    Abstract The potential for rural areas to benefit from telecommunications technology is a persistent question. This paper examines data for the USA concerning the ‘Digital divide’ and access of residences and businesses, which tend to suggest that all is (or will soon be) well. The paper also presents data on aspects of Digital infrastructure in rural America, including points of presence and Digital telephone switches, which suggest that there are major shortcomings in most rural communities. Demand aggregation is a possible solution, but more serious pitfalls are those related to shortages of human capital. These might be resolved in some rural places, where inmigration and return migration bring needed cerebral inputs to rural areas. A final set of improvements concerns how businesses use the Internet and e-commerce. In the end, telecommunications is not a ‘quick fix’ solution for rural Development, and the desired improvements will be limited to a fraction of rural places.

    Background Design thinking and human-centered design approaches have become increasingly common in health care literature, particularly in relation to health information technology (HIT), as a pathway toward the Development of usable, diffusible tools and processes. There is a need in academic medical centers tasked with Digital innovation for a comprehensive process model to guide Development that incorporates current industry trends, including design thinking and lean and agile approaches to Digital Development. Objective This study aims to describe the foundations and phases of our model for user-centered HIT Development. Methods Based on our experience, we established an integrated approach and rigorous process for HIT Development that leverages design thinking and lean and agile strategies in a pragmatic way while preserving methodological integrity in support of academic research goals. Results A four-phased pragmatic process model was developed for user-centered Digital Development in HIT. Conclusions The model for user-centered HIT Development that we developed is the culmination of diverse innovation projects and represents a multiphased, high-fidelity process for making more creative, flexible, efficient, and effective tools. This model is a critical step in building a rigorous approach to HIT design that incorporates a multidisciplinary, pragmatic perspective combined with academic research practices and state-of-the-art approaches to Digital product Development to meet the unique needs of health care.

    Background: Despite a proliferation of patient-facing mobile apps for mental disorders, there is little literature guiding efforts to incorporate mobile tools into clinical care delivery and integrate patient-generated data into care processes for patients with complex psychiatric disorders. Objective: The aim of this study was to seek to gain an understanding of how to incorporate a patient-provider mobile health (mHealth) platform to support the delivery of integrated primary care–based mental health services (Collaborative Care) to rural patients with posttraumatic stress disorder and/or bipolar disorder. Methods: Using the Principles for Digital Development as a framework, we describe our experience designing, developing, and deploying a mobile system to support Collaborative Care. The system consists of a patient-facing smartphone app that integrates with a Web-based clinical patient registry used by behavioral health care managers and consulting psychiatrists. Throughout Development, we engaged representatives from the system’s two user types: (1) providers, who use the Web-based registry and (2) patients, who directly use the mobile app. We extracted mobile metadata to describe the early adoption and use of the system by care managers and patients and report preliminary results from an in-app patient feedback survey that includes a System Usability Scale (SUS). Results: Each of the nine Principles for Digital Development is illustrated with examples. The first 10 patients to use the smartphone app have completed symptom measures on average every 14 days over an average period of 20 weeks. The mean SUS score at week 8 among four patients who completed this measure was 91.9 (range 72.5-100). We present lessons learned about the technical and training requirements for integration into practice that can inform future efforts to incorporate health technologies to improve care for patients with psychiatric conditions. Conclusions: Adhering to the Principles for Digital Development, we created and deployed an mHealth system to support Collaborative Care for patients with complex psychiatric conditions in rural health centers. Preliminary data among the initial users support high system usability and show promise for sustained use. On the basis of our experience, we propose five additional principles to extend this framework and inform future efforts to incorporate health technologies to improve care for patients with psychiatric conditions: design for public health impact, add value for all users, test the product and the process, acknowledge disruption, and anticipate variability.     Abstract:

    Academic medical centers (AMCs) today prioritize Digital innovation. In efforts to develop and disseminate the best technology for their institutions, challenges arise in organizational structure, cross-disciplinary collaboration, and creative and agile problem solving that are essential for successful implementation. To address these challenges, the Digital DesignLab was created at NYU Langone Health to provide structured processes for assessing and supporting the capacity for innovative Digital Development in our research and clinical community. Digital DesignLab is an enterprise level, multidisciplinary, Digital Development team that guides faculty and student innovators through a Digital Development “pipeline”, which consists of intake, discovery, bootcamp, Development. It also provides a framework for Digital health innovation and dissemination at the institution. This paper describes the Digital DesignLab’s creation and processes, and highlights key lessons learned to support Digital health innovation at AMCs.

    An increasingly lamented consequence of re-regulatory efforts following the 2007/2008 global financial crisis has been ‘de-risking’—the growing disengagement by banks and other financial institutions with markets perceived as posing greater risks than justified by potential profits. As banks based in the Global North have moved to withdraw financial services from many emerging economies, de-risking has attracted attention for undermining financial inclusion and Developmental efforts. Novel technologies are being harnessed to address this problem, including applications of blockchain, the Digital ledgers of transactions originally underpinning cryptocurrencies. Contributing to IR theorising of legitimacy and re-risking, this article illustrates how technology-based de-risking efforts that seek to attend to the perceptions of foreign financiers can undermine the legitimacy of financial inclusion projects. Contrasting unfolding blockchain-based financial inclusion initiatives in two regions of small states in the Eastern Caribbean and Eastern Europe, our analysis stresses the need for greater local participation and clearer distribution of benefits from finance and technology (fintech) centred forms of Digital Development.
