    Objective: To examine the reliability of scores obtained from a proposed critical appraisal tool (Cat). Study Design and Setting: Based on a random sample of 24 health-related research papers, the scores from the proposed Cat were examined using intraclass correlation coefficients (ICCs), generalizability theory, and participants’ feedback. Results: The ICC for all research papers was 0.83 (consistency) and 0.74 (absolute agreement) for four participants. For individual research designs, the highest ICC (consistency) was for qualitative research (0.91) and the lowest was for descriptive, exploratory and observational research (0.64). The G Study showed a moderate research design effect (32%) for scores averaged across all papers.     Objectives To evaluate critical appraisal tools (Cats) that have been through a peer-reviewed development process with the aim of analyzing well-designed, documented, and researched Cats that could be used to develop a comprehensive Cat. Study Design and Setting A critical review of the development of Cats was undertaken. Results Of the 44 Cats reviewed, 25 (57%) were applicable to more than one research design, 11 (25%) to true experimental studies, and the remaining 8 (18%) to individual research designs. Comprehensive explanation of how a Cat was developed and guidelines to use the Cat were available in five (11%) instances. There was no validation process reported in 11 Cats (25%) and 33 Cats (77%) had not been reliability tested. The questions and statements that made up each Cat were coded into 8 Categories and 22 items such that each item was distinct from every other. Conclusions Cats are being developed while ignoring basic research techniques, the evidence available for design, and comprehensive validation and reliability testing. The basic structure for a comprehensive Cat is suggested that requires further Study to verify its overall usefulness. Meanwhile, users of Cats should be careful about which Cat they use and how they use it.

    Purpose Recently, we developed a computer-adaptive test (Cat) for assessing health-related quality of life (HRQoL) in children and adolescents: the Kids-Cat. It measures five generic HRQoL dimensions. The aims of this article were (1) to present the Study design and (2) to investigate its psychometric properties in a clinical setting. Methods The Kids-Cat Study is a longitudinal prospective Study with eight measurements over one year at two University Medical Centers in Germany. For validating the Kids-Cat, 270 consecutive 7- to 17-year-old patients with asthma ( n  = 52), diabetes ( n  = 182) or juvenile arthritis ( n  = 36) answered well-established HRQoL instruments (Pediatric Quality of Life Inventory™ (PedsQL), KIDSCREEN-27) and scales measuring related constructs (e.g., social support, self-efficacy). Measurement precision, test–retest reliability, convergent and discriminant validity were investigated. Results The mean standard error of measurement ranged between .38 and .49 for the five dimensions, which equals a reliability between .86 and .76, respectively. The Kids-Cat measured most reliably in the lower HRQoL range. Convergent validity was supported by moderate to high correlations of the Kids-Cat dimensions with corresponding PedsQL dimensions ranging between .52 and .72. A lower correlation was found between the social dimensions of both instruments. Discriminant validity was confirmed by lower correlations with non-corresponding subscales of the PedsQL. Conclusions The Kids-Cat measures pediatric HRQoL reliably, particularly in lower areas of HRQoL. Its test–retest reliability should be re-investigated in future studies. The validity of the instrument was demonstrated. Overall, results suggest that the Kids-Cat is a promising candidate for detecting psychosocial needs in chronically ill children.

    Two single-site, laboratory, negatively controlled, masked, randomised dose confirmation studies were performed: one in dogs, the other in Cats. After a period of acclimatisation, both the dogs and Cats were orally infected with Echinococcus multilocularis protoscoleces. In the dog Study, 10 dogs received a single dose of Milpro® tablets at a minimum dose of 0.5 mg/kg milbemycin oxime and 5 mg/kg praziquantel 18 days post-infection and 10 dogs received no treatment. In the Cat Study, 10 Cats received a single dose of Milpro® tablets at a minimum dose of 2 mg/kg milbemycin oxime and 5 mg/kg praziquantel 7 days post-infection, 10 Cats received a single dose of the treatment 18 days post-infection and 10 Cats remained untreated. In both studies, intestinal worm counts were performed 23 days post-infection at necropsy. No worms were retrieved from any of the 30 treated animals. Nine of 10 control dogs had multiple worms (geometric mean 91, arithmetic mean 304) and all 10 control Cats had multiple worms (geometric mean 216, arithmetic mean 481). The difference in worm counts between all three treated groups and their controls was highly significant (ANOVA p values of log transformed data

