2019Co-Authors: Novotný RóbertAbstract:Aortic valve-sparing procedures treating patients with aortic root aneurysm with or without aortic insufficiency and patients with ascending aortic aneurysm and aortic insufficiency are no longer experimental and unproven procedures. A successful aortic valve-sparing or repair operation aims not only to correct the failing part of the aortic root but also to restore the intro- and the inter-Component Relationship of the aortic root elements to optimal dimensions and relations. The avoidance of anticoagulation therapy and prosthesis-related complications makes aortic valve repair a tempting procedure. Considering the increasing rate of cusp repair reported in scientific literature, conservative aortic valve surgery seems to be developing into aortic valve repair surgery. This Dissertation Theses are devoted to the study of some specific technical aspects of aortic root sparing surgery, namely to the study of structural changes occurring in cryopreserved human aortic root allografts and the reproducibility of Coroneo ring implantation procedure. One part of the Introduction is...Souhrn Záchovné operace aortálního kořene použité v léčbě pacientů s aneurysmem ascendentní aorty v přítomnosti nebo za absence aortální regurgitace již nepředstavuji experimentální a nedostatečně probádanou proceduru. Úspěšné záchovné operace aortálního kořene nebo opravné operace chlopně jsou uskutečňovány s cílem nejenom korigovat distrofický aortální kořen, ale jde při nich o obnovu funkčnosti jednotlivých komponentů kořene aorty, co se týká jejich rozměrů a propojenosti. Eliminace nutnosti antikoagulační terapie a komplikací, které vznikají při implantaci chlopenních protéz činí záchovné operace aortálního kořene velmi atraktivní pro chirurga i pacienta. Když vezmeme do úvahy údaje z odborné literatury o zvyšujícím se využití plastik chlopní, zdá se, že záchovné operace aortální chlopně a aortálního kořene získávají stále větší význam. Positioning of the prosthetic Components in the transverse plane has theoretically been suggested to affect rotation. Twenty-seven patients who received a Delta III reversed total shoulder prosthesis for CTA were analyzed (mean follow-up, 43 months) using standard radiographs and computed tomography. The position of the prosthetic Components and the possible influence of scapular rotation was analyzed using a uniform spatial reference system using axes in reference to the sagittal or coronal plane. We assessed impingement of the humeral Component on the glenoid neck in neutral and internal rotation. An increase in the anterior divergence of the glenoid and humeral prosthetic Components correlates with an increase in radiologically measured internal rotation ( r = 0.932, P

Reliability Engineering & System Safety, 2016Co-Authors: Liyang Xie, Wenxue QianAbstract:Abstract Time-dependent multi-configuration is a typical feature for mechanical systems such as gear trains and chain drives. As a series system, a gear train is distinct from a traditional series system, such as a chain, in load transmission path, system-Component Relationship, system functioning manner, as well as time-dependent system configuration. Firstly, the present paper defines time-domain series system to which the traditional series system reliability model is not adequate. Then, system specific reliability modeling technique is proposed for gear sets, including Component (tooth) and subsystem (tooth-pair) load history description, material priori/posterior strength expression, time-dependent and system specific load-strength interference analysis, as well as statistically dependent failure events treatment. Consequently, several system reliability models are developed for gear sets with different tooth numbers in the scenario of tooth root material ultimate tensile strength failure. The application of the models is discussed in the last part, and the differences between the system specific reliability model and the traditional series system reliability model are illustrated by virtue of several numerical examples.
