In both products, the putative spliced leader 1 sequences (SL1) show some nucleotide substitutions compared to the canonical SL1 sequence of nematodes. SL1 is a short sequence transferred to the 5′ end of most messenger RNAs through a trans-splicing reaction. All the other filariae studied thus far amplify a single fragment showing a canonical SL1 sequence. D. repens appears thus peculiar.

The PCR-based identification was found to be unambiguous and allowed specific diagnosis of microfilariae in blood samples, of developing larvae in the mosquito vector and of immature adults in bioptic material, overcoming the serious constraints of the morphological separation of these filarial parasites at the pre-adult stages. The technique was found to be very sensitive and applicable to samples stored either dry or in various preservation media, with the exception of formalin. The reliable identification of D. repens and D. immitis from bioptic material is expected to greatly enhance the chances of detecting human infections and to further clarify the role of the two parasites as pathogens of man. The possibility of routine identification of developing larvae in the vector will substantially improve the perspectives for epidemiological investigations, particularly in Southern European regions, such as Italy, where the two nematode species are largely sympatric.

Parasitology Research, 2016Co-Authors: Carla Maia, Susanne Lorentz, Luis Cardoso, Domenico Otranto, Torsten J NauckeAbstract:Dirofilaria repens causes subcutaneous infection in dogs and cats, and is the main agent of human dirofilariosis in Europe. Detection of D. repens is described in a dog from Portugal, a finding that simultaneously represents the first presumable case of autochthonous infection with the parasite in any animal host species in the country. A mixed D. repens/Dirofilaria immitis infection (20:1 ratio) was found, with an overall density of 1267 microfilariae per millilitre of blood. Morphological features, including morphometry, and acid phosphatase histochemical staining confirmed identity of the two filarioids. Distribution of D. repens in Portugal should be further assessed at the population level both in vertebrate hosts and in vectors. Due to the zoonotic potential of the parasite, preventive measures such as the administration of microfilaricides and insect repellents should be put into practice to protect animals and public health.

A case of human Dirofilaria repens infection, causing an asymptomatic subcutaneous nodule

The diagnosis was confirmed by ultrasound and histology of the excised subcutaneous nodule. The parasite species was identified as Dirofilaria repens by polymerase chain reaction. We expect to see more cases of human Dirofilariasis also due to climate change and associated increase of the spectrum of suitable mosquito vectors.
