    A variety of tumors develop or exacerbate during pregnancy and it is well known that a noticeable increase of Eccrine activity occurs in this period. Eccrine Poroma is a benign neoplasm that originates from the Eccrine sweat gland. The development of this tumor during pregnancy is extremely infrequent. We report a rare case of rapidly growing Eccrine Poroma in an unusual location (the nose) in a pregnant woman.

    Congenital polypoid pigmented Eccrine Poroma of a young woman Editor Eccrine Poroma (EP) usually occurs in middle- aged or elderly people and is very rare in children. Only threecases of EP in childhood have been reported, and one ofthese cases evolved rapidly into an Eccrine porocarcinoma in 2 years. 2,3 To the best of our knowledge, no congenital case has been reported yet.A 30-year-old woman presented with an asymptomatic pigmented tumour. This lesion was noted since birth, with a tendency of gradual enlargement (fig. 1 a). She appeared otherwise healthy , and no family history of similar condition was obtained. On physical examination, a polypoid blackish tumour, which measured 1 cm in diameter, was seen on the right lower back. Histopathology revealed a tumour composed of lobules of uniform cuboidal basaloid cells with significant melanin deposition and scattered dendritic melanocytes that were highlighted with Fontana–Masson stain. Occasional Eccrine ductule formation could be seen. The tumour showed not only a dermal nodular growth pattern but also connection to the epidermis. Neither atypical mitoses nor other signs of malignant transformation were found with scrutiny(fig. 1b–d). The pathogenesis of EP is unknown, but some cases may be associated with trauma. Intrauterine trauma may be the possible mechanism for this patient to develop a congenital EP. The colour of EP varies from flesh to black, and pigmented EP is uncommon. Both EP and Eccrine porocarcinoma may contain melanin and melanocytes. Although the primordia of Eccrine duct contain melanocytes during the 14th week of gestation, the melanocytes are lost in the late embryonic life. It has been shown that melanocytes in pigmented EP may originate from the adjacent epidermis. Endothelin-1 expression is up- regulated in the tumour cells, with consequence of migration and proliferation of epidermal melanocytes. Melanocyte colonization may be an indicator of malignant transformation of EP andEccrine porocarcinoma, which develops from a pre- existing EP in nearly half of the cases.Long-standing solitary EP is a risk factor for malignant degeneration.This case was a rare congenital pigmented EP with peculiar polypoid appearance. Considering the possibility of carcinomatous change, the lesion was excised totally, and this was the treatment of choice. The outcome is good without recurrence

  • Treatment of advanced malignant Eccrine Poroma with locoregional chemotherapy

    Abstract:

    Malignant Eccrine Poroma is a rare disease with approximately 200 cases reported in the literature. Regional cutaneous and systemic metastases are rarely observed and their management has been generally unsuccessful. We report on a case in which topical 5-fluorouracil application and intra-arterial chemotherapy with docetaxel resulted in a histologically confirmed complete response of multiple regional skin metastases for more than 2 years. Despite intravenous administration of docetaxel, slow progression of systemic disease was observed.
