    Abstract:

    Abstract A list of 144 words was presented to 15 school-age stutterers who were instructed to mark those words on which they would expect to be dysfluent if they were to readthem aloud. Subsequently, a computer-controlled eyemarker recorded the subjects' saccadic movements as they silently read a passage made up of these words. Then, they were videotaped as they orally read the passage. The childrens' expectations were not predictive of the words on which stuttering occured. Moreover, during silent reading, the subjects did not fixate more frequently on words subsequently stuttered than on those later spoken fluently. However, the duration of eye fixations during silent reading were longer for words that were subsequently stuttered than they were for those later spoken fluently. The latter finding suggest that school-aged children, like adults, show evidence of word-specific anticipation of Dysfluency.

    Abstract:

    This case report demonstrates challenges diagnosing psychosis in language dysfluent deaf patients. Treatment of a 34-year-old deaf man on an inpatient psychiatric unit is described. He had a history of physical aggression and possible symptoms of paranoia and thought disorganization, in addition to learning difficulties and minimal language skills. The patient was placed on a combined hearing/deaf inpatient unit, received specialized programming for deaf patients and was prescribed risperidone and divalproex sodium to treat his aggressive behavior and possible psychosis. Uncertainty if the patient were having psychotic symptoms remained throughout his hospitalization, although he improved behaviorally and was discharged after 13 months of treatment. The patient’s pre-existing language deficits made accurate diagnosis and appropriate treatment challenging.   • Identifying and Assessing Psychosis in Deaf Psychiatric Patients

    Abstract:

    This article reviews recent research in the area of psychotic disorders in deaf psychiatric patients. Comparisons of the rates of psychotic disorders in the deaf and hearing populations suggest that psychotic disorders occur equally as often or even somewhat less often in the deaf population as in the hearing population. Consideration is given to the limitations of this small body of research, and recommendations for future research are provided. The contradictory literature on hallucinations in the deaf is also reviewed, and current theory about the manner in which hallucinations are manifested and experienced based on audiological history is presented. The challenges encountered by clinicians in accurately assessing symptoms of thought disorganization in deaf people are reviewed. Specifically, the etiology of deafness, language Dysfluency, and the skill and training of American Sign Language interpreters are considered as factors impacting accurate diagnosis. Recommendations from the current literature are also provided.

  • Speech Disfluency and Autism in Schools: Identifying Needs and Providing Support to SLPs☆

    Abstract:

    Abstract In recent years, there has been a gradually increasing body of literature documenting patterns of disfluency in samples of children on the autism spectrum (see Scaler Scott, Tetnowski, Flaitz, &Yaruss, 2014, for review). This study discusses the results of two surveys. The first survey, discussing 395 students, demonstrates the identification of students with autism and disfluency among school-based speech-language pathologists (SLPs) in three areas of the United States of America. This increased documentation suggests growing concern about treatment methods for disfluencies within this population. The second part of this study discusses a pilot training conducted with one school district in the United States. SLPs were surveyed regarding their knowledge and confidence level in identifying and working with fluency disorders in students with autism. The survey was conducted before and after a training seminar on fluency disorders in children with autism. Three months after training, the SLPs in that district were resurveyed to determine the numbers of cases identified and their comfort level with identifying, evaluating, and treating disfluencies. Despite increases in comfort following training, participants indicated that they were less than comfortable with procedures of referral and treatment. Implications for future SLP training will be discussed.

    Abstract This paper will present the latest information regarding what is known and unknown about the presence, possible causes, and potential effective treatments of fluency disorders in Autism Spectrum Disorders (ASDs). Advanced review of cognitive features of ASDs which may play a role in contributing to dysfluencies in this population will be discussed. Examples of practical application of existing information to evaluation and treatment will be presented.

