Patients and methods This was a retrospective observational case series reviewing clinical data and fundus fluorescein angiography (FFA), swept-source optical coherence tomography (SS-OCT), and SS-OCTA images of patients with CNV and comparing the findings. The swept-source technology enables deeper penetration and superior axial resolution. The incorporated blood flow detection algorithm, optical coherence tomography angiography ratio analysis (OCTARA), enables visualization of CNV in vivo without the need for dye injection. Results The study included 136 eyes of 105 patients. Active lesions on SS-OCTA images showed increased capillary density, extensive arborization, vascular anastomosis and looping, and peri-lesional hollow. Inactive lesions showed decreased capillary density, presence of large linear vessels, and presence of feeder vessels supplying the CNV. We detected positive correlation between SS-OCTA, FFA, and SS-OCT images in 97% of eyes. In the remaining 3%, SS-OCTA confirmed the absence of CNV, whereas FFA and SS-OCT either were inconclusive in the diagnosis of CNV or yielded false-positive results. Conclusion SS-OCT and SS-OCTA represent a reproducible risk-free analog for FFA in imaging CNV. SS-OCTA is particularly versatile in cases where FFA and SS-OCT are inconclusive.

Choroidal Neovascular Membrane associated with optic nerve head drusen in a child

Methods We examined a 9-year-old boy who had suspected papilledema and a retinal mass. He had undergone neuroradiologic imaging at an outside facility. Results Clinical examination of the patient provided the diagnosis of optic nerve head drusen, pseudopapilledema, and a cicatrized Choroidal Neovascular Membrane. Examination of the boy's parents disclosed optic nerve head drusen in the father. Conclusions Choroidal Neovascular Membranes caused by optic nerve head drusen are uncommon in children. Clinical examination of the patient and family members, along with B-scan ultrasonography, can establish this cause. Neuroradiologic testing is unnecessary, and carries risk related to the need for sedation.

Our aim was to determine intra- and inter-rater agreement of inflammatory CNVM manual measurements obtained with OCTA. OCTA (Triton® Topcon Corporation) images in patients with inflammatory CNVM were imported into ImageJ software v1.50 (NIH image). Two experienced observers performed manual area and perimeter measurements independently, and one of the observers performed the same measurements twice. Agreement was evaluated with intraclass correlation coefficients (ICC) and concordance correlation coefficients (CCC). Bland-Altman plots were plotted to graphically assess concordance. Statistical analysis was performed using STATA v13.0. Sixteen eyes of 16 subjects, with a mean age of 39.0 ± 16.6 years (range 13–71), were included. Mean CNVM area and perimeter was 124.83 ± 117.80 and 4.20 ± 2.00 mm, respectively. Intra-rater ICC for both area and perimeter measured was 0.99 (95% confidence interval (CI) 0.99–0.99). Inter-rater ICC for area and perimeter measured was 0.95 (95%CI 0.87–0.98) and 0.81 (95%CI 0.17–0.94), respectively. Intra-rater CCC for both area and perimeter measured was 0.99 (95%CI 0.99–0.99). Inter-rater CCC for both area and perimeter measured was 0.91 (95%CI 0.81–0.99) and 0.66 (95%CI 0.44–0.88), respectively. Inflammatory CNVM manual measurement showed high intra-rater agreement and moderate inter-rater agreement. Repeatability and reproducibility studies are essential in manual analysis to establish thresholds that can distinguish measurements variation from true clinical change. An automatic algorithm may be helpful to accurately grade lesions and monitor disease activity and response to treatment.
