    Abstract:

    We have studied the effects of rifampicin on the overnight 1-mg Dexamethasone Suppression Test usually employed to exclude suspected Cushing's syndrome. Previous observations indicate that in humans, rifampicin profoundly attenuates the biological effects of hydrocortisol and prednisolone, probably by increasing the metabolism of these drugs in the liver. The study was carried out in 16 normal volunteers. All subjects had a normal overnight 1-mg Dexamethasone Suppression Test (468 +/- 86 vs. 32 +/- 21 nmol/L; mean +/- SD). In 8 subjects treated with rifampicin (600 mg) for 10 days, the inhibitory effect of Dexamethasone on serum cortisol was completely prevented (575 +/- 114 vs. 434 +/- 82). In the remaining 8 rifampicin-treated subjects, the inhibitory effect of 1, 2, or 3 mg Dexamethasone on serum cortisol was not observed. When 4 mg Dexamethasone were administered, the serum cortisol level was 193 nmol/L, above the expected normal Suppression value. The plasma Dexamethasone concentration was very low a...

    Abstract:

    A connection between a high level of plasma cortisol and psychopathology has long been observed. In depressive patients increased plasma cortisol and non-Suppression to the Dexamethasone Suppression Test (DST) has often been described. In this study it was found that endogenously depressed patients more often than non-endogenously depressed patients showed abnormal results to the DST. No difference was found in delta MAX TSH between suppressors and non-suppressors to the DST.

    Abstract:

    Depressive psychiatric patients often shown non-Suppression to the Dexamethasone Suppression Test (DST). Stroke patients shows a high frequency of depression. In the present study the DST was studied in 76 stroke patients and 26 controls. No difference was found in frequency of non-Suppression to the DST between depressive and non-depressive stroke patients. It was found that postDexamethasone plasma cortisol level at 08 a.m. was significantly higher in patient with the lesion in the right hemisphere compared to patients with the lesion in the left hemisphere.

    Abstract:

    The overnight 1-mg Dexamethasone Suppression Test is a very good screening Test for subjects suspected of having Cushing's syndrome. To simplify the procedure, we evaluated the 1-mg Dexamethasone Suppression Test with measurement of salivary cortisol. We performed this Test with plasma and salivary cortisol measurements in 27 patients with Cushing's syndrome and 64 normal controls. The sensitivity and specificity of plasma cortisol measurements were 100% and 87%, respectively, for a cut-off point of 100 nmol/l, in accordance with previous studies. The results of salivary cortisol showed the absence of overlap between the two groups, with a sensitivity and specificity of 100% for a cut-off point of 2.8 nmol/l. On a larger series, however, one might occasionally miss the diagnosis of a patient with Cushing's syndrome. Therefore, we favor a cut-off point of 1.9 nmol/l, the sensitivity remaining at 100% and the specificity being 94%. In conclusion we recommend the overnight Dexamethasone Suppression Test with measurement of salivary cortisol as a screening Test for Cushing's syndrome.
