    Abstract:

    Objectives To evaluate the efficacy of ultrasound-guided percutaneous Closed Drainage in the management of abscesses in the head and neck. Study design Prospective, controlled study. Methods From January 1998 to December 1999, patients presenting with a unilocular abscess in the head and neck and who subsequently underwent echo-guided percutaneous Drainage were enrolled in the study. Results Fourteen patients were enrolled in the study. No patients needed open surgical Drainage afterward. By our protocol, only 8 patients had an indwelling catheter. After treatment, all patients were cured of their disease completely without any complication. Conclusions Ultrasound-guided percutaneous Closed Drainage is a safe and effective procedure and might be most beneficial in well-defined and/or unilocular abscess in the head and neck. Closed Drainage could be tried initially in an abscess of the deeper visceral spaces and will not impede any subsequent intervention by surgery. Hospital stay can be shortened and potential surgical complications may be avoided by using this procedure.

    Abstract:

    The lithological and palaeobotanical research conducted in a Closed-Drainage depression located in the watershed zone between the Mroga and Mrozyca Rivers (Central Poland) allowed for the reconstruction of its palaeogeography in the period subsequent to the retreat of the last ice sheet (Wartanian Stage of the Odranian Glaciation). The purpose of this article is to gather and synthesise the obtained results, as well as to reconstruct the processes of development and filling in the studied depression over the last 135,000 years. The post-glacial evolution of the relief was recorded in the mineral and biogenic sediments deposited on glacial formations. The modern Closed-Drainage depression is a trace of a fossil extensive glacial meltout depression. The post-glacial development of the land relief within the depression involved three stages, dominated by different types of morphogenesis, i.e. the deglaciation stage during the Wartanian Stage of the Odranian Glaciation, the Eemian Interglacial-Early Vistulian aggradation stage, and the Vistulian aggradation-denudation stage. High thickness of the identified biogenic sediments suggests considerable transformations of the relief of the watershed during the two cycles of temperate climate – the Eemian Interglacial and the Holocene. The thickness of the mineral sediments suggest low dynamics of the environment during the Last Glacial Maximum (LGM), and undermine the hypotheses on the determining effect of periglacial conditions on the character of the relief of Central Poland. The obtained results constitute an inspiration and starting point for another research project concerning the reconstruction of the watersheds in Central Poland.

    Abstract:

    OBJECTIVE: Most symptomatic chronic subdural hematomas are treated by subdural Drainage. However, a subperiostal (i.e., extracranial) passive Closed-Drainage system in combination with double burr hole trepanation is used at our institution. Therefore, we wanted to analyze our results and compare them with the alternate treatment strategies reported in the current literature. METHODS: In a retrospective single-center study, we analyzed the data of all patients undergoing double burr hole trepanation with a subperiostal passive Closed-Drainage system. Data analysis included general patient data, complications, postoperative seizure rate, and outcome. RESULTS: One hundred forty-seven patients underwent surgery for 183 symptomatic chronic subdural hematomas. The perioperative mortality rate was 3.4%. Hematoma persistence or recurrence occurred in 13.1% of the cases. The postoperative seizure rate was 6.6%, and the infection rate was 1.6%, including 3 cases of superficial wound infection and 1 case with deep infection. The reintervention rate was 9.3%, including trepanation in 8.2% of the patients and craniotomy in 1.1%. The overall complication rate was 10.9%. CONCLUSION: Double burr hole trepanation combined with a subperiostal passive Closed-Drainage system is a technically easy, highly effective, safe, and cost-efficient treatment strategy for symptomatic chronic subdural hematomas. The absence of a drain in direct contact with the hematoma capsule may moderate the risk of postoperative seizure and limit the secondary spread of infection to intracranial compartments.
