    Abstract:

    Abstract Sepsis is a leading cause of death in the United States, with a mortality rate in excess of 215,000 deaths per year. It may lead to septic shock, a complex pathophysiological process with microbial and host response events that progress to multisystem derangement. There is poor documentation of the relationship between Dental infection and septic shock, with only a few case reports of septic shock secondary to dentoalveolar Abscess. Presented is a case of sepsis/septic shock in a 23-year-old man with signs and symptoms of pulpal necrosis, acute apical Abscess, and canine space infection that rapidly progressed to an altered mental state, hyperthermia, tachycardia, hypotension, acute respiratory failure, diarrhea, renal insufficiency, lactic acidosis, leukocytosis, and hyperglycemia. Once septic shock develops, the mortality rate is nearly 50%. Early antimicrobial intervention is associated with surviving severe sepsis, making it critical for dentists to understand local factors leading to the crisis and the signs and symptoms of the sepsis–septic shock continuum.

    Abstract:

    A previously undescribed filamentous, beaded, Gram-positive, rod-shaped bacterium was isolated from pus of a human Dental Abscess. Based on its cellular morphology and the results of biochemical testing the organism was tentatively identified as a member of the genus Actinomyces, but it did not correspond to any currently recognized species of this genus. Comparative 16S rRNA gene sequencing studies showed the bacterium represents a distinct subline within the genus Actinomyces, clustering within a group of species that includes Actinomyces bovis, the type species of the genus. Sequence divergence values of >8 % with other recognized species within this phylogenetic group clearly demonstrated that the organism represents a hitherto unknown species. Based on biochemical and molecular phylogenetic evidence, it is proposed that the unidentified organism recovered from a Dental Abscess be classified as a novel species, Actinomyces Dentalis sp. nov. The type strain is R18165T (=CCUG 48064T=CIP 108337T).

    Abstract:

    In the imaging evaluation of Emergency Department patients presenting with facial pain, there is a condition that can mimic the radiographic appearance of a periapical Dental Abscess. This condition, cemento-osseous dysplasia, may appear similar on Dental X-rays, panoramic radiographs, and computed tomography examinations to and must be distinguished from the more common periapical Abscess in order to avoid inappropriate intervention. This review highlights the easy confusion of these two entities based on radiographic appearance and the clinical implications of such a mistake and suggests some approaches to avoid this error.
