    AbstractPurpose: We study toxicity and efficacy of sequential intravesical therapy with mitomycin C and bacillus Calmette-Guerin (BCG) in patients with intermediate or high risk superficial bladder cancer compared to the use of intravesical mitomycin C alone.Materials and Methods: Patients with intermediate and high risk papillary superficial bladder cancer and carcinoma in situ were randomized after transurethral resection between 4 weekly instillations with 40 mg. mitomycin C followed by 6 weekly instillations with BCG (group 1, 90 patients) or 10 weekly instillations with mitomycin C (group 2, 92 patients).Results: The frequency of bacterial and Chemical Cystitis, and other local side effects was similar in both groups. Allergic reactions, including skin rash, were more frequent in the mitomycin C only group (12 of 92 patients versus 5 of 90, p = 0.08), and other systemic side effects were more frequent in the sequential group (16 of 90 versus 8 of 92, p = 0.07).     Purpose Approximately 30% to 40% patients with a superficial bladder cancer treated with Bacille Calmette-Guerin (BCG) or epirubicin do not respond; of the initial responders, 35% have a relapse within 5 years. We compare the therapeutic efficacy and toxicity of intravescical infusions of gemcitabine (GEM) with mitomycin (MMC) in patients with a recurrent superficial bladder cancer. Patients and Methods Patients with a history of a previously treated, recurrent Ta-T1, G1-G3 bladder transitional cell carcinoma were enrolled in the study. The patients received a 6-week course of GEM infusions or 4-week course of MMC. In both arms, for the initial responders who remained free of recurrences, maintenance therapy consisted of 10 monthly treatments during the first year. Results A total of 120 patients were enrolled and randomly assigned to either the MMC or GEM treatment arm. At the end of the study, 109 patients (55 in MMC and 54 in GEM) were assessable. The median duration of follow-up was 36 months for either arm. In the GEM arm, 39 (72%) of 54 patients remained free of recurrence versus 33 (61%) of 55 in MMC arm. Among patients with recurrences, 10 in the MMC arm and six in the GEM arm also had a progressive disease by stage. The incidence of Chemical Cystitis in the MMC arm was statistically higher than in the GEM arm (P .012). Conclusion This study demonstrates that GEM has better efficacy and lower toxicity than MMC; therefore, GEM appears as a logical candidate for intrabladder therapy in patients with refractory transitional cancer.     Aims The purpose of this study was to investigate the effect of tamoxifen citrate on bladder functions and histology, and also to investigate the role of estrogen receptor β (ER β) in a rat Chemical Cystitis model. Methods The study included 37 female Sprague–Dawley rats. Chemical Cystitis was induced by intravesical instillation of hydrochloric acid in 32 rats, and the treatment group (n = 15) received daily 0.4 mg/kg of tamoxifen citrate with orogastric tube, and the control group (n = 17) received no treatment. The sham group consisted of five rats having no acid instillation and no treatment. Cystometric studies were performed in all rats at the beginning and end of the experiment. The rats were euthanized at 2 months. The bladders were removed and examined histologically for mast cells, inflammatory cells, and ER β. Results The mean maximal bladder volume increased by 73.6% ± 25.2 in the treatment group and decreased by 7.2% ± 10.8 in the control group, revealing a significant difference (P = 0.007). The mean bladder compliance increased by 81.2% ± 25.2 in the treatment group and decreased by 4.8% ± 12.7 in the control group, revealing a significant difference between the two groups (P = 0.005). ER β positive cells were significantly lower in the bladders with chronic Cystitis than in the sham group (P = 0.038). Conclusions Tamoxifen citrate may be an alternative choice, as easy, to other treatment options in the treatment of chronic inflammatory condition to improve deteriorated bladder function.     The purpose of this study was to investigate the effect of botulinum toxin type A on bladder function and histology in a rat Chemical Cystitis model. The study included 41 female Sprague-Dawley rats with Chemical Cystitis induced by intravesical instillation of hydrochloric acid. The acid instillation was repeated monthly to maintain chronic inflammation. The treatment group (n=21) received 2–3 units of botulinum toxin type A injected into the bladder detrusor at the 3, 6, 9 and 12 o'clock positions, and the control group (n=20) underwent saline injection into the bladder detrusor at the same positions. Urodynamic studies were performed in all rats before the treatment and at death. The rats were killed at 1 week, 2 weeks, 1 month and 2 months after treatment. The bladders were removed and examined histologically for mast cells and inflammatory changes. The cystometric findings showed that, at the beginning and end of the experiment, the increases in the maximum bladder capacity and compliance were significantly higher in the treatment group than in the control group (P=0.000 and P=0.025, respectively). The histological studies revealed similar mast cell counts and leukocyte infiltration for the treatment and control groups. In conclusion, in this rat Chemical Cystitis model, botulinum toxin type A injected into the bladder detrusor led to a functional improvement. Thus, botulinum toxin type A injection may be an alternative, minimally invasive choice to other surgical treatment options in the treatment of a chronic inflammatory condition to improve deteriorated bladder function.

    Purpose: We investigated the feasibility of augmentation in a diseased bladder with a bladder acellular matrix graft.Materials and Methods: In 50 female Sprague-Dawley rats Chemical Cystitis was induced by intravesical instillation of HCl repeated monthly to maintain chronic inflammation. Urodynamic studies were performed in all rats 1 week after the induction of Chemical Cystitis and repeated at sacrifice. The 29 rats in the experimental group underwent partial cystectomy (50% or greater), followed by bladder acellular matrix graft augmentation, while the 21 controls underwent monthly HCl instillation only. The rats were sacrificed at 2 weeks, 1, 2 and 3 months, respectively.