    Background In recent years, there has been increased identification of disfluencies in individuals with autism, but limited examination of disfluencies in the school-age range of this population. We currently lack information about whether the disfluencies of children with autism represent concomitant stuttering, normal disfluency, excessive normal disfluency, or some form of disfluency unique to the school-age population of children with autism. Aims This paper explores the nature of disfluencies in school-aged children with autism in comparison with matched children who stutter and controls. It explores stuttering-like disfluencies, non-stuttering-like disfluencies and word-final disfluencies. Methods & Procedures This study compared disfluency patterns in 11 school-aged children with Asperger's syndrome (AS), 11 matched children who stutter (CWS), and 11 matched children with no diagnosis (ND). Analyses were based on speech samples collected during an expository discourse task. Outcomes & Results Results reveal statistically significant differences between children with AS and CWS and between children with AS and those with ND for the percentage of words containing stuttering-like disfluencies. In the AS group, four out of 11 (36%) met the common diagnostic criteria for a fluency disorder. Disfluencies in the AS group differed qualitatively and quantitatively from the CWS, and included a larger distribution of word-final disfluencies. Conclusions & Implications This study provides initial data regarding patterns of disfluency in school-aged children with AS that, with careful consideration and the cautious application of all findings, can assist therapists in making more evidence-based diagnostic decisions. Findings offer evidence that when working with children with AS, disfluencies both similar and dissimilar to those of CWS may be identified in at least a subset of those with AS. Therefore, children with AS should be screened for fluency disorders during their initial evaluation and treated if it is determined that the fluency disorder negatively impacts the effectiveness of communication.

    Method: This cross-sectional descriptive-analytic study was performed on 14 adults who stuttered. Their average age was 25 years. The frequency of non-fluency instances was evaluated upon reading two lists containing 60 words and 60 non-words. The words were selected on the basis of common Persian syllable structures. ‘Kolmogoro-Smirnov one sample test’ and paired t-test was used to analyze data; the significance level was set at P < 0.05. Results: There was a significant difference between the Dysfluency in word and non-word lists (p<0.05). Conclusion: The findings of this study indicate a significant increase in the frequency of Dysfluency on non-words than on real words. It seems that the phonological encoding process of non-word reading is much more complex than for word reading, because, in non-word reading, the component of semantic content retrieval (word meaning) is missing when compared to word reading.

The obtained results can pave the way to reach a better understanding of stuttering, as well as finding more appropriate treatments. This study investigates the effect of utterance length on speech Dysfluency comparing stuttering Persian(Farsi) -speaking children and nonstuttering ones. Methods: Ten stuttering and ten nonstuttering Persian-speaking children between 4 and 6 years of age, matched by age and gender where included in this cross-sectional study. All individuals were investigated by ten sets of simple and complex sentences. Morphemes were added one by one to sentences in each set in order to assess the impact of utterance length on Dysfluency. Data analysis was performed using SPSS software. Results: In practicing ten sets of sentences, the speech Dysfluency shows significant enhance with increase of utterance length in both stuttering and nonstuttering children (p=0.001). Furthermore, while comparing two groups of simple and complex sentences, significant increase of speech Dysfluency was observed by length enhance of utterance, in both stuttering and nonstuttering children (p<0.05). The comparison of stuttering and nonstuttering children showed significant difference between the two groups as well (p=0.001). Conclusions: The results of this study show that increase in utterance length in both simple and complex sentences lead to increase in Dysfluency in stuttering and nonstuttering children. Stuttering children indicated more Dysfluency than nonstuttering ones.

  • Stuttering induced by thalamic deep brain stimulation for dystonia

    Abstract:

    We report a patient in whom deep brain stimulation of the ventral intermediate nucleus of the thalamus (Vim) for treating dystonia reversibly induced stuttering at suboptimal stimulation parameters. Adjustments of stimulation parameters resulted in excellent control of dystonic motor symptoms and complete resolution of speech Dysfluency. This is the first report on stuttering as an adverse effect of Vim stimulation which is primarily used to treat tremors of various etiologies.