    Venous thromboembolism (VTE) is a common compliCation in newly diagnosed symptomatic multiple myeloma (NDMM) patients. We explored cellular and plasma hypercoagulability in NDMM patients to identify relevant biomarkers that can be used in combination with clinical factors in the development of a risk assessment model (RAM) for VTE. Untreated patients ( n  = 144) with NDMM were prospectively enrolled, baseline biomarkers prior to anti-myeloma treatment and thromboprophylaxis initiation were obtained. These were compared against values in a group of healthy individuals with similar age and sex distribution. The primary Study end point was symptomatic VTE occurrence. At 12-month follow-up cumulative VTE rate was 10.4%. NDMM patients showed biological signs of cellular and plasma hypercoagulability and endothelial cell activation. Procoagulant phospholipid clotting time (Procoagulant-PPL) was shorter, P-selectin levels lower and thrombin generation attenuated overall compared to healthy subjects. Longer Procoag-PPL^®, lower endogenous thrombin potential (ETP), and higher levels of tissue factor pathway inhibitor (TFPI) were associated with VTE occurrence. Multivariate analysis showed that Procoag-PPL^® and ETP were independent risk factors for VTE. We conclude that Procoag-PPL^® and ETP can be prospectively incorporated into a RAM for VTE in MM in combination with clinical and disease risk factors.

    BACKGROUND: The aim of this prospective Study was to identify the most clinically relevant hypercoagulability biomarkers in lung adenocarcinoma patients for elaboration of an improved risk assessment model (RAM) for venous thromboembolism (VTE). SUBJECTS, MATERIALS, AND METHODS: One hundred fifty ambulatory patients with lung adenocarcinoma were prospectively enrolled. Thrombin generation, procoagulant phospholipid-dependent clotting time (Procoag-PPL), tissue factor activity (TFa), factor VIIa (FVIIa), factor V (FV), antithrombin, D-Dimers, P-selectin, and heparanase levels were assessed in platelet-poor plasma at inclusion (baseline) and at the end of the third chemotherapy cycle (third chemotherapy). Cox regression analysis was used to identify independent VTE predictors. RESULTS: At baseline, patients had significantly attenuated thrombin generation, shorter Procoag-PPL, higher levels of TFa, D-Dimers, and heparanase, and lower levels of FVIIa and P-selectin, compared with controls. A significant increase in Procoag-PPL, FV, and FVIIa and a decrease of P-selectin levels were observed between baseline and third chemotherapy. Hospitalization within the last 3 months prior to assessment, time since cancer diagnosis less than 6 months, mean rate index (MRI) of thrombin generation, and Procoag-PPL were independently associated with symptomatic VTE. Accordingly, a prediction model including Procoag-PPL and MRI showed significant discriminating capacity (area under the curve: 0.84). CONCLUSION: Ambulatory patients with lung adenocarcinoma may display pronounced blood hypercoagulability due to decreased Procoag-PPL, increased endothelial cell activation, and increased degradation of fibrin. Incorporation of Procoag-PPL and MRI of thrombin generation may improve the accuracy of a VTE-RAM in the above setting. IMPLICatIONS FOR PRACTICE:The prospective ROADMAP-Cat Study identified two biomarkers of hypercoagulability, the procoagulant phospholipid-dependent clotting time (Procoag-PPL) and the mean rate index (MRI) of the propagation phase of thrombin generation assessed with the Calibrated Automated Thrombinoscope, as being clinically relevant for the classifiCation of ambulatory patients with lung adenocarcinoma receiving a maximum of one cycle of chemotherapy into high and intermediate/low risk for venous thromboembolism. Measurement of Procoag-PPL and MRI within 1 month after the administration of the first chemotherapy cycle provides significant accuracy of the assessment. Association of the Procoag-PPL and MRI with the clinical risk assessment model for cancer-associated thrombosis in ambulatory patients with solid tumors (COMPASS-Cat RAM) further improved its accuracy.

    Background The combination of milbemycin oxime–praziquantel is widely used against the most common tapeworms and roundworms affecting Cats. New veterinary presentations of this combination have recently been approved. Objective The objective of this Study was to compare the palatability of two products using this combination, Milpro ® and Milbemax ® . Methods In all, 20 adult Cats and 20 kittens were offered each product according to a randomisation table using a blinded crossover design. Prehension from the bowl, prehension from the hand and total consumption were assessed. Results Both presentations were very well tolerated in adult Cats and kittens. Total prehension in adult Cats and kittens was 100 and 45 per cent, respectively, for Milpro ® , and 95 and 30 per cent, respectively, for Milbemax ® . The percentages of adult Cats and kittens which swallowed the pill after taking it into their mouth (total spontaneous consumption) were respectively 40 and 45 per cent for Milpro ® , and 35 and 20 per cent for Milbemax ® . Conclusion In this Study, both presentations were highly attractive to Cats and their respective coatings successfully covered the unpleasant odour of praziquantel, which usually repels Cats. These results indiCate that the palatability of Milpro ® is at least as good as Milbemax ® and both tablets are well accepted by adult Cats and kittens.
