In this neuroimaging study, Cue Reactivity in problem gamblers (PRG) was compared with Cue Reactivity in heavy smokers (HSM) and healthy controls (HC). A functional magnetic resonance imaging event-related Cue Reactivity Paradigm, consisting of gambling, smoking-related and neutral pictures, was employed in 17 treatment-seeking non-smoking PRG, 18 non-gambling HSM, and 17 non-gambling and non-smoking HC. Watching gambling pictures (relative to neutral pictures) was associated with higher brain activation in occipitotemporal areas, posterior cingulate cortex, parahippocampal gyrus and amygdala in PRG compared with HC and HSM. Subjective craving in PRG correlated positively with brain activation in left ventrolateral prefrontal cortex and left insula. When comparing the HSM group with the two other groups, no significant differences in brain activity induced by smoking Cues were found. In a stratified analysis, the HSM subgroup with higher Fagerstrom Test for Nicotine Dependence scores (FTND M = 5.4) showed higher brain activation in ventromedial prefrontal cortex, rostral anterior cingulate cortex, insula and middle/superior temporal gyrus while watching smoking-related pictures (relative to neutral pictures) than the HSM subgroup with lower FTND scores (FTND M = 2.9) and than non-smoking HC. Nicotine craving correlated with activation in left prefrontal and left amygdala when viewing smoking-related pictures in HSM. Increased regional responsiveness to gambling pictures in brain regions linked to motivation and visual processing is present in PRG, similar to neural mechanisms underlying Cue Reactivity in substance dependence. Increased brain activation in related fronto-limbic brain areas was present in HSM with higher FTND scores compared with HSM with lower FTND scores.

Effect of rapamycin on Cue-induced drug craving in abstinent heroin addicts

This randomized double-blind clinical trial enrolled 60 abstinent heroin addicts and randomly assigned them to three groups: placebo, 2.5 mg and 5 mg rapamycin. The participants were given the Cue-Reactivity Paradigm with 5 min exposures to neutral and drug-related imagery while craving, anxiety, blood pressure and heart rate pre- and post-exposure were assessed. We found that drug-related Cues increased both craving and anxiety of abstinent heroin addicts, and had no effect on blood pressure and heart rate. A single high-dose of rapamycin significantly reduced the craving, but not anxiety induced by drug-related Cues. Our findings suggested that rapamycin merits outpatient clinical trials as a potential pharmacotherapy for relapse prevention from drug-related Cue induced craving. (C) 2009 Elsevier B.V. Diurnal variation in Cue-induced responses among protracted abstinent heroin users

However, there is no study to demonstrate whether the responses to drug Cue exposure are diurnal dependence. The present study was to examine whether there was a variation in drug-related Cue Reactivity across the diurnal cycle among recently abstinent opiate addicts. Methods: Four groups of 20 abstinent heroin dependent patients (n=80) were exposed to both neutral and drug-related videos at four separate times during the day: 8:00,12:00,16:00, and 20:00 h. Physiological and psychological responses, including heart rate, blood pressure, heroin craving, and subjective anxiety were assessed before and after each Cue exposure. Results: Drug Cue significantly increased craving ratings compared to neutral Cues across all the four separate times of day. Drug Cue-induced craving was greater in the morning (8:00 am) than noon (12:00 pm), but was similar to evening assessments (8 pm). Drug Cues also significantly increased anxiety, which positively correlated with Cue-induced craving. Drug Cues increased heart rate, systolic and diastolic blood pressures, which were not correlated with Cue-induced craving or anxiety. However, no time effects were found on the three physiological measures. Conclusions: Cue-induced craving could be profoundly affected by the time points of Cue exposure, using Cue-Reactivity Paradigm. The relative sensitivity of morning and evening assessments of drug craving suggests a need for replication and further research on mechanisms contributing to these diurnal variations. striatal alcohol Cue Reactivity is stronger in male than female problem drinkers

Wiers, Taco J De Vries, Tommy Pattij, Anna E GoudriaanAbstract:Despite apparent sex differences in the development and treatment of alcohol use disorder, relatively little is known about the underlying neural mechanisms. In this study, we therefore investigated neural Cue-Reactivity in a sample of male (n = 28) and female (n = 27) problem drinkers (matched on age and alcohol use severity) with an average alcohol use disorder identification test score of 12 which is indicative of a likely alcohol use disorder. Neural Cue-Reactivity data were extracted from four regions of interest: the ventral and dorsal striatum and the ventral and dorsal anterior cingulate cortex, with a significance level set at p < 0.05. While the Cue-Reactivity Paradigm induced similar levels of self-reported craving in men and women, visual alcohol Cues induced significantly stronger striatal activation in men compared to drinkers. While sex differences in ventral striatal Cue-Reactivity were partly explained by sex differences in alcohol intake, cannabis use, negative affect and anxiety, this was not the case for sex differences in dorsal striatal Cue-Reactivity. These results suggest that alcohol Cues are differentially processed by men and women and that the neurobiological mechanisms behind Cue-Reactivity differ between the sexes. Consequently, Paradigms using alcohol-related pictures may not be optimal to induce Cue-Reactivity in female drinkers and may not be optimal to measure neurobiological markers of alcohol use severity and relapse. Future alcohol Cue-Reactivity studies should, in addition to including both men and women, include different types of Cues (e.g., stressors and imagery in addition to pictures) to assess sex differences in alcohol Cue-Reactivity.

