    Background and Objectives Scalp psoriasis is a frustrating disorder whose treatments can all too often be cumbersome and ineffective. Our objective was to test a Combination Device involving the 308-nm excimer laser in tandem with a hair blower for treating scalp psoriasis. Study Design/Materials and Methods Adult subjects with scalp psoriasis unresponsive to class I topical steroids used in conjunction with medicated shampoos were treated with 308-nm excimer laser pulses in conjunction with a hair blower that parted the obstructing hair twice a week for up to 15 weeks. Half of the scalp served as a control. Starting doses were based on standard minimal erythema dose (MED)'s with subsequent increments of up to 20%. Results Thirteen subjects completed the study without adverse events. Two were dropped due to lack of compliance. At the end of the investigation, the difference in the mean modified PASI scores between the control and treated sites was 4.0 (<0.0001). Conclusions The air blower Device in conjunction with the 308-nm excimer laser can safely and effectively treat otherwise refractory scalp psoriasis. Lasers Surg.   • Physical suppression effects of the reversed magnetic coupling on the saturation inductance of saturable pulse transformer

    Abstract:

    As a Combination Device of pulse transformer and magnetic switch, saturable pulse transformer (SPT) is widely employed in high power laser pumping and pulsed power technology. A reversed magnetic coupling mechanism of the diverging magnetic fields among multiple windings is put forward to explain the physical suppression effect on the saturation inductance of the SPT windings. The theoretical and experimental results reveal that the saturation inductance of the SPT can decrease to a level less than its structural inductance through regulating the reversed currents in the multiple windings, which helps to achieve a magnetic switch with ultra-low saturation inductance.

    Abstract:

    As important Devices for voltage boosting and switching, respectively, pulse transformer and magnetic switch are widely used in pulsed power technology. In this paper, a new kind of transformer type magnetic switch (TTMS) with coaxial cylindrical conductors is put forward to combine the functions of voltage boosting and switching in one power Device. As a compact Combination Device of discrete pulse transformer and magnetic switch, the compact TTMS decreases the required volume of magnetic cores in a large scale. The primary windings of the TTMS have a parallel Combination structure so that the TTMS which only has 3 turns of secondary windings has a step-up ratio at 1:9. Before the magnetic core saturates, the TTMS has low unsaturated inductances of windings and good pulse response characteristics, so it can be used to substitute the Marx generator to charge the pulse forming line (PFL) at the ranges of several hundred kV and several hundred ns. After the core saturates, the cylindrical conductors can decrease the saturated inductance of the secondary windings of TTMS to a level less than 400 nH. As a result, the proposed TTMS can be used as the boosting transformer and main switch of helical Blumlein PFL to form the quasi-square voltage pulse on the 160 Ω load with a short pulse rise time only at 60 ns.

  • Effect of Implantation of Augment^® Bone Graft on Serum Concentrations of Platelet-Derived Growth Factors: A Pharmacokinetic Study

    Abstract:

    Background Augment^® Bone Graft is a bone graft substitute intended to be used as an alternative to autologous bone graft in the fusion of hindfoot and ankle joints. Augment^® Bone Graft is a Combination Device comprised of beta-tricalcium phosphate (β-TCP) and recombinant human platelet-derived growth factor BB homodimer (rhPDGF-BB). Objective This human pharmacokinetic study was undertaken to assess the effect of Augment^® Bone Graft implantation on the serum concentration of platelet-derived growth factors (PDGFs). Methods Under the terms of a Research Ethics Board-approved protocol, Augment^® Bone Graft was implanted in patients ( n  = 7) undergoing hindfoot and ankle arthrodesis procedures requiring graft material. The control cohort of the study ( n  = 4) received autologous bone graft. The serum concentrations of PDGF isoforms AA, AB and BB in blood samples, obtained prior to and at ten time points (up to 7 days) after surgery, were measured using enzyme-linked immunosorbent assays (ELISA). Results The serum concentration of PDGF-BB did not vary significantly from baseline (median of the combined cohorts 3.89 ng/mL) throughout the course of the study. The serum concentrations of PDGF-AA, PDGF-AB and total PDGF did not deviate from their baseline values (medians of the combined cohorts were 2.87, 14.95 and 20.19 ng/mL for PDGF-AA, PDGF-AB and total PDGF, respectively) except for the last time point in which they were increased (medians for the combined cohorts were 4.71, 20.42 and 30.29 ng/mL for PDGF-AA, PDGF-AB and total PDGF, respectively). There were no differences between the two treatment groups with regard to changes in the serum concentrations of PDGF. None of the samples tested contained anti-PDGF-BB antibodies. Conclusion Analysis of the data demonstrated that the serum concentrations of all three PDGF isoforms analysed were unaffected by implantation of Augment^® Bone Graft.
