    Abstract:

    Background: Hepatitis A virus infection (HAV) has generated about half of the total infection of hepatitis in the world. Poor envi­ronmental sanitation and unhealthy behavior of the society can increase the risk of Hepatitis A transmission. This study aimed to analyze the Contextual Effect of the village on biopsycho­social determinants of Hepatitis A. Subjects and Method: This was an analytic observational study with a case-control ap­proach. This study was conducted from Janu­ary to February 2020. This study used fixed disease sampling. The sample of this study was 200 patients consisted of 50 Hepatitis A pati­ents and 150 non-Hepatitis A patients in villages in Pacitan Regency. The dependent variable was Hepatitis A. The independent vari­ables were age, education, income, history of Hepatitis A vaccination, handwashing behavior, food consumption, availability of clean water, and availability of latrines. This study used questionnaires to collect the data. The data were analyzed by multilevel multiple logistic regression using Stata 13.     Abstract:

    Background: Cervical cancer is a major prob­lem in women's. Cervical cancer can be prevented by early examinations i.e. visual inspection acetic acid (VIA). This study aimed to analyze the contex­tual Effect of community health center on VIA up­take in Magelang, Central Java. Subjects and Method: This was analytic obser­­­­vational study with cross-sectional design. The stu­dy was conducted at 25 community health cen­­­­­ters in Magelang, Central Java, from Sep­tem­ber to October 2019. A to­tal sample of 200 women aged 30 to 50 years was selected ran­domly. The dependent variable was VIA uptake. The independent variables were knowledge, ob­ser­­­vational lear­ning, behavioral reinforcement, cues for action, access to information, family sup­port, attitude, self-efficacy, outcome expec­tation, and Contextual factor of community health cen­ter. The data were collected by ques­tion­naire and analyzed by a multilevel multiple logistic reg­res­sion run on Stata 13. Results: VIA uptake increased with good know­ledge (b= 4.02; 95% CI= 0.61 to 7.42; p= 0.021), obser­­vational learning (b= 2.28; 95% CI= -0.35 to 4.93; p= 0.090), strong behavioral rein­force­ment (b= 4.11; 95% CI= 0.82 to 7.40; p= 0.014), strong cues to action (b= 5.36; 95% CI= 0.65 to 10.07; p= 0.026), good access to information (b= 3.71; 95% CI= 0.42 to 7.01; p= 0.027), strong family support (b= 4.23; 95% CI= 0.21 to 8.25; p= 0.039), positive attitude (b= 3.29; 95% CI= -0.23 to 6.81; p= 0.067), strong self-efficacy (b= 2.94; 95% CI= 0.10 to 5.78; p= 0.042), and strong outcome expectation (b= 3.39; 95% CI= 0.35 to 6.44; p= 0.029). Community health cen­ter had strong Contextual Effect on VIA uptake with intra-class correlation (ICC)= 23%. Conclusion: VIA uptake increases with good know­ledge, observational learning, strong beha­vi­o­ral rein­forcement, strong cues to action, good access to information, strong family support, posi­tive attitude, strong self-efficacy, and strong out­come expectation. The community health center has a strong Contextual Effect on VIA uptake.   • Contextual Effect of Community Health Service on the Use of Triple Elimination Service

    Abstract:

