    Abstract:

    Objective To compare the safety of haemophilus influenzae type b (Hib) vaccine vaccination on vastus lateralis Muscle and Deltoid Muscle of infant. Methods A total of 408 3-4 months old infants were divided into vastus lateralis Muscle group and Deltoid Muscle group in Beijing, 2014. They were divided into the vastus lateralis Muscle group (204) and Deltoid Muscle group (204) by extracting random number. Each observation object was given 3 doses of Hib vaccine according to the program. Collected systemic and local reactions after vaccination and calculated the incidence of adverse reactions. Results A total of 61 infants were quitted during the study, 1 132 doses were observed. The total reactions incidence of Vastus lateralis Muscle group and Deltoid Muscle group were 33.0% (186/564) and 27.6% (157/568) with no statistical differences (χ2=3.818, P=0.059). The two groups incidence at the same day of vaccination (day 0) which the highest were 23.2% (131/564) and 20.6% (117/568), then declined with time (linear trend test vastus lateralis Muscle group χ2=36.600, P<0.001, Deltoid Muscle group χ2=29.947, P< 0.001), day 1 were 20.4% (115/564) and 17.6% (100/568), day 2 were 16.0% (90/564) and 13.4% (76/568), day 3 were 10.3%(58/564) and 10.6%(60/568), day 4-7 were 11.2%(63/564) and 11.3%(64/568). No serious adverse events (SAE) were reported during the study. The local reactions incidence of two groups were 7.1% (40/564)and 7.7% (44/568)with no statistical differences (χ2=0.176, P=0.675). The systemic reactions incidence of two groups were 25.9% (146/564) and 20.6% (117/568) with obvious statistical differences (χ2= 4.437, P=0.035). The fever incidence of vastus lateralis Muscle group (11.5% (65/564)) was higher than Deltoid Muscle group (4.4%(25/568)) with obvious statistical differences (χ2=4.868, P=0.027). The 1st dose incidence of fever and abnormal crying of vastus lateralis Muscle group (fever 11.3% (23/204), abnormal crying 19.1% (39/204)) was higher than Deltoid Muscle group (fever 4.4% (9/204), abnormal crying 11.8% (24/204)) and the 2nd dose of diarrhea of Deltoid Muscle group (11.6% (22/190)) was higher than vastus lateralis Muscle group (5.9% (11/187)) with obvious statistical differences (fever χ2=15.288, P<0.001, abnormal crying χ2=4.224, P=0.040, diarrhea χ2=3.829, P=0.046). Conclusion Both vastus lateralis Muscle group and Deltoid Muscle group had lower incidence of adverse reactions after vaccination. No serious adverse events were associated with vaccination. Vastus lateralis Muscle vaccination as well as Deltoid Muscle vaccination demonstrated safe. Key words: Infant; Vaccination; Injections, intramuscular

    We aimed to estimate the interrelation between preoperative Deltoid Muscle status by measuring the 3-dimensional Deltoid Muscle volume and postoperative functional outcomes after reverse total shoulder arthroplasty(RTSA). Thirty-five patients who underwent RTSA participated in this study. All patients underwent preoperative magnetic resonance imaging(MRI) as well as pre- and postoperative radiography and various functional outcome evaluations at least 1 year. The primary outcome parameter was set as age- and sex-matched Constant scores. The 3-dimensional Deltoid Muscle model was generated using a medical image processing software and in-house code, and the Deltoid Muscle volume was calculated automatically. Various clinical and radiographic factors comprising the Deltoid Muscle volume adjusted for body mass index(BMI) were analyzed, and their interrelation with the outcome parameters was appraised using a multivariate analysis. As a result, all practical consequences considerably improved following surgery(all p<0.01). Overall, 20 and 15 indicated a higher and a lower practical consequence than the average, respectively, which was assessed by the matched Constant scores. The Deltoid Muscle volume adjusted for BMI(p = 0.009), absence of a subscapularis complete tear (p = 0.040), and greater change in acromion-Deltoid tuberosity distance(p = 0.013) were associated with higher matched Constant scores. Multivariate analysis indicated that the Deltoid Muscle volume was the single independent prognostic factor for practical consequences(p = 0.011). In conclusion, the preoperative Deltoid Muscle volume significantly affected the functional outcome following RTSA in patients with cuff tear arthropathy or irreparable cuff tears. Therefore, more attention should be paid to patients with severe atrophied Deltoid Muscle who are at a high risk for poor practical consequences subsequent to RTSA.

    BACKGROUND The present study aimed to determine functional outcomes in patients undergoing Deltoid Muscle resection for soft tissue sarcoma. METHODS Between 2002 and 2014, 18 patients with soft tissue sarcoma of the shoulder who underwent wide resection including the Deltoid Muscle, and were followed up for more than 12 months, were retrospectively included in the study. In all, 11 patients were male and 7 were female. The median age was 59 years, median follow-up duration was 37 months. The extent of resection of Deltoid Muscle, with or without rotator cuff damage, reconstruction methods, adjuvant therapy, oncological outcomes, and the International Society of Limb Salvage (ISOLS) score as functional outcomes were analyzed. RESULTS Six patients underwent total resection, and twelve underwent partial resections of Deltoid Muscle. The rotator cuff was resected in four patients. Soft tissue reconstruction was performed in 17 patients using a pedicled latissimus dorsi Muscle flap. Two local recurrences and three distant metastases occurred during follow-up. Median overall survival was 72 months. The mean ISOLS score was 25.0 points (±4.6points). Univariate analysis revealed that there was no significant difference in ISOLS score regarding the extent of Deltoid Muscle resection. Multivariate analysis identified only combined resection of the rotator cuff as a significant prognostic factor for poor functional outcomes (P < 0.001). CONCLUSIONS The extent of resection of the Deltoid Muscle might not affect the functional outcomes determined by ISOLS score. If the rotator cuff is resected concurrently, satisfactory functional outcomes might not be obtained.

    Background Although many advantages of reverse total shoulder arthroplasty (RTSA) have been demonstrated, a variety of complications indicate there is much to learn about how RTSA modifies normal shoulder function. This study used a subject-specific computational model driven by in vivo kinematic data to assess how RTSA affects Deltoid Muscle moment arms after surgery. Methods A subject-specific 12 degree-of-freedom musculoskeletal model was used to analyze the shoulders of 26 individuals (14 RTSA and 12 normal). The model was modified from the work of Holzbaur to directly input 6 degree-of-freedom humeral and scapular kinematics obtained using fluoroscopy. Results The moment arms of the anterior, lateral, and posterior aspects of the Deltoid were significantly different when RTSA and normal cohorts were compared at different abduction angles. Anterior and lateral Deltoid moment arms were significantly larger in the RTSA group at the initial elevation of the arm. The posterior Deltoid was significantly larger at maximum elevation. There was large intersubject variability within the RTSA group. Conclusions Placement of implant components during RTSA can directly affect the geometric relationship between the humerus and scapula and the Muscle moment arms in the RTSA shoulder. RTSA shoulders maintain the same anterior and posterior Deltoid Muscle moment-arm patterns as healthy shoulders but show much greater intersubject variation and larger moment-arm magnitudes. These observations provide a basis for determining optimal implant configuration and surgical placement to maximize RTSA function in a patient-specific manner.
