    Abstract:

    Despite being curable reproductive tract infections (RTIs) including sexually transmitted infections continue to be a major health problem in developing countries. The present study was undertaken to know the prevalence of trichomoniasis, vaginal candidiasis, genital herpes, Chlamydiasis, and actinomycosis in rural and urban women of Haryana by using wet mount, PAP smear, and fluorescent microscopic examination. Patients suspected of suffering from bacterial vaginosis were given treatment and were not included in the study. RTIs were seen in 16.6% of urban and 28.7% of rural women. The highest prevalence seen was that of trichomoniasis in both rural (24.2%) and urban (15.7%) women, followed by candidiasis (4.2% in rural and 0.6% in urban women), genital herpes (0.3% in rural and 0.2% in urban women), and Chlamydiasis (0.02% in rural and 0.05% in urban women). Pelvic actinomycosis was seen in 1.4% of rural and 0.06% of urban women using intrauterine contraceptive devices. Mixed infection of Trichomonas vaginalis with Candida spp. was seen in 6.3% of rural women only. It is desirable to have a baseline profile of the prevalence of various agents causing RTIs in a particular geographic area and population which will help in better syndromic management of the patients.

    Objectives The data concerning treatment of such sexually transmitted infection as Chlamydia trachomatis (CTR) among Ukrainian population are still limited and controversial. This information is of great scientific and health care interest. The aim of investigation is to find out efficiency of urogenital chlamydiosis treatment involving new effective ways of treatment and choose the most appropriate and expedient method of treatment, which would completely meet recommendations of WHO. Methods We have analysed literature, records and recommendations concerning treatment of patients with chlamydial infection and concluded that there is no a perfect method of treatment, which would ensure complete clinical and laboratory cure. According to literature data, macrolids are good drugs with insignificant resistance to agent, nevertheless, recovery of a patient is, sometimes, incomplete due to complications. We have examined patients with urogenital chlamydiosis. All patients (n=134) were divided into four groups and treated with antimicrobial drugs jozamicyn and azytromicyn, and immunomodulators - manax and kagotsel. Patients from the 1st group (n=35) were treated with jozamicyn and manax, 2nd group (n=35) with azytromicyn and manax, 3rd group (n=34) with jozamicyn and kagotsel, 4th group (n=30) with azytromicyn and kagotsel. Results Following treatment results of our investigation showed complete laboratory and clinical recovery in 93.2 % of patients from the 1st group, 98.1% of patients from the 2nd group, 91.8% of patients from the 3rd group, and 93.5% of patients from the 4th group. Conclusion According to obtained data, our method of treatment, which was used for treatment of patients from the 2nd group, is the most appropriate for common application in the treatment of urogenital chlamydiosis

    Background Chlamydiosis and Q fever, two zoonosis, are important causes of ruminants' abortion around the world. They are caused respectively by strictly intracellular and Gram negative bacterium Chlamydophila abortus (Cp. abortus ) and Coxiella burnetii (C. burnetii). Chlamydophila pecorum (Cp. pecorum) is commonly isolated from the digestive tract of clinically inconspicuous ruminants but the abortive and zoonotic impact of this bacterium is still unknown because Cp. pecorum is rarely suspected in abortion cases of small ruminants. We have developed a multiplex PCR (m-PCR) for rapid simultaneous differential detection of Cp. abortus , Cp. pecorum and C. burnetii in clinical samples taken from infected animals. Results Specific PCR primers were designed and a sensitive and specific m-PCR was developed to detect simultaneously, in one tube reaction, three specific fragments of 821, 526 and 687-bp long for Cp. abortus, Cp. pecorum and C . burnetii respectively. This m-PCR assay was performed on 253 clinical samples taken from infected ruminant's flocks that have showed problems of abortion diseases. Thus, 67 samples were infected by either one of the three pathogens: 16 (13 vaginal swabs and 3 placentas) were positive for Cp. abortus , 2 were positive for Cp. pecorum (1 vaginal swab and 1 placenta) and 49 samples (33 vaginal swabs, 11 raw milks, 4 faeces and 1 placenta) were positive for C. burnetii . Two vaginal swabs were m-PCR positive of both Cp. abortus and C. burnetii and none of the tested samples was shown to be infected simultaneously with the three pathogens. Conclusion We have successfully developed a rapid multiplex PCR that can detect and differentiate Cp. abortus , Cp. pecorum and C. burnetii; with a good sensitivity and specificity. The diagnosis of chlamydiosis and Q fever may be greatly simplified and performed at low cost. In addition, the improvement in diagnostic techniques will enhance our knowledge regarding the prevalence and the pathogenetic significance of Q fever and chlamydiosis.

    Abstract Chlamydiosis is a significant cause of abortion in goats and sheep in Tunisia. A new commercially available ELISA (ELISA- Chlamydophila abortus , ELISA- C. abortus ) for detecting antibodies against C. abortus was evaluated using field sheep and goat serum samples. This ELISA is based on a recombinant antigen that expresses part of a protein from 80 to 90 kDa family that is specific to C. abortus . Serum samples collected from 219 sheep and 53 goats flock presenting abortion problems were used in this study. To assess the sensitivity of this new serological test, it was compared with the complement fixation test (CFT) using ELISA-antigen (Elisa-ag) detection and/or isolation as reference technique. Only 61.1% of the sera gave concordant results in both ELISA- C. abortus and CFT, and results were non-concordant for 22.4% due to 16.5% of doubtful results on CFT. The concordance of ELISA- C. abortus and Elisa-ag result was 65% while that of the CFT with ELISA-antigen was only 45%. This study using field material demonstrated that ELISA- C. abortus is a suitable method for diagnosis of chlamydiosis in order to implement early prophylactic measures from the first cases of abortion, both to stop the spread of infection and to prevent associated economic loss.

    As chlamydiosis and Q fever can affect ovine and caprine flocks simultaneously or successively, mouse models were used to demonstrate that simultaneous vaccination with the live Chlamydophila abortus 1B vaccine and the killed phase-I-Coxiella vaccine, administrated in two separate injection sites had no effect on their efficacy against subsequent challenges with either C. abortus or Coxiella burnetii.
