    Abstract:

    This paper presents the findings Based on a survey conducted to examine the status and extent of the use of ComputerBased Technology (CBT) to support “quality”, including such aspects as quality control, quality assurance and quality management, in Hong Kong. Five main categories of CBTs, namely decision support systems (DSSs), group support systems (GSSs), executive information systems (EISs), expert systems (ESs) and artificial neural networks (ANNs), in support of quality are surveyed. The survey findings indicate that CBT usage to support quality in Hong Kong is low, particularly ESs and ANNs. This is partly due to a lack of awareness regarding the potential of CBTs in supporting quality among their potential users, and partly because of a lack of availability of suitable software to support their use. This paper represents a first attempt to examine the real‐life use of CBTs in support of quality. Based on the research findings, we have identified several opportunities for further research and suggested a number of research directions for CBTs to support “quality”in practice.

    Presents the findings of two surveys of ComputerBased technologies to support operations management (OM) in Hong Kong. One of the surveys is to examine the status of the use of ComputerBased Technology and to uncover the extent to which it is being used to support quality including such aspects as quality control, quality assurance and quality management in Hong Kong. The other survey is to investigate the average level of information Technology (IT) applications ‐ the Internet, World Wide Web, local area network, etc. ‐ in small and medium manufacturing enterprises (SMEs) in Hong Kong. It is focused on the information needs and access of the SMEs with a view to understanding the existing operation, problems and needs of SMEs in regard to communication, information access and dissemination and the difficulties of applying IT in these areas.

    Evidence indicates that post − stroke rehabilitation improves function, independence and quality of life. A key aspect of rehabilitation is the provision of appropriate information and feedback to the learner. Advances in information and communications Technology (ICT) have allowed for the development of various systems to complement stroke rehabilitation that could be used in the home setting. These systems may increase the provision of rehabilitation a stroke survivor receives and carries out, as well as providing a learning platform that facilitates long-term self-managed rehabilitation and behaviour change. This paper describes the application of an innovative evaluative methodology to explore the utilisation of feedback for post-stroke upper-limb rehabilitation in the home. Using the principles of realistic evaluation, this study aimed to test and refine intervention theories by exploring the complex interactions of contexts, mechanisms and outcomes that arise from Technology deployment in the home. Methods included focus groups followed by multi-method case studies (n = 5) before, during and after the use of Computer-Based equipment. Data were analysed in relation to the context-mechanism-outcome hypotheses case by case. This was followed by a synthesis of the findings to answer the question, ‘what works for whom and in what circumstances and respects?’ Data analysis reveals that to achieve desired outcomes through the use of ICT, key elements of Computer feedback, such as accuracy, measurability, rewarding feedback, adaptability, and knowledge of results feedback, are required to trigger the theory-driven mechanisms underpinning the intervention. In addition, the pre-existing context and the personal and environmental contexts, such as previous experience of service delivery, personal goals, trust in the Technology, and social circumstances may also enable or constrain the underpinning theory-driven mechanisms. Findings suggest that the theory-driven mechanisms underpinning the utilisation of feedback from Computer-Based Technology for home-Based upper-limb post-stroke rehabilitation are dependent on key elements of Computer feedback and the personal and environmental context. The identification of these elements may therefore inform the development of Technology; therapy education and the subsequent adoption of Technology and a self-management paradigm; long-term self-managed rehabilitation; and importantly, improvements in the physical and psychosocial aspects of recovery.

    AbstractPurpose: Building on previous research findings, this article describes the development of the feedback interfaces for a Personalised Self-Managed Rehabilitation System (PSMrS) for home-Based post-stroke rehabilitation using Computer-Based Technology. Method: Embedded within a realistic evaluative methodological approach, the development of the feedback interfaces for the PSMrS involved the incorporation of existing and emerging theories and a hybrid of health and social sciences research and user-centred design methods. Results: User testing confirmed that extrinsic feedback for home-Based post-stroke rehabilitation through Computer-Based Technology needs to be personalisable, accurate, rewarding and measurable. In addition, user testing also confirmed the feasibility of using specific components of the PSMrS.     Background —Time is of the essence for effective intervention in acute ischemic stroke. Efforts including stroke teams that are “on call” around-the-clock are emerging to reduce the time from emergency room arrival to evaluation and treatment. Summary of Comment —Based on the results of the NINDS rt-PA Stroke Trial, which demonstrated both clinical effectiveness in reducing neurological deficits and disability and cost savings to health care systems, many community hospitals and managed-care organizations are exploring methods to enhance and expedite acute stroke care in their local communities. Only a small fraction of acute stroke victims is currently treated with thrombolytics ( Conclusions —Computer-Based Technology can now be used to integrate electronic medical information, clinical assessment tools, neuroradiology, laboratory data, and clinical pathways to bring state-of-the-art expert stroke care to underserved areas.

