    Der Sonographie wird in der Untersuchung des Hufgelenks und des Podotrochlearapparats zunehmend Beachtung zuteil. Ziel dieser Arbeit war es daher, Beispiele eines sonographischen Profils des Hufgelenks und seiner erfassbaren assoziierten Strukturen zu erarbeiten, um diese in den Rahmen der Podotrochlose einzuordnen. Vergleiche zu Sektionsbildern werden hinzugezogen, damit Aussagen in Bezug auf Verlässlichkeit und Empfindlichkeit der Methode eine fundierte Basis erhalten. Eingegangen wird auf Anatomie, Artefakte und sonstige Fehlerquellen, um Fehlinterpretationen der erhobenen Befunde zu vermeiden. Die Studie beruht auf zehn physiologischen und sieben pathologischen Referenzaufnahmen aus dem Patientengut der Ecole Nationale Vétérinaire d'Alfort und der veterinärmedizinischen Fakultät der Universität Liège. Ferner wurden transcuneale Aufnahmen des Podotrochlearapparats an einem 18 Monate alten Warmblutfohlen am Fachbereich Veterinärmedizin der Universität Lüttich post mortem durchgeführt. Linear- und Sectorscanner wurden im Real-Time-Motion Verfahren eingesetzt. Die sonographische anatomische Dokumentation zahlreicher Weichteilstrukturen des equinen Hufgelenks und des Podotrochlearapparats werden anhand von Beispielen demonstriert. Mit Ultraschall nachweisbare pathologische Veränderungen zeigen, dass sich die Sonographie als eine wichtige Bereicherung der gängigen bildgebenden Verfahren im Bereich der distalen Pferdegliedmaße erweist.Ultrasound is becoming more common in the examination of the Coffin Joint and podotrochlear apparatus in horses.   • Study of equine articular biomechanics by tribometry and inverse dynamic analysis

In supra-maximal conditions, all of these contraints would be responsible for some disease of equine locomotor systems (Radin et al., 1972 ; Radin, 1983 ; Smith et al., 2002). In order to understand better the constraints in the equine locomotor dynamics, some investigations have been carried out. Moreover, some measurement methods, based on the mechanics of Newton, have been performed. This work consists of three different experimental approaches: The first approach was an ex vivo micro-systemic model of the diarthrodial cartilage-cartilage contact : from osteochondral specimens in contact, dynamic frictional constraints were measured by tribometry using a pin-on-disc tribometer in Coulomb’s regime. The effects of age and load on the frictional response were studied. The second approach was an ex vivo macro-systemic model of the diarthrodial cartilage-cartilage contact : from whole fetlock Joints fixed into an original pendulum tribometer and mobilized, dynamic visco-frictional constraints have been followed in viscous regime. The effect of a lipid solvent on the synovial fluid was studied. The third approach was an in vivo macro-systemic model of the diarthrodial cartilage-cartilage contact : from distal forelimbs, cinematic, cinetic and radiographic data have been collected, then tendon tensions and Joint contact forces components were calculated using an inverse dynamic analysis. From a sensitivity analysis, the effect of raised hell (0°, 6°, 12°) were elucidated. In the first approach : (i) an original equine Joint friction test model using a cartilage-on-cartilage arrangement (pin-on-disc) has been performed, (ii) cartilage ageing was found, under experimental operating conditions, to be responsible for an increase in friction coefficient, (iii) lubrication remained stable when young cartilage was loaded whereas lubrication of older cartilage was affected by increased load. In the second approach : (i) an original equine Joint pendulum tribometer has been designed, (ii) visco-frictionnal data have confirmed the Joint pendulum damping can be modelled like a harmonic oscillator fluid damping, (iii) the lipid solvent injection into the fetlock Joint increased the visco-frictional parameters of the Joint and the damping time decreased, suggesting that the lubrication capacity was compromised. In the third approach : (i) an original equine distal limb model with tendinous variable pulleys was described, (ii) polynomial relationships of second