    Abstract:

    One hundred twelve consecutive primary total hip replacements were performed for osteoarthritis or osteonecrosis by a single surgeon using the Mittelmeier Ceramic Prosthesis without cement between January 1983 and July 1984. Ninety-three of these hips were followed prospectively to a minimum of 60 months and a median of 108 months. Five (5.4%) hips have been revised for loosening. Of the remaining hips, 77 (87.5%) were rated clinically satisfactory. Cumulative survival probability using revision as endpoint was estimated to be 92.7% at 5 years and 87.9% at 10 years. However, overall mechanical failure rates were 21.5% for the cup and 22.6% for the stem. There was no case of either femoral or acetabular osteolysis observed.

Methods Fifty patients were prospectively enrolled and randomized for Posterior or Heuter procedures. Informed patient consent and local ethics approval was obtained. All patients received an uncemented, Ceramic-on-Ceramic Prosthesis performed by a single surgeon. Functional outcome was assessed by time to achieve milestones of walking, stair climbing, hip movement and balance. Kinematic data on level-ground walking and the effect of fatigue was assessed using a portable gait analysis system at 6-, 12-, and 24-weeks post-operatively. Results The Heuter group showed significantly quicker balance control, on average 10 days earlier, than the posterior group (p 0.05). Kinematic data suggested that there were no statistical differences in stance phase stability (p=0.73) or pulling acceleration (p=0.77) between the 2 groups at each time point. Fatigue, effected via timed, gentle mobilisation on a treadmill, also did not significantly result in a difference. Discussion The Heuter approach is rarely used in the UK, although its recent resurgence can be attributed to its perceived theoretical benefit of sparing muscle; this contrasts with the detachment of muscle required for the Posterior approach. Our experience show that the Heuter approach does not compromise the quality of gait, and it offers advantages in the rate of functional recovery, especially that of balance control.

    The development of metallosis as a complication following rupture of a hip replacement is known to occur as a result of contact with metal components of the Prosthesis (1).In such cases, high cobalt (Co), chromium (Cr) and molybdenum (Mo) levels in the blood have been reported by several Authors (2).Recently, it has been stressed that the clinical investigation should focus on general reactions to high circulating metal levels, such as toxicity for the central nervous system (CNS) and the immune system (3).Despite the increasing interest of literature in Ceramic-on-Ceramic hip arthroplasty (4),little is known about these complications, and in particular of metallosis. To our knowledge this is the first description of a condition of extensive metallosis and radiographic signs presenting as a result of wear of a Ceramic-on-Ceramic Prosthesis.

    The aim of this study was to evaluate the effect of the Prosthesis material (metal–acrylic resin or metal–Ceramic) on loosening torque of the prosthetic screws in an implant-supported mandibular denture under two levels of vertical misfit. Ten frameworks were fabricated with commercially pure titanium, and five of them received acrylic resin and acrylic artificial teeth as veneering material and the other five were veneered with porcelain. Two levels of vertical fit were also created by fabricating 20 cast models to obtain four experimental groups according to the Prosthesis material and misfit: Group 1 (metal–acrylic resin Prosthesis with a passive fit); Group 2 (metal–acrylic resin Prosthesis with a non-passive fit); Group 3 (metal–Ceramic Prosthesis with a passive fit); and Group 4 (metal–Ceramic Prosthesis with a non-passive fit). Two hundred prosthetic titanium-alloy screws were divided in 40 sets (five screws per set, n=10). After 24 h, the loosening torque of the screws was evaluated using a digital torque meter. The results were submitted to two-way ANOVA analysis of variance followed by a Tukey's test (α=0.05). The mean values and standard deviations for each group were G1=7.05 (1.64), G2=5.52 (0.90), G3=6.46 (1.34), and G4=4.35 (0.99). Overall, the Prosthesis material and misfit factors showed a statistically significant influence on the loosening torque (p<0.05). Metal–Ceramic Prosthesis and misfits decreased the loosening of the torque of the prosthetic screws.
