8 (IQ range = 6-10). Neither ceiling nor floor effects were found for the score. None of the items in the pool had an IER of 100% and it was over 5% for 27.1% of the items. Test Information Function (TIF) peaked between levels -1 and 0 of HRQoL. Statistically significant differences were observed in the Cat-Health scores according to the number and type of conditions. Conclusions Although domain-specific Cats exist for various areas of HRQoL, Cat-Health is one of the first IRT-based Cats designed to evaluate generic HRQoL and it has proven feasible, valid and efficient, when administered to a broad sample of individuals attending primary care settings.

    Resumen Fundamento y objetivo Desarrollar un test adaptativo informatizado (TAI) para la medicion generica del estado de salud percibido por el paciente. Material y metodo A partir de cuestionarios de calidad de vida relacionada con la salud (CVRS) validados en Espana, un panel de expertos elaboro un banco de items con respuestas tipo Likert de 5 alternativas, que se aplico a una muestra piloto y posteriormente a otra muestra general de pacientes en 7 centros de salud de Asturias. A la muestra general se le ofrecieron 2 modalidades de respuesta: a) en papel; b) en pantalla tactil. Se evaluo el cumplimiento de los supuestos requeridos por los modelos psicometricos de la teoria de la respuesta al item (TRI) (discriminacion, fiabilidad y validez) mediante analisis factorial y otros metodos. Se analizo la funcion de informacion y se realizo un estudio de simulacion. Resultados Se creo un banco de 140 items. Comprobado el cumplimiento de los supuestos de la TRI mediante analisis factorial, se realizo una seleccion de items mediante criterio estadistico y aplicacion de la TRI: se eliminaron 35, lo que dio como resultado un test de 105 items. Al aplicarlos a la muestra general, se estimaron nuevamente los parametros de los items, se comprobo el supuesto de invariancia de los parametros y se eliminaron 9 items mas; esto dio como resultado una version definitiva de 96 items. Conclusiones El TAI desarrollado resulta apto para evaluar genericamente y de forma rapida y fiable la CVRS. Este test supondra un avance muy importante para la medicion de los resultados reportados por los pacientes en la practica clinica.

    Background Feline vector-borne pathogens (FeVBPs) have been increasingly investigated for their impact on Cat Health and their zoonotic potential. The aim of the present study was to assess the prevalence of FeVBPs and haemoplasmas in Cats across Italy and to identify potential risk factors linked to their occurrence. Methods Blood samples from 958 owned Cats living in the North ( n  = 556), Centre ( n  = 173) and South ( n  = 229) of Italy were tested for Babesia spp., Hepatozoon spp., Ehrlichia spp., Anaplasma spp. and filarioids by conventional PCR (cPCR) and for haemoplasmas and Bartonella spp. by SYBR green real-time PCR. Cats included in the study represent a sub-sample from a larger number of animals enrolled in a previous study, which were selected based on the geographical origin. Data on Cats’ positivity for Leishmania infantum , feline leukaemia virus (FeLV) and for feline immunodeficiency virus (FIV), available from the previous study, were included and examined. Potential risk factors for pathogen infection were assessed in relationship to Categorical variables including sex, geographical origin, breed, neutering status and age of Cats. Results Out of the 958 Cats, 194 (20.2%) were positive for at least one of the tested pathogens, 89 (16%) from the North, 32 (18.5%) from the Centre and 73 (31.9%) from the South of Italy. A high prevalence of FeVBPs was detected in male Cats ( n  = 125, 27.8%), living in the southern part of the country ( n  = 73, 31.9%), younger than 18 months of age ( n  = 24, 22.4%) and not neutered (n = 39; 27.5%). In particular, 24 Cats (2.5%) tested PCR-positive for Bartonella spp., of which 1.6% for B. henselae and 0.9% for B. clarridgeiae . A total of 111 Cats scored PCR-positive for haemoplasmas (11.6%), specifically “ Candidatus Mycoplasma haemominutum” ( n  = 95, 9.9%), M. haemofelis ( n  = 14, 1.5%) and “ Candidatus Mycoplasma turicensis” ( n  = 2, 0.2%). Moreover, 39, 31 and 8 Cats were positive for FeLV (4.1%), L. infantum (3.2%) and FIV (0.8%), respectively. Co-infections were registered for 19 (9.8%) Cats. Conclusions These results confirm the occurrence of haemoplasmas and FeVBPs throughout Italy. Preventive measures to protect both animal and human Health should be carried out also for owned Cats, even if no Health status of animals has been assessed in this study.

    There has been little research into brachycephalism and associated disorders in Cats. A questionnaire aimed at Cat owners was used to determine the relationship between feline facial conformation and owner-reported Cat management requirements and respiratory abnormalities. Owner-submitted photographs of Cats were used to develop novel measures of skull conformation. One thousand valid questionnaires were received. Within these there were 373 valid photographs that allowed measurement of muzzle ratio (M%) and 494 that allowed nose position ratio (NP%). The data included 239 Cats for which both measurements were available. Owners reported lifestyle factors (e.g. feeding type, grooming routine, activity level), physical characteristics (e.g. hair length) and other Health characteristics of their Cat (e.g. tear staining, body condition score). A composite respiratory score (RS) was calculated for each Cat using their owner’s assessment of respiratory noise whilst their Cat was asleep and then breathing difficulty following activity. Multivariate analyses were carried out using linear models to explore the relationship between RS and facial conformation, and lifestyle risk factors. The results showed that reductions in NP% and M% were significantly associated with RS (P < 0.001 and P = 0.026, respectively) and that the relationship was significantly negatively correlated (r = -0.56, P < 0.001 for both). Respiratory score was also significantly associated with increased presence of tear staining (P < 0.001) and a sedentary lifestyle (P = 0.01). This study improves current knowledge concerning Cats with breeding-related alterations in skull confirmation and indiCates that brachycephalism may have negative respiratory impliCations for Cat Health and welfare, as has been previously shown in dogs.