order between the variation of the dorsal angle of the fetlock Joint and the moment arm variation of the deep digital and superficial digital flexors tendons have been established, (iii) the Joint contact force components of the Coffin Joint and of the fetlock Joint have been calculated during the phase of the trot, (iv) the effect of raised heel on the tendinous tensions and on the Joint contact force components was established, suggesting that the use of heel wedges to reduce the loading of the Coffin Joint may in fact increase the loading of the fetlock Joint. The two ex vivo equine articular models (tribometry) are some original and complementary mechanical analysis tools: the pin-on-disc tribometer may highlight the mechanical properties of the triplex cartilage-synovial fluid-cartilage that are related with the structural properties (contact surface), and the pendulum tribometer may clarify the mechanical properties of the articular lubricant in a very realistic intra-articular environment (quasi anaerobic). These two tools could be used to characterize the mechanical properties of some new articular prosthesis and lubricants in the future. Finally, the numerical in vivo model (dynamic inverse analysis), calculating dynamic tendinous tensions and dynamic Joint contact force components in the equine distal forelimb, is equally a very interressing and original tool. Indeed, it measures some articular intrinsic informations, in non-invasive manner (no using of intra-articular and intra-tendinous constraint gaugues) and can be perfectly integrated in a classical equine locomotor analysis

  • Comparison of the success of intra-articular treatment with sodium Hyaluronate and Corticosteroids vs. Autologous conditioned serum in horses with positive anaesthesia of the Coffin Joint

    Bei der vorliegenden Arbeit handelt es sich um eine Feldstudie, in der zwei Behandlungsformen der aseptischen Hufgelenkentzündung beim Pferd miteinander verglichen werden. Es handelt sich zum einen um die Behandlung mit einer hochmolekularem Na-Hyaluronat/Corticosteroid-Kombination (Hylartil®/Celestovet®), zum anderen um die Behandlung mit autologem konditioniertem Serum (IRAP®). Von 54 Pferden, bei denen die Lahmheitsursache im Hufgelenk lag, wurden 27 Pferde mit Hylartil® und Celestovet® (Guppe I) und 27 Pferde mit IRAP® (GruppeII) intraartikulär behandelt. Bei den Pferden der Gruppe I wurde nur die erste Behandlung mit Celestovet® vorgenommen, für die Folgeinjektionen wurde ausschließlich Hylartil® verwendet. Außerdem wurde der Hufbeschlag wenn nötig den orthopädischen Erfordernissen angepasst und den Pferden wurde ein einheitliches Ruhe- bzw. Bewegungsprogramm verordnet. Die Zuordnung der Pferde zu den Behandlungsgruppen erfolgte anhand der Besitzerentscheidung. Dabei zeigten die Pferde beider Gruppen keine signifikanten Unterschiede hinsichtlich der entscheidenden Merkmale Alter, Nutzung und Röntgenklasse. Alle Pferde wurden nach einem einheitlichen Protokoll untersucht, behandelt und nachkontrolliert. Der Beobachtungszeitraum betrug 6 Monate vom Behandlungsbeginn bis zur letzten Nachkontrolle. Unerwünschte Nebenwirkungen traten in keiner der beiden Gruppen auf. Nach 6 Monaten lag der Behandlungserfolg bei den Pferden der Gruppe I bei 63,0% nach durchschnittlich 2,8 Injektionen und der Behandlungserfolg bei den Pferden der Gruppe II lag bei 88,9% nach durchschnittlich 3,3 Injektionen. Damit liegt ein signifikanter Unterschied hinsichtlich des Behandlungserfolges zwischen beiden Behandlungsformen vor. Die Behandlung mit autologem konditioniertem Serum wird nach den Ergebnissen unserer Studie als sinnvolle Alternative zur Hufgelenkbehandlung mit Hyaluronsäure angesehen.In this clinical Study two treatment protocols for intra-articular medical therapy of aseptic arthritis of the Coffin Joint in horses were compared. A group of 54 lame horses which had been diagnosed with Coffin Joint pain based on positive anaesthesia of the Joint were selected. They were divided into two equal groups of 27 horses. One group was treated with sodium hyaluronate and Betamethasone (Group 1), and the second