    Background: Triple elimination is the elimination of transmission of HIV, syphilis, and hepatitis B from mother to child. The Prevention of Mother to Child Transmission (PMTCT) program is a comprehensive activity. The basic condition of the case is carried out because of the importance of health promotion and early detection in strategies for managing HIV, Syphilis, and Hepatitis B infections. This study aimed to analyzeContextual Effect of community health service on the use of triple elimination service in pregnant women. Subjects and Method: This was a cross sectional study conducted at 25 community health centers in Sragen Regency, Central Java, from June to July 2019. A sample of 200 pregnant women was selected by stratified random sampling. The dependent variable was the use of triple elimi­nation. The independent variables were age, perceived susceptibility, perceived seriousness, cues to action, attitude, motivation, occupation, income, and distance to health service. The data were collected by a set of questionnaire and analyzed by multilevel multiple logistic regression. Results : The use of triple elimination increased with age ≥35 years (b=1.29; 95% CI=0.37 to 2.21; p=0.006), working outside the home (b=1.20; 95%CI=0.17 to 2.23; p=0.012), income by ≥Rp 1,600,000 (b=1.52; 95% CI=0.54 to 2.50; p=0.002), perceived susceptibility (b=1.46; 95% CI=0.53 to 2.38; p=0.002), perceived seriousness (b=1.53; 95% CI=0.55 to 2.50; p=0.002), cues to action (b=1.30; 95% CI=0.30 to 2.30; p=0.010), attitude (b=1.10; 95% CI=0.02 to 2.18; p=0.046), and strong motivation (b=1.68; 95% CI=0.66 to 2.71; p=0.001). The use of triple elimination decreased by a distance ≥5km (b=-1.36; 95% CI=-2.27 to -0.46; p= 0.003). Community health center had strong Contextual Effect on the use of triple elimination with ICC= 22.6%. Conclusion: The use of triple elimination increases with age ≥35 years, working outside the home, income by ≥Rp 1,600,000, perceived susceptibility, perceived seriousness, cues to action, attitude, and strong motivation. The use of triple elimination decreases by a distance ≥5km. Community health center has strong Contextual Effect on the use of triple elimination.     Background: Family planning will help reduce population growth, fertility, improvement quality of life, women's health status, unwanted pregnancy, and abortion. The selection of the use of con­traceptives needed leads to a contraceptive that has higher Effectiveness, namely the long-term contraceptive method. This study aimed to analyze Contextual Effect of posyandu on the decision to use long term contraceptive in Tulungagung, East Java Subjects and Method: This was a cross-sectional study conducted at 25 integrated health posts (posyandu) in Kauman health service, Tulungagung, East Java, in January 2019. A sample of 200 women of childbearing age was selected by stratified random sampling. The dependent variable was the use of long-term contraceptive method.     Background: Breastfeeding provides amazing benefits for the health of the child and the mother. WHO and UNICEF suggest a mother should be able to practice and maintain exclusive breast­feeding during the first six months of her baby's life. This study aimed to analyze contextal Effect of integrated health post on exclusive breastfeeding. Subjects and Method: This was cross sectional study conducted in 25 integrated health posts (posyandu) in Karanganyar, Central Java, from August to September 2019. A sample of 200 nursing lactating mothers who had children aged 7 to 12 months was selected by stratified random sampling. The dependent variable was exclusive breastfeeding. The independent variables were maternal age, education, occupation, type of birth delivery, knowledge, attitude, family support, and so­cial capital. The data were collected by questionnaire. The data were analysis by multilevel mul­ti­ple logistic regression with Stata 13. Results: Exclusive breastfeeding increased with maternal age 20 to 34 years (b= 2.38; 95% CI= 0.62 to 4.15; p= 0.008), maternal education ≥Senior high school (b= 1.64; 95% CI= 0.01 to 3.23; p= 0.049), good knowledge (b= 2.13; 95% CI= 0.41 to 3.86; p= 0.015), positive attitude (b= 1.67; 95% CI= 0.10 to 3.23; p= 0.036), strong family support (b= 1.58; 95% CI= 0.10 to 3.07; p= 0.036), and strong social capital (b= 1.64; 95% CI= 0.01 to 3.27; p= 0.049).     Background: There is a piece of increasing evidence that exclusive breastfeeding (EBF) up to six-month of age has important consequences on health and nutritional outcomes of children. However, studies on the Contextual Effect of integrated health post (posyandu) on EBF are scarce. The purpose of this study was to examine the Contextual Effect of posyandu on exclusive breastfeeding using multilevel analysis. Subjects and Method: A cross-sectional study was conducted in Sleman, Yogyakarta, from November to December 2018. A sample of 200 lactating mothers was selected by simple random sampling. The dependent variable was exclusive breastfeeding. The independent variables were knowledge, education, age, parity, birth spacing, employment, and family support. The data were collected by a multilevel logistic regression.     Background : It is estimated that 200 million children in developing countries do not develop optimally. Child development is a complex improvement in body structure and function that include gross and fine motoric, speech, language, socialization, and independence aspects of development. The success of children in achieving optimal development determines the future of a nation. The purpose of this study was to analyze the Contextual Effect of integrated health post (posyandu) in child development in Kubu Raya District, West Kalimantan. Subje cts an d Method : This study was a cross sectional conducted at 25 integrated health post in Kubu Raya District, West Kalimantan, from November to December 2018. A sample 200 of children under five was selected by simple random sampling. The dependent variable was child development. The independent variables were maternal age at marriage, maternal education, parental income, parenting style, birth weight, birth length, child development monitoring, child development intervention, and integrated health post strata. Data collection was conducted using questionnaire and analyzed using multilevel logistic regression analysis in Stata 13. Results : Child development disorder increased with early maternal age at marriage (b= 3.18; 95% CI= 0.77 to 5.59; p = 0.010), but decreased with high maternal education (b= -2.22; 95% CI= -3.92 to -0.52; p= 0.010), high parental income (b= -4.01; 95% CI = -6.55 to -1.47; p= 0.002), proper parenting style (b= -2.89; 95% CI= -5.18 to -0.60; p= 0.013), normal birth weight (b= -2.87; 95% CI= -4.80 to -0.95; p= 0.003), normal birth length (b= -3.13; 95% CI= -5.63 to -0.63; p= 0.014), child development monitoring (b= -2.97 95% CI= -5.58 to -0.36; p= 0.025), and child development intervention (b = -3.53; 95% CI= -6.19 to -0.87; p= 0.009). The integrated health post had a Contextual Effect on child development with ICC= 19.9%. Conclusions : Child development disorder increases with early maternal age at marriage, but decreased with high maternal education, high parental income, proper parenting style, normal birth weight, normal birth length, child development monitoring, and child development intervention.     Background: Evidence suggests that prenatal care, healthy behaviors such as exercise and nutrition, general stress level, and routine blood pressure examination, are associated with fetal and maternal health. However, there is a lack of studies on factors affecting these healthy prenatal behaviors in Indonesia.   • Contextual Effect of School on the Risk Obesity among High School Students in Surakarta, Central Java: A Multilevel Analysis Evidence

