    Abstract:

    Purpose:To report a case demonstrating successful endovascular treatment of a right Common Carotid Artery pseudoaneurysm using a commercially prepared balloon-expandable covered stent.Methods and Results:A 50-year-old man was evaluated for syncopal episodes. He had a history of severe trauma sustained in a motor vehicle accident 3 years before symptom onset. Doppler ultrasound scanning detected a pseudoaneurysm at the origin of the right Common Carotid Artery. The defect measured 25 mm × 20 mm with a 22-mm-long neck on angiography and computed tomography; there was no evidence of Carotid stenosis or associated vascular pathology. Via a percutaneous femoral access, 2 Jostent peripheral stent-grafts were placed at the level of the aneurysm, safely achieving complete repair of the arterial wall defect. The patient was asymptomatic at his 12-month evaluation.   • Automatic Segmentation of Common Carotid Artery in Longitudinal Mode Ultrasound Images Using Active Oblongs

    Abstract:

    We propose a fully automated algorithm for the segmentation of Common Carotid Artery in longitudinal mode ultrasound images using active oblongs. The problem of segmentation and subsequent delineation of lumen-intima layer is solved as an optimization of a locally defined contrast function with respect to five degrees-of-freedom that characterize the active oblong. The detection of the Common Carotid Artery and subsequent initialization of the active oblong inside the Common Carotid Artery region has been done using a combination of binary thresholding, Hough transform, and pixel-offset operations. The algorithm has been validated on the Brno university signal processing lab B-mode ultrasound image database, which contains 84 longitudinal mode ultrasound images of the Common Carotid Artery. The segmentation results are validated against the ground truth provided by two practising radiologists using Jaccard and Dice similarity measures. We have achieved a detection and segmentation accuracy of 95.2% and 97.5%, respectively.

  • Tortuous Common Carotid Artery: A Report of Four Cases Observed in Cadaveric Dissections.

    Abstract:

    A tortuous Common Carotid Artery poses a high risk of injury during tracheotomy. Preoperative diagnosis is therefore important to avoid serious complications. We found four cases of tortuous Common Carotid Artery during an anatomical dissection course for students. The first case was a 91-year-old woman who had bilateral tortuous Common Carotid arteries without arteriosclerosis. Case was a 78-year-old woman who had bilateral tortuous Common Carotid arteries without arteriosclerosis. Case was an 86-year-old woman who died from bladder cancer and who also had a right tortuous Common Carotid Artery without arteriosclerosis. Case was an 89-year-old woman who had bilateral tortuous Common Carotid arteries and a tortuous brachiocephalic Artery with severe arteriosclerosis. Case was also examined using computed tomography to evaluate the arteriosclerosis. Computed tomography revealed severe calcification of the vascular wall, which was confirmed in the aortic arch and origins of its branches. In all four cases, the tortuosity was located below the level of the thyroid gland. Based on prior study results indicating that fusion between the Carotid sheath and visceral fascia was often evident at the level of the thyroid gland, we speculated that the major region in which tortuosity occurs is at the same level or inferior to the level of the thyroid gland.

    Abstract:

    Anomalies of the Carotid Artery are rare in clinical experience. To our knowledge a tortuous Common Carotid Artery with an abnormal course encountered during neck dissection has never been reported in the available world literature. During dissection of the lower neck in a 60-year-old Korean man, a tortuous right Common Carotid Artery was found to cross over the lower cervical trachea anteriorly and then was positioned in its usual site in the Carotid sheath in the mid-neck.