group was treated with autologous conditioned serum (Group 2). In Group 1 sodium hyaluronate was combined with Betamethasone only for the first intraarticular injection. Subsequent injections for this group consisted of sodium hyaluronate alone. The assignment of the horses to the two individual groups was based on owner preference. There was no significant difference in horse age, horse activity, or radiographic classification between the two groups. Neither group had any adverse reactions to the treatment. If deemed necessary, the horses were shod with orthopaedic shoes, and all horses were put in a uniform training program. All the horses were examined, treated and re-evaluated following the same protocol. The observational period for the study was six month after the first injection. There was a statistically significant difference in the treatment outcomes between the two groups. Group 1 had a 63% success rate with an average of 2,8 treatments. Group 2 had a 88,9% success rate with an average of 3,3 treatments

  • Autologous Conditioned Serum (ACS) in the treatment of osteoarthritis of the horse Coffin Joint and complications associated with intra-articular injections

    Autologes Conditioniertes Serum (ACS) wird seit mehreren Jahren zur intraartikulären Behandlung der Osteoarthritis (OA) eingesetzt. Obwohl bisher der Therapieeffekt und das Komplikationsrisiko beim Pferd wenig erforscht sind, erfreut sich ACS bei Tierärzten und Besitzern zunehmender Beliebtheit. Das Ziel dieser Arbeit war es deshalb, einen langfristigen Behandlungseffekt bei Pferden mit OA des Hufgelenks, die ausschließlich mit ACS behandelt wurde, zu untersuchen. Zudem wurden Komplikationshäufigkeit und potentielle Risikofaktoren einer ACS-Behandlung bei einer größeren Anzahl von Pferden, bei denen mindestens ein Gelenk der Vorder- oder Hintergliedmaße mit ACS behandelt wurde, bestimmt. Dafür wurden die Krankenakten von Pferden, die in dem Zeitraum von Januar 2007 bis Juni 2010 eine intraartikuläre ACS-Injektion erhielten, retrospektiv ausgewertet. Die Diagnose OA des Hufgelenks wurde gestellt wenn, die Hufgelenksanästhesie die Lahmheit um mindestens 50 % reduzierte oder Leitungsanästhesien die Lahmheit auf den Hufbereich begrenzten und OA durch Bildgebung eindeutig bestätigt wurde. Dafür wurden die Röntgen- und MRT-Aufnahmen erneut beurteilt. Der Langzeiteffekt wurde durch telefonische Besitzerbefragung ermittelt. Mindestens zwei Jahre nach der ACS-Behandlung der OA des Hufgelenkes konnten acht der 26 Pferde (31 %) auf ihrem ursprünglichen oder einem besseren und vier (15 %) auf einem niedrigerem Leistungsniveau eingesetzt werden. Kein langfristiger Behandlungserfolg trat bei 14 Pferden (54 %) ein. Ein Zusammenhang zwischen Schweregrad der OA und Therapieerfolg konnte nicht nachgewiesen werden. Für die Bestimmung der Komplikationshäufigkeit und potentieller Risikofaktoren standen die Krankenakten von 387 Pferden mit insgesamt 1.445 Gelenksinjektionen zur Verfügung. Als potentielle Risikofaktoren galten diagnostische Gelenksinjektionen bis zu drei Tage vor der ACS-Injektion, Medikamente oder Antibiotika, die systemisch oder intraartikulär gleichzeitig mit ACS verabreicht wurden, und Arthroskopien bzw. Gelenkspülungen, die in einem zeitlichem Abstand von bis zu 47 Tagen vor der intraartikulären ACS-Injektion durchgeführt wurden. Mithilfe der adjustierten Waldmethode wurde die Komplikationshäufigkeit nach einer intraartikulären ACS-Injektion von 4,8 pro 1.000 Injektionen (95 % KI 1,7-9,3) bestimmt. Risikofaktoren, die die Entstehung einer Komplikation beeinflussten, konnten nicht ermittelt werden. Zur Absicherung dieser Komplikationshäufigkeit gegenüber der allgemeinen Komplikationsrate von 0,14 % müssen mindestens 5.900 Probanden pro Gruppe in einer prospektiven Studie behandelt werden. Schwerwiegende oder lebensbedrohliche Komplikationen wurden nicht beobachtet. Diese Arbeit zeigt erstmalig auf, dass nach mindestens zwei Jahren weniger als ein Drittel der mit ACS behandelten Pferde mit einer OA des Hufgelenks auf ihrem ursprünglichem oder einem besseren Leistungsniveau geritten werden können. Das