Cue-Reactivity in behavioral addictions: A meta-analysis and methodological considerations

The aim of the current meta-analysis is to systematically analyze the effects of learning-based Cue-Reactivity in behavioral addictions. Methods: The current meta-analysis includes 18 studies (29 data sets, 510 participants) that have used a Cue-Reactivity Paradigm in persons with gambling (eight studies), gaming (nine studies), or buying (one study) disorders. We compared subjective, peripheral physiological, electroencephal, and neural responses toward addiction-relevant Cues in patients versus control participants and toward addiction-relevant Cues versus control Cues in patients. Results: Persons with behavioral addictions showed higher Cue-Reactivity toward addiction-relevant Cues compared with control participants: subjective Cue-Reactivity (d = 0.84, p = .01) and peripheral physiological and electroencephal measures of Cue-Reactivity (d = 0.61, p Discussion and conclusions: Cue-Reactivity not only exists in substance-use disorders but also in gambling, gaming, and buying disorders. Future research should differentiate between Cue-Reactivity in addictive behaviors and Cue-Reactivity in functional excessive behaviors such as passions, hobbies, or professions.

pathological buying online as a specific form of internet addiction a model based experimental investigation
PLOS ONE, 2015Co-Authors: Patrick Trotzke, Katrin Starcke, Astrid Muller, Matthias BrandAbstract:The study aimed to investigate different factors of vulnerability for pathological buying in the online context and to determine whether online pathological buying has parallels to a specific Internet addiction. According to a model of specific Internet addiction by Brand and colleagues, potential vulnerability factors may consist of a predisposing excitability from shopping and as mediating variable, specific Internet use expectancies. Additionally, in line with models on addiction behavior, Cue-induced craving should also constitute an important factor for online pathological buying. The theoretical model was tested in this study by investigating 240 female participants with a Cue-Reactivity Paradigm, which was composed of online shopping pictures, to assess excitability from shopping. Craving (before and after the Cue-Reactivity Paradigm) and online shopping expectancies were measured. The tendency for pathological buying and online pathological buying were screened with the Compulsive Buying Scale (CBS) and the Short Internet Addiction Test modified for shopping (s-IATshopping). The results demonstrated that the relationship between individual’s excitability from shopping and online pathological buying tendency was partially mediated by specific Internet use expectancies for online shopping (model’s R² = .742, p < .001). Furthermore, craving and online pathological buying tendencies were correlated (r = .556, p < .001), and an increase in craving after the Cue presentation was observed solely in individuals scoring high for online pathological buying (t(28) = 2.98, p < .01, d = 0.44). Both screening instruments were correlated (r = .517, p < .001), and diagnostic concordances as well as divergences were indicated by applying the proposed cut-off criteria. In line with the model for specific Internet addiction, the study identified potential vulnerability factors for online pathological buying and suggests potential parallels. The presence of craving in individuals with a propensity for online pathological buying emphasizes that this behavior merits potential consideration within the non-substance/behavioral addictions.

Cybersex addiction: Experienced sexual arousal when watching pornography and not real-life sexual contacts makes the difference

Schulte, Matthias BrandAbstract:Background and aims: Cybersex addiction is discussed controversially, while empirical evidence is widely missing. With respect to its mechanisms of development and maintenance Brand et al. (2011) assume that reinforcement due to cybersex should lead to the development of Cue-Reactivity and craving explaining recurrent cybersex use in the face of growing but neglected negative consequences. To support this hypothesis, two experimental studies were conducted. Methods: In a Cue-Reactivity Paradigm 100 pornographic Cues were presented to participants and indicators of sexual arousal and craving were assessed. The first study aimed at identifying predictors of cybersex addiction in a freely recruited sample of 171 heterosexual males. The aim of the second study was to verify the findings of the first study by comparing healthy (n = 25) and problematic (n = 25) cybersex users. Results: The results show that indicators of sexual arousal and craving to Internet pornographic Cues predicted tendencies towards cybersex addiction in the first study. Moreover, it was shown that problematic cybersex users report greater sexual arousal and craving reactions resulting from pornographic Cue presentation. In both studies, the number and subjective quality of real-life sexual contacts were not associated to cybersex addiction. Discussion: The results support the gratification hypothesis, which assumes reinforcement, learning mechanisms, and craving to be relevant processes in the development and maintenance of cybersex addiction. Poor or unsatisfying sexual real-life contacts cannot sufficiently explain cybersex addiction. Conclusions: Positive reinforcement in terms of gratification plays a major role in cybersex addiction
