Between May 1998 and December 2000, 257 patients (236 males, 21 females), aged 37–80, with histologically confirmed laryngeal cancer, as well as 769 population control persons (702 males, 67 females), were included (1:3 frequency matched by age and sex). History of occupational exposures, as well as other risk factors (tobacco, alcohol), was obtained with face-to-face interviews using a detailed standardized questionnaire. The complete individual work history was assessed. A detailed assessment of work conditions was obtained by job-specific questionnaires (JSQs) for selected jobs known to be associated with exposure to potential laryngeal carcinogens. Estimates for total exposure hours by substance were calculated based on JSQs. Published occupational hygiene data were used to infer semiquantitative scores of exposure intensity for specific job tasks. After adjustment for tobacco and alcohol intake, a significant elevated odds ratios (OR) could be demonstrated for persons that were exposed to Cement during their work as building and construction workers. An OR of 2.42 was calculated for workers of the high exposed subgroup (95% confidence interval: 1.14-5.15; p < 0.001). Smoking was the main confounding factor because the unadjusted Cement OR of 3.20 dropped down to 2.42 after adjustment for tobacco intake. We conclude that there is good evidence for Cement Dust exposure acting as a tobacco, alcohol and asbestos independent risk factor for laryngeal carcinoma. The underlying mechanisms, however, are poorly understood. To examine the effect of Cement, nine blinded Cement-related particulates (<10 μm) were assessed with regard to their induction of the proinflammatory cytokines IL-6 and IL-8 in human primary epithelial cells (pEC) from oropharyngeal mucosa as well as from nonsmall-cell lung carcinoma (non-SCLC) cells A549. It was demonstrated that the Cement specimens did not act cytotoxic as assessed by the lactate dehydrogenase (LDH) assay. The basal and IL-1β-induced IL-8 expression was suppressed, in contrast to an unchanged IL-6. At the transcript level the basal and induced IL-6 and IL-8 gene expression was not influenced by Cement Dust. To discover the mechanism by which Cement influenced the IL-8 expression the following experiments were performed. Submerse exposure experiments have shown that the release of IL-8 was suppressed by Cement Dust. Furthermore, the incubation of IL-8 with Cement-related specimens under cell-free condition led to a loss of immunoreactive IL-8. An immunological masking of IL-8 by free soluble components of respiratory epithelial cells was excluded. Thus, the decrease of IL-8 protein content after Cement exposure seems to be a result of the adsorption of IL-8 protein to Cement particles and the inhibition of IL-8 release. In conclusion, due to absent cytotoxic and inflammatory effects of Cement-related specimens in both human pEC and A549 cell models it remains open how Cement exposure may lead to the respiratory adverse effects in humans.

Deutsche Medizinische Wochenschrift, 2008Co-Authors: H Maier, A Dietz, D Zielinski, K H Junemann, W D HellerAbstract:: This case-control-study, carried out in two University clinics, comprised 200 men (mean age 57.5 +/- 10.8 [range 33-89] years) with squamous epithelioma of the larynx (44.5%), oral cavity (23.5%), oropharynx (24%) or hypopharynx (8%) and 800 controls. Enquiries were directed at social status, life style and occupational exposure to substances such as asbestos, solvents, wood Dust and Cement. The peak incidence of these cancers was from 50 to 60 years of age. The proportion of unmarried or divorced men among the cancer patients was more than twice as high as in the controls (25.8% vs 11.8%; P less than 0.001). The proportion of cancer patients who had completed technical college or university education was significantly lower than in the controls (9.6% vs 24.4%; P less than 0.001). Tobacco and alcohol consumption by the cancer patients was roughly twice as great as in the controls: the cancer patients admitted to an average cigarette consumption of 43.2 +/- 27.9 pack years as compared with 20.1 +/- 26.7 pack years for the controls (P less than 0.001), and an alcohol intake of 69.2 +/- 58.1 g/d as compared with 29.8 +/- 27.5 g/d for the controls (P less than 0.001). Both these factors--drinking alcohol and smoking cigarettes--acted independently of one another to raise the relative risk of squamous epithelioma of the upper respiratory or digestive tract. The effects of alcohol and tobacco on cancer risk were multiplicative rather than merely additive. Enquiries into diet failed to reveal any clear differences tending to incriminate any particular food. Long-term exposure to Cement Dust was linked with an increased risk of cancer.