Risiko einer Komplikation bei intraartikulären Injektionen mit ACS ist gering.Autologous Conditioned Serum (ACS) has been used for the intra-articular treatment of osteoarthritis (OA) for several years. Although the therapeutic effect and the risk of complications are not well documented in horses, ACS enjoys increasing popularity among veterinarians and horse owners. The aim of this study was therefore to investigate the long-term outcome in horses with OA of the Coffin Joint, when treated exclusively with ACS. In addition, the frequency of complications and potential risk factors of ACS treatment were determined in a larger number of horses in which at least one Joint of the fore- or hindlimb was treated with ACS. For this purpose, medical records of horses who received an intra-articular ACS injection in the period from January 2007 to June 2010 were evaluated retrospectively. Diagnosis of OA of the Coffin Joint was based on a positive response to intra-articular analgesia or a positive nerve block and radiological or magnetic resonance evidence of OA of the Coffin Joint. The radiographs and MRI images were re-evaluated. The long-term outcome was determined through telephone interviews with the owners. At least two years after the ACS treatment of OA of the Coffin Joint, eight out of the 26 horses (31 %) could be ridden at a level equal to or greater than their previous performance level and four (15 %) on a lower performance level. No long-term treatment success occurred in the case of 14 horses (54 %). There was no significant correlation between the severity level of OA and therapy outcome. For the determination of the frequency of complications and potential risk factors, the medical records of 387 horses with a total number of 1,445 Joint injections were available. Potential risk factors were diagnostic Joint injections up to three days prior to ACS injection, drugs or antibiotics administered systemically or intra-articularly at the same time as ACS, and arthroscopies and Joint lavages, respectively, at a time interval of up to 47 days before intra-articular ACS injection. Using the adjusted Wald method, the complication rate after an intra-articular ACS injection was determined to be 4.8 per 1,000 injections (95 % CI 1.7-9.3). Risk factors that influenced the development of a complication could not be identified. In order to ensure this complication rate against the general complication rate of 0.14 %, at least 5,900 subjects per group would have to be treated in a prospective study. Serious or life-threatening complications were not observed. This study demonstrates for the first time that less than a third of horses whose Coffin Joints were treated with ACS can be ridden on their original or better performance level after at least two years.     Abstract:

    The aim of the present study was to evaluate the intra-articular pressure in horses with chronic distal forelimb lameness. Fifty-six distal interphalangeal (DIP) Joints of the forelimbs of 36 horses were clinically and radiographically investigated in addition to measuring intra-articular pressure. Forty Coffin Joints of sound horses were used as controls. The peak pressure was registered for each articulation. The mean and Standard deviation pressure measured in sound horses bearing weight on both limbs was 36.8±16.0 mm Hg and 69.6±23.7 mm Hg when the contra-lateral was raised. In diseased horses the mean and standard deviation pressure measured in the two positions was 66.5±7.6 mmHg and 132.8±43.3 mm Hg respectively. These results indicate that pressure values of the Coffin Joint are higher in horses with a painful condition of the distal portion of the forefoot in comparison to normal horses. No correlation was found between articular pressure and the pattern of radiographical changes of the DIP Joint or distal sesamoid bone. No weight, breed or attitude differences were detected among sound horses and no complications have been reported after pressure measurements. Pressure measurement may be considered as a useful aid to localize pathologic conditions involving the DIP Joint or the navicular bone
