    Objective Our purpose was to estimate and compare the cost-effectiveness of the most commonly used diagnostic Tests for onychomycosis: potassium hydroxide preparation (KOH), interpreted both by a dermatologist (KOH-CLINIC) and a laboratory technician (KOH-LAB); KOH with dimethyl sulfoxide (KOH-DMSO) and with chlorazol black E (KOH-CBE), interpreted by a dermatologist; culture using Dermatophyte Test Medium, culture with Mycobiotic and Inhibitory Mold Agar (Cx); and histopathologic analysis using periodic acid–Schiff stain (PAS). Methods This was a repeated-measure, blinded, cross-sectional study conducted at the Minneapolis Veterans Affairs Medical Center. Inclusion criteria included: at least one toenail with 25% or more clinical disease, which was defined as subungual debris with onycholysis and/or onychauxis. Exclusion criteria included other nail dystrophies, use of oral antifungal medication for 2 months or longer within the past year, or topical ciclopirox lacquer within 6 weeks of enrollment. The main outcome measure was the cost-effectiveness (Medicare and non-Medicare costs) of 7 diagnostic Tests. Sensitivity (at least 3 positive Tests) was the unit of effectiveness. Results Two hundred four participants were enrolled; their average age was 69.5 years and 95.5% were male. PAS was the most sensitive Test (98.8%); it was statistically significantly more sensitive than all other diagnostic Tests except KOH-CBE (94.3%). Dermatophye Test Medium was the least sensitive Test (57.3%). KOH-CBE was statistically significantly more cost effective than any other Test, with the exception of KOH-CLINIC and KOH-LAB. PAS was the least cost effective. Limitations Test specificities were not evaluated. Conclusion KOH-CBE should be the Test of choice for practitioners confident in interpreting KOH preparations because of its combination of high sensitivity and cost-effectiveness.

Location data are displayed only to the county (or sometimes state) level due to concerns with disclosing specific locations of rare or sensitive snake populations. Fungal infection was assessed by examining histologic sections of skin lesions. The number of gross lesions consistent with dermatitis were categorized as "none" (no gross skin lesions observed), "single" (one lesion), "multiple" (more than one discrete lesion), or "not assessed" (no information was available on the number of skin lesions present). The type of fungal growth Medium upon which samples were cultured is listed as "DTM" (Dermatophyte Test Medium) or "SD" (Sabouraud dextrose Medium containing chloramphenicol and gentamycin). The number of unique internal transcribed spacer region DNA sequences identified per snake (or operational taxonomic units [OTUs] at 100% sequence identity) is listed.     There are some reports about the risk of manipulating wild hedgehogs since they can be reservoirs of potential zoonotic agents like Dermatophytes. The aim of this study was to describe the integument mycobiota, with special attention to Dermatophytes of wild European hedgehogs. Samples from spines and fur were cultured separately in Sabouraud dextrose agar (SDA) with antibiotic and Dermatophyte Test Medium (DTM) plates. Nineteen different fungal genera were isolated from 91 cultures of 102 hedgehogs. The most prevalent genera were Cladosporium (79.1%), Penicillium (74.7%), Alternaria (64.8%), and Rhizopus (63.7%). A lower prevalence of Aspergillus (P = 0,035; χ (2) = 8,633) and Arthrinium (P = 0,043; χ (2) = 8,173) was isolated during the spring time and higher frequencies of Fusarium (P = 0,015; χ (2) = 10,533) during the autumn. The prevalence of Acremonium was significantly higher in young animals (70%, 26/37) than in adults (30%, 11/37) (P = 0,019; χ (2) = 5,915). Moreover, the majority of the saprophytic species that grew at the SDA culture were also detected at the DTM. Finally, no cases of ringworm were diagnosed and no Dermatophytes spp. were isolated. Concluding, this study provides the first description of fungal mycobiota of the integument of wild European hedgehogs in Spain, showing a large number of saprophytic species and the absence of Dermatophytes.
