    Abstract Background Investigators have hypothesized that sodium-glucose cotransporter 2 (SGLT2) inhibitors exert Diuretic effects that contribute to their ability to reduce serious heart failure events, and this action is particularly important in patients with fluid retention. Objectives This study sought to evaluate the effects of the SGLT2 inhibitor empagliflozin on symptoms, health status, and major heart failure outcomes in patients with and without recent volume overload. Methods This double-blind randomized trial compared the effects of empagliflozin and placebo in 3,730 patients with heart failure and a reduced ejection fraction, with or without diabetes. Approximately 40% of the patients had volume overload in the 4 weeks before study enrollment. Results Patients with recent volume overload were more likely to have been hospitalized for heart failure and to have received an intravenous Diuretic Agent in an outpatient setting in the previous 12 months, and to experience a heart failure event following randomization, even though they were more likely to be treated with high doses of a loop Diuretic Agent as an outpatient (all p  0.05). Changes in body weight, hematocrit, and natriuretic peptides (each potentially indicative of a Diuretic action of SGLT2 inhibitors) did not track each other closely in their time course or in individual patients. Conclusions Taken together, study findings do not support a dominant role of diuresis in mediating the physiological changes or clinical benefits of SGLT2 inhibitors on the course of heart failure in patients with a reduced ejection fraction.     Abstract The Diuretic activity of the stem-bark extracts of Steganotaenia araliacea (SbESa) and effects on urine electrolytes in rats was studied. Furthermore, a toxicological effect of the SbESa on several tissues was investigated. Groups of male Wister albino rats (170 ± 0.77 g) were employed. Four doses of 20 mg/kg body weight (b.w.), of SbESa (water, methanol, ethanol) and furosemide were administered intraperitoneally (IP). The control group received normal saline alone by oral administration. The 24-h urine outputs per day (in ml) were: normal saline (1.57 ± 0.11); water extract (3.18 ± 0.24); methanol extract (3.22 ± 0.29); ethanol extract (3.62 ± 0.27) and furosemide (4.22 ± 0.23). The urine output among the extracts (water, methanol, ethanol) and the furosemide against the control was statistically significant, (P  The authors conclude that though there are compelling evidence of Diuretic potentials in the use of the stem-bark of Steganotaenia araliacea, the toxic effects on vital organs is a drawback to its recommendation for use as a Diuretic Agent.

    Abstract Ethnopharmacological relevance Smilax canariensis is an endemic species of the Canary Islands, popularly known as “Zarzaparrilla sin espinas”. This species has wide use in folk-medicine practice on the islands, especially as Diuretic. So the aim of our study is to evaluate the Diuretic activity of an aqueous and a methanol extract of this species. Material and methods Three infusions doses (250, 500 and 750 mg/kg) and two methanol extract doses (100 and 200 mg/kg) were orally administered to laboratory rats. Water excretion rate, pH, density, conductivity, and content of Na + and K + were measured in the urine of saline-loaded rats. Results Water excretion rates were significantly increased in a dose-dependent manner by both hot water infusions and the alcohol extract. The electrolytic excretion was also dose-dependent, although potassium excretion was markedly reduced when using the alcohol extract compared with that observed for the infusion. Conclusions Smilax canariensis presents a notable Diuretic effect which appeared to be related both to its potassium content and to the presence of polar organic compounds. The present results provide a quantitative basis explaining the traditional folk-medicine use of Smilax canariensis as a Diuretic Agent by the Canary Island population.

    This study reports on the pharmacological evaluation of the Diuretic activity of an infusion and a methanol extract of Withania aristata Ait. in laboratory rats. Water excretion rate, pH, density, conductivity, and content of Na + ,K + and Cl − were measured in the urine of rats subjected to hypersaline conditions. Both the infusion and the methanol extract showed a significant Diuretic effect compared with non-treated controls, with notable increases in the rates of water and sodium excretion. There was also a potassium retention effect. The Diuretic effect did not appear to be related to the potassium content in the material tested, but did have some relation to its content of active polar compounds. The results justify the use of Withania aristata as a Diuretic Agent in folk medicine of the Canary Islands. © 2007 Elsevier Ireland Ltd. All rights reserved.

    Abstract A pharmacological evaluation for Diuretic activity of infusions at 5, 10 and 15% of Artemisia thuscula Cav. was carried out. Urinary excretion of water, pH, density, conductivity and Na + , K + and Cl − content were investigated in saline-loaded rats. The infusions showed a dose-dependent decrease Diuretic effect, but augmented significantly with respect to the control group for the urinary excretion of water and sodium. Furthermore, a potassium-sparing effect at 5 and 10% was showed. The Diuretic effect does not seem to be related to the potassium content of the starting material. The results justify the use of Artemisia thuscula as Diuretic Agent by the canary traditional medicine.

  • In-Hospital Diuretic Agent Use and Post-Discharge Clinical Outcomes in Patients Hospitalized for Worsening Heart Failure: Insights From the EVEREST Trial.

    Abstract Objectives The aim of this study was to characterize the association between decongestion therapy and 30-day outcomes in patients hospitalized for heart failure (HF). Background Loop Diuretic Agents are commonly prescribed for the treatment of symptomatic congestion in patients hospitalized for HF, but the association between loop Diuretic Agent dose response and post-discharge outcomes has not been well characterized. Methods Cox proportional hazards models were used to estimate the association among average loop Diuretic Agent dose, congestion status at discharge, and 30-day post-discharge all-cause mortality and HF rehospitalization in 3,037 subjects hospitalized with worsening HF enrolled in the EVEREST (Efficacy of Vasopressin Antagonism in Heart Failure: Outcome Study With Tolvaptan) study. Results In univariate analysis, subjects exposed to high-dose Diuretic Agents (≥160 mg/day) had greater risk for the combined outcome than subjects exposed to low-dose Diuretic Agents (18.9% vs. 10.0%; hazard ratio: 2.00; 95% confidence interval: 1.64 to 2.46; p  Conclusions Short-term Diuretic Agent exposure during hospital treatment for worsening HF was not an independent predictor of 30-day all-cause mortality and HF rehospitalization in multivariate analysis. Congestion status at discharge did not modify the association between Diuretic Agent dose and clinical outcomes.
