    Abstract:

    TNF-α plays a key role in the inflammatory cytokine cascade involved in the pathogenesis of Chronic Arthritis, including rheumatoid Arthritis, psoriatic Arthritis, and ankylosing spondylitis. Systemic inflammation and the increased production of pro-inflammatory cytokines in these patients may favor the onset of metabolic syndrome. In patients affected by Chronic Arthritis, TNF-α is considered as one of the factors responsible for favouring insulin resistance and dyslipidemia, which are important features of the metabolic syndrome. Even if TNF-α is a single player in the great molecular cauldron of inflammation, the use of TNF-α inhibitors may be an important approach for not only treating articular and cutaneous symptoms but also for ameliorating glucose and lipid metabolism. Nevertheless, further research and clinical trials are needed to better determine the effects of biologic therapies on metabolic components in Chronic Arthritis patients and to identify the most appropriate strategies on the basis of the comorbidities in these patients.

    Various factors determine the outcome of rheumatoid Arthritis and juvenile Chronic Arthritis. One of these factors is the neuro-endocrine system. In rheumatoid Arthritis as well as in juvenile Chronic Arthritis, alterations of the autonomous nervous system do occur. In polyarticular and systemic juvenile Chronic Arthritis, during active disease the sensitivity for catecholamines changes. Under these conditions catecholamines cannot inhibit the immune response, which may have negative consequences for the course of the disease. In certain autoimmune animal models defects in the hypothalamus-pituitary-adrenal axis play a role in the pathogenesis of the disease

    Purpose of Review Current technical advances enable the assessment of the complex changes in body fluid proteomes and thus allow for the discovery of biomarker signatures rather than just following differences of a single marker. In this review, we aim to summarize current approaches to discover and evaluate multi-biomarker panels for improved monitoring of Chronic Arthritis disease activity. Recent Findings Mass spectrometry and affinity proteomic methodologies have been used to identify biomarker panels in synovial fluid, serum, plasma, or urine of pediatric and adult Chronic Arthritis patients. Notably, despite the numerous efforts to develop new and better biomarker panels, very few have undergone extensive analytical and clinical validation and been adopted into routine use for patient benefit. Summary There remains a significant gap between discovery of Chronic Arthritis biomarker signatures and their validation for clinical use.
