    Chemical peeling, or Chemexfoliation, has been used for centuries to improve signs of ultraviolet light–induced sun damage. Over the last 30 years, the science behind chemical peeling has evolved, increasing our understanding of the role of peeling ingredients and treatment indications. The depth of peels is directly related to improved results and to the number of complications that can occur. Key principles for superficial and medium depth peeling are discussed, as well as appropriate indications for these treatments.

  • Formulation of trichloroacetic acid peeling solution: a bibliometric analysis.

    Since the beginning of this century, trichloroacetic acid solutions of various concentrations have been used for chemical exfoliation. These solutions have been prepared by using four different formulas. To prepare a 50% solution, for instance, water may be added to 50 g of trichloroacetic acid crystals until 100 ml of solution is obtained (weight-to-volume solution). Alternatively, 50 g of water may be added to 50 g of trichloroacetic acid crystals (weight-to-weight solution), or 50 g of trichloroacetic acid crystals may be solved in 100 ml of water (weight-plus-volume solution). Finally, a saturated trichloroacetic acid solution (or "100% solution") may be diluted by an equal volume of water (dilution). Depending on the method used, these so-called 50% solutions contain 40 to 71 weight-to-volume percentages of trichloroacetic acid. From a review of 120 publications on trichloroacetic acid peeling that have appeared since 1926, it was concluded that the authors of 87 of these publications (73 percent) did not report their formula for the trichloroacetic acid solution. Any one of the four methods was reported to have been used by the 33 authors who did report their formula. Eight of 10 internationally reputed pharmacopeias were found not to include the formula of a trichloroacetic acid solution. Proper evaluation of results and prevention of complications of trichloroacetic acid Chemexfoliation is only feasible if both the concentration and the formula of trichloroacetic acid solution are reported by the author. Practitioners who use a trichloroacetic acid solution need to establish that the concentration of the solution they apply corresponds with that of the solution reported in the literature.

  • The effect of Chemexfoliation and anti-aging regiments on fibroblast expression of IGF-1: A pilot Study

    The normalized IGF-1 levels for subjects 1,2 and 3 at baseline were 0.85, 0.59 and 0.87 respectively. When looking at the antioxidant creams the effect of the weaker antioxidant cream regiment of Effulgere and Effulgere P.M. demonstrated a decrease in IGF-1 levels in subject 1 while that of subjects 2 and 3 were virtually unchanged. The stronger antioxidant cream, Effulgere ultra, showed a decrease in IG F-1 levels of subject 1 while demonstrating a significant increase of IGF-1 levels of subjects 2 and 3. (Tables 1,2 and 3) When looking at the post-auricular sites, medium depth Chemexfoliation via Jessner’s and 35% TCA peel, showed significant decrease in IGF-1 levels for subjects 1 and 3 and a slight decrease in levels for subject 2. The deeper Chemexfoliation utilizing Baker’s solution, showed significant decrease in subjects 1 and 3 while it demonstrated an increase in subject 2. (Tables 1,2 and 3) Profilometry When evaluating the profilometry results the median depth of the wrinkles impression on the silicone cast were evaluated. Evaluating the effects of weaker antioxidant cream showed in subject 1 the median depth changed from 0.52 mm to 0.80 mm while, in subject 2 it changed from 1.15 mm to 0.71 mm and in subject 3 it changed from 0.68 mm to 0.62 mm. Looking at the stronger antioxidant cream, the values in subject 1 changed from 0.57 mm to 0.77 mm, in subject 2 these values changed from 1.07 mm to 0.70 mm and in subject 3 this valued changed from 0.74 mm to 0.50 mm. The specimens from the areas treated with medium depth Chemexfoliation method showed change from 0.50 mm to 135 μ m in subject 1, while in subject 2 it increased from 0.64 mm to 1.05 mm and in subject 3 it increased from 0.87 mm to 1.44 mm. The sites treated with deeper Chemexfoliation method showed a change from 0.89 mm to 0.52 mm in subject 1, change from 0.47 mm to 0.91 mm in subject 2 and a change from 1.0 mm to 0.41 mm. (Figure 1)
