Colonic Diverticulosis is associated with abdominal wall hernia

Studies have suggested that connective tissue alterations play a role in the formation of both Diverticulosis and abdominal wall hernia. The aim of this cohort study was to evaluate the association between Diverticulosis and abdominal wall hernia in a large cohort of patients undergoing colonoscopy. Methods All consecutive patients who underwent colonoscopy between 2001 and 2013 at Bispebjerg Hospital were eligible for inclusion. The endoscopists prospectively registered the findings of Diverticulosis in a database. Data were merged with the Danish Hernia Database identifying patients who underwent groin and ventral hernia repair. Calculated odds ratios (ORs) were used to describe the extent of association and multivariable logistic regression models were utilized to adjust for age and gender. Results A total of 13,855 patients were included, 3685 (26.6%) of whom were diagnosed with Diverticulosis. Diverticulosis was independently associated with direct inguinal, OR 1.33, 95% CI 1.00–1.76, P = 0.049, and umbilical/epigastric hernia repair, OR 1.74, 95% CI 1.16–2.63, P = 0.008. Conclusions Colonic Diverticulosis was associated with direct inguinal and umbilical/epigastric hernia repair suggesting that connective tissue alterations, herniosis , could be a common etiologic factor of colonic Diverticulosis and these abdominal wall hernias.

Compagna, Maria Rosaria Chierchia, Fabiana CastiglioneAbstract:Colonic Diverticulosis is characterized by abnormal thickening of the bowel wall, associated with luminal overpressure and increase of sigmoid contractility. However, patients with ulcerative colitis show chronic inflammatory alterations determining a reduction of both bowel wall muscle tone and contractility. Thus, we could presume ulcerative colitis and colonic Diverticulosis as two pathophysiologically and mutually excluding diseases. This study was designed to evaluate the prevalence of colonic Diverticulosis in patients with ulcerative colitis compared with a control endoscopic population. We prospectively analyzed the prevalence of colonic Diverticulosis in 85 patients, older than aged 45 years, with known ulcerative colitis compared with that in 85 age/gender-matched patients without colitis. All patients underwent pancolonoscopy with ulcerative colitis and colonic Diverticulosis diagnosis made by endoscopy and histopathology. The patients with ulcerative colitis also were divided in three subgroups according to the age at diagnosis ( 45 years) and extension of disease (sigmoiditis, left colitis, extensive colitis). Midgut volvulus due to jejunal diverticula: a case report.

It can produce significant complications, and some complications are potentially life threatening and require early surgical treatment, such as obstruction, hemorrhage and perforation. There is no consensus on the management of this disease. Only a few cases of jejunal Diverticulosis with midgut volvulus have been reported. We herein report a case of 57-year-old woman with jejunal Diverticulosis causing small bowel volvulus who complained of intermittent upper abdominal pin-prick for 5 years that eventually progressed to a complete obstruction. The computed tomography scans revealed a mesenteric vessel "whirlpool" and laparotomy showed midgut volvulus secondary to jejunal diverticula. This case highlights jejunal Diverticulosis causing small bowel volvulus as an uncommon mechanism of small bowel obstruction, which should be included in the differential diagnosis of small bowel obstruction.