In the near future these systems may increase the provision of rehabilitation a stroke survivor receives and carries out, as well as providing a learning platform that facilitates long-term self-management.Aim: To explore the utilisation of extrinsic feedback from Computer-Based Technology to facilitate post-stroke upper-limb rehabilitation in the home.Method: Using the principles of realistic evaluation, this study explored the context of utilising Computer feedback for upper limb stroke rehabilitation in the home setting; the underpinning theory-driven mechanisms of the intervention and how these impacted on theoretical outcomes. Focus groups with Community Stroke Teams were followed by initial testing and multi-method case studies before, during and after the use of Computer-Based equipment. Data were analysed in relation to the hypotheses case by case. This was followed by a synthesis of the findings to answer the question, 'what works for whom and in what circumstances and respects?' Findings: Data analysis reveals that in order to achieve desired outcomes through the use of Computer Technology, key elements of Computer-feedback such as; accurate, measurable, rewarding, adaptable, and knowledge of results feedback are required to trigger the mechanisms underpinning the intervention. In addition, the pre-existing context and the personal and environmental contexts such as; previous experiences of service delivery, personal goals, trust in Technology, social circumstances and practicalities may also enable or constrain the underpinning theory-driven mechanisms. Discussion: This research has explored the implications of the paradigm shift from therapist led rehabilitation where feedback is provided face-to-face to a more patient led model of rehabilitation where feedback is provided by a Computer in the absence of a therapist. Findings suggest that the delivery of feedback in clinical practice is led by their empirical knowledge and understanding of the therapists. However, this model of delivery may not be conducive to motor learning and self-management and may also influence the personal context of users.The theory-driven mechanisms underpinning the utilisation of feedback from Computer-Based Technology for home-Based upper-limb post-stroke rehabilitation are dependent on key elements of Computer feedback and the personal and environmental context. The discovery of these elements may therefore result in the development of Technology; therapy education and the subsequent adoption of Technology and a self-management paradigm; long-term self-managed rehabilitation; and importantly, improvements in the physical and psychosocial aspects of recovery. Further work is required to; develop the Technology so that it incorporates the elements of feedback highlighted by this research; ensure the Technology is robust, reliable and accurate; investigate the clinical utility of Technology for home-Based stroke rehabilitation, and the extent to which it might encourage utilisation by the end user.

    Abstract:

    Strokes are the largest single cause of disability in the UK (DH, 2007). It is estimated that the incidence of first strokes will increase by 30% between 2000 and 2025 (Truelson et al., 2006). Evidence indicates that intensive post−stroke rehabilitation improves function, independence and quality of life (Kwakkel, 2004; Pollock et al., 2007), but according to the Chartered Society of Physiotherapy, the demand for rehabilitation outweighs supply (CSP, 2007). Nevertheless, recent technological advances have promoted the development of tools that may potentially complement the direct efforts of therapists and could in the future even act as surrogates (Liebermann et al., 2006). They include robotassisted movement therapy (Kwakkel et al., 2008), virtual reality Technology (Henderson et al., 2007) and inertial tracking devices (Mountain et al., 2010). These systems have the potential to provide consistent, detailed, individually adapted feedback to the user (Intercollegiate Stroke Working Party, 2008) in the absence of the therapist. However, much of the evidence supporting conventional post-stroke rehabilitation suggests that feedback is provided verbally face to face by a therapist and typically involves hands-on therapy (Hartvelt and Hegarty, 1996; Ballinger et al., 1999; DeJong et al., 2004; Wohlin-Wottrich et al., 2004). The demand for post stroke rehabilitation means that service demand cannot be met and other solutions are necessary. Additionally there are unanswered questions regarding the reliance that a stroke survivor can have upon a therapist for both motor learning skills (Magill, 2007) and the self management of the resultant longterm disability (Jones, 2006).