    Abstract:

    Background: Obesity is a public health problem in various countries, one of which is Indonesia. The impact of obesity in adulthood is high blood pressure and high cholesterol which can later trigger cardiovascular disease. This study aimed to examine Contextual Effect of school on the risk obesity among high school students in Surakarta. Subjects and Method: This was an observational analytic study with a case control approach. The study was conducted in April 2019. The sample was selected by stratified random sampling of 225 students in 15 schools in Surakarta with a case and control group. The dependent variable was obesity. The independent variables were parental salary, parental knowledge, pocket money, school food environment, food intake, physical activity, maternal BMI, paternal BMI. Data collection techniques used questionnaires and analyzed by multilevel logistic regression in Stata 13. Results: Obesity in adolescents was directly and significantly affected by parental income ≥Rp 1,800,000 (OR= 4.53; 95% CI= 1.80 to 11.36; p = 0.001), high parental knowledge (OR= 0.36; 95% CI= 0.15 to 0.91; p= 0.030), allowance ≥Rp 10,000 (OR= 3.80; 95% CI= 1.63 to 8.86; p= 0.002), food intake ≥recommended dietary allowances (OR= 3.62; 95% CI= 1.43 to 9.19; p = 0.007), high physical activity (OR= 0.06; 95% CI= 0.02 to 0.18; p 27kg/m² (OR= 7.08; 95% CI= 2.39 to 20.99; p 27 kg/m² (OR= 2.63; 95% CI= 1.10 to 20.96; p= 0.031). School had a Contextual Effect on obesity with intra-class correlation (ICC)= 16.99%. Conclusion: Obesity in adolescents is directly and significantly affected by parental income ≥Rp 1,800,000, high parental knowledge, allowance ≥Rp 10,000, high physical activity, maternal BMI >27kg/m², and paternal BMI >27 kg/m².     Despite the longstanding interest of social researchers in the social factors that influence suicide and suicidal behavior, multilevel research on this topic has been limited. Using nested survey data on 5331 Icelandic adolescents (born in 1990 and 1991) in 83 school-communities, the current study examines the Contextual Effect of community household poverty on adolescent suicidal behavior (suicide ideation and suicide attempt). The findings show that the concentration of household poverty in the school-community has a significant, Contextual Effect on adolescent suicidal behavior. Furthermore, we test an "epidemic" explanation for this Effect, examining the mediating role of suicide suggestion (contact with suicidal others). We find that suicide suggestion mediates a substantial part of the Contextual Effect of community household poverty on suicide attempt, while mediation is modest in the case of suicide ideation. The findings indicate that community household poverty increases the risk of adolescent suicidal behavior in part because communities in which household poverty is common entail a higher risk for adolescents of associating with suicidal others. The study demonstrates how the concentration of individual problems can have macrolevel implications, creating social mechanisms that cannot be reduced to the circumstances or characteristics of individuals.

