Objective: This multicenter study evaluated the long-term effectiveness of psyllium husk fiber as an adjunct to Diet in the treatment of persons with primary hypercholesterolemia. Design: Men and women with hypercholesterolemia were recruited. After following an American Heart Association Step I Diet for 8 wk (Dietary adaptation phase), eligible subjects ts with serum LDL-cholesterol concentrations between 3.36 and 4.91 mmol/L were randomly assigned to receive either 5.1 g psyllium or a cellulose placebo twice daily for 26 wk while continuing Diet Therapy. Results: Serum total and LDL-cholesterol concentrations were 4.7% and 6.7% lower in the psyllium group than in the placebo group after 24-26 wk (P < 0.001). Other outcome measures did not differ significantly between groups. Conclusions: Treatment with 5.1 g psyllium twice daily produces significant net reductions in serum total and LDL-cholesterol concentrations in men and women with primary hypercholesterolemia. Psyllium Therapy is an effective adjunct to Diet Therapy and may provide an alternative to drug Therapy for some patients.

Case report Polyunsaturated fatty acid-enriched Diet Therapy for a child with epilepsy

Fatty acids are the most important constituent of the KD in all aspects of efficacy and complications. Among fatty acids, polyunsaturated fatty acids (PUFAs) increase anticonvulsant properties and reduce the complications associated with the high-fat Diet. Here, we report a 7-year-old boy with Lennox–Gastaut syndrome combined with mitochondrial respiratory chain complex I deficiency, whose medically intractable seizures have been successfully controlled with a PUFA-enriched modified Atkins Diet without any significant adverse events. The Diet consists of canola oil and diverse menu items like fish and nuts instead of olive oil and has an ideal 1:2.8 ratio of omega-3 to omega-6. In addition, fractionation of this boy’s plasma showed normal levels of fatty acids, including omega-3 (alpha-linoleic acid, eicosapentaenoic acid) and omega-6 (linoleic acid, arachidonic acid) as well as monounsaturated fatty acids (oleic acid). Plasma docosahexanoic acid remained low after PUFA-enriched Diet Therapy. PUFA-enriched Diet Therapy is likely to increase the efficacy of Diet Therapy and reduce complications of a high-fat Diet in children with refractory epilepsy.

The knack of treatment for obese patients with hypertension and diabetes mellitus: combined the newest tailor-made Diet Therapy with the usual stress-management Therapy

But, the effect of Diet Therapy on obese patients differs in each individual because of the differences in energy expenditure, such as the resting metabolic rate (RMR). We have recently reported that obese Japanese with the missense mutation (Trp64Arg) of the beta 3-adrenergic receptor (beta 3-AR) gene and/or the uncoupling protein 1 (UCP1) gene mutation (A-3826G) have difficulties in weight loss by the reducing RMR, while Japanese with beta 2-AR gene mutation (Arg16Gly) have an under-weight condition through increased RMR. Here I introduce the importance of tailor-made Diet Therapy to improve obesity based on analysis of gene polymorphisms and in order to maintain this tailor-made Diet Therapy, I emphasize that the stress-management Therapy should be needed to remove mental problems which caused obesity.

But effect of the Diet Therapy differs in each individual, because of the difference of energy expenditure. Since the relationship between the missense mutation (Trp64Arg) of the beta 3-adrenergic receptor (beta 3-AR) gene and low energy expenditure in the obese Pima Indians was made clear, gene polymorphisms that mediate body weight are reported one after another. We also reported that one third of Japanese have the Trp64Arg mutation of beta 3-AR gene, which may produce obesity or difficulties in weight loss. Here we introduce the obesity-related genes, and report the importance of Tailor-made Diet Therapy based on molecular genetics to improve obesity.

All of the patients were enrolled in the combined Diet Therapy and exercise [1255 kj (300 kcal/day)] for 7 months. At the end of the first month, the patients were given, at random, three tablets of either 0.5 mg mazindol (18 women) or placebo (18 women) per day, in three doses for three months, in addition to the Diet and exercise therapies. At 7 months, the key for the double-blind test was opened by the controller. Four of the 18 patients in the placebo group dropped out of the programme at 2 months after drug treatment, because they could not endure the intense hunger. Therefore, data for these 4 patients were excluded from evaluation. However, none of the patients treated with mazindol+combined Diet Therapy dropped out. The body weight and body-fat ratio showed marked reduction (p<0.01 vs. premedication weight). The percent weight loss and percent reduction in body-fat ratio in the mazindol group was significantly (p<0.01) greater than those found in the placebo group, which also showed a significant decrease in body weight and body-fat ratio (p<0.01 vs. pre-medication weight) during the same period. In the mazindol group, the daily caloric intake and the exercise level at 4 months was significantly (p<0.01) lower and higher, respectively, than those in the placebo group. At 7 months, the body weight in both groups was significantly decreased as compared to that at the start of treatment, and the difference between the groups in body weight and weight loss was still apparent. These results suggest that this combined Therapy with mazindol plus Diet Therapy (low-calorie Diet+Optifast) is a useful means of treating severe obesity

We report a case of repeated hypertriglyceridemia during pregnancy. During the patient's first pregnancy, acute pancreatitis was elicited in the third trimester by pregnancy-induced hypertriglyceridemia. The patient was treated successfully with a conservative treatment course. The hypertriglyceridemia recurred during her second pregnancy. She carried the pregnancy to term without incident while maintaining a Diet low in fat Diet and high in ω-3 fatty acids. Early diagnosis and intensive treatment can help to preserve the lives of the patient and the fetus. Prophylactic Diet Therapy and ω-3 fatty acids may prevent recurrent hypertriglyceridemia during pregnancy.