Results : The use of triple elimination increased with age ≥35 years (b=1.29; 95% CI=0.37 to 2.21; p=0.006), working outside the home (b=1.20; 95%CI=0.17 to 2.23; p=0.012), income by ≥Rp 1,600,000 (b=1.52; 95% CI=0.54 to 2.50; p=0.002), perceived susceptibility (b=1.46; 95% CI=0.53 to 2.38; p=0.002), perceived seriousness (b=1.53; 95% CI=0.55 to 2.50; p=0.002), cues to action (b=1.30; 95% CI=0.30 to 2.30; p=0.010), attitude (b=1.10; 95% CI=0.02 to 2.18; p=0.046), and strong motivation (b=1.68; 95% CI=0.66 to 2.71; p=0.001). The use of triple elimination decreased by a distance ≥5km (b=-1.36; 95% CI=-2.27 to -0.46; p= 0.003). Community health center had strong Contextual Effect on the use of triple elimination with ICC= 22.6%. Conclusion: The use of triple elimination increases with age ≥35 years, working outside the home, income by ≥Rp 1,600,000, perceived susceptibility, perceived seriousness, cues to action, attitude, and strong motivation. The use of triple elimination decreases by a distance ≥5km. Community health center has strong Contextual Effect on the use of triple elimination.     Background: Family planning will help reduce population growth, fertility, improvement quality of life, women's health status, unwanted pregnancy, and abortion. The selection of the use of con­traceptives needed leads to a contraceptive that has higher Effectiveness, namely the long-term contraceptive method. This study aimed to analyze Contextual Effect of posyandu on the decision to use long term contraceptive in Tulungagung, East Java Subjects and Method: This was a cross-sectional study conducted at 25 integrated health posts (posyandu) in Kauman health service, Tulungagung, East Java, in January 2019. A sample of 200 women of childbearing age was selected by stratified random sampling. The dependent variable was the use of long-term contraceptive method. The independent variables were age, education, knowledge, occupation, husband and family supports, women's perception, local culture, and posyandu. The data were collected by questionnaire and analyzed by a multilevel multiple logistic regression run on Stata 13.     Background: Postpartum depression is a condition that affects 20% women in the first four weeks of the puerperium. Postpartum depression can be affected by biological, psychological, socio-cultural, and economic factors. This study aimed to analyze biopsychosocial factors affecting postpartum depression in Sleman, Yogyakarta. Subjects and Method: This was a cross-sectional study.     Background: The maternal mortality rate (MMR) which is still high has now become the priority in order to find its solution. Midwives play a very significant role in making efforts to reduce MMR through antenatal care (ANC) services. The performance of midwives directly influences the quality of service and health outcomes in the public health centers. The performance of midwives in ANC can be seen based on the coverage of K1 and K4. This study aimed to determine the Contextual Effect of public health centers and other factors that influence the performance of midwives in antenatal care services. Subjects and Method: This study was conducted using observational analytic with cross sectional approach. It was done in 25 health centers in Samarinda, East Kalimantan in April-May 2019. The number of samples was 180 midwives selected using the total sampling technique. The dependent variable is the performance of the midwife. The independent variables include age, knowledge, tenure, training, workload, incentive, supervision, and motivation. The data were collected by questionnaire and analyzed by multilevel multiple logistic regression.     Despite the longstanding interest of social researchers in the social factors that influence suicide and suicidal behavior, multilevel research on this topic has been limited. Using nested survey data on 5331 Icelandic adolescents (born in 1990 and 1991) in 83 school-communities, the current study examines the Contextual Effect of community household poverty on adolescent suicidal behavior (suicide ideation and suicide attempt). The findings show that the concentration of household poverty in the school-community has a significant, Contextual Effect on adolescent suicidal behavior. Furthermore, we test an "epidemic" explanation for this Effect, examining the mediating role of suicide suggestion (contact with suicidal others). We find that suicide suggestion mediates a substantial part of the Contextual Effect of community household poverty on suicide attempt, while mediation is modest in the case of suicide ideation. The findings indicate that community household poverty increases the risk of adolescent suicidal behavior in part because communities in which household poverty is common entail a higher risk for adolescents of associating with suicidal others. The study demonstrates how the concentration of individual problems can have macrolevel implications, creating social mechanisms that cannot be reduced to the circumstances or characteristics of individuals.

    Abstract:

    The current study examines the Contextual Effects of community structural characteristics, as well as the mediating role of key social mechanisms, on youth suicidal behavior in Iceland. We argue that the Contextual influence of community structural instability on youth suicidal behavior should be mediated by weak attachment to social norms and values (anomie), and contact with suicidal others (suggestion-imitation). The data comes from a national survey of 14-16 years old adolescents. Valid questionnaires were obtained from 7018 students (response rate about 87%). The findings show that the community level of residential mobility has a positive, Contextual Effect on adolescent suicidal behavior. The findings also indicate that the Contextual Effect of residential mobility is mediated by both anomie and suggestion-imitation. The findings offer the possibility to identify communities that carry a substantial risk for adolescent suicide as well as the mechanisms that mediate the influence of community structural characteristics on adolescent risk behavior.
