We searched PubMed (1966–December 2011), Embase (1974–December 2011) and the Cochrane Library Database (2011, Issue 12). The assessed outcome measures were the change from baseline for the International Prostate Symptom Score (IPSS), quality of life (QoL) score, peak urine maximum flow rate (Qmax), QoL related to urinary symptoms and adverse effects. Two authors independently assessed the study quality and extracted data. All data were analysed using RevMan 5.1. The meta-analysis included four randomized controlled trials with a total of 2504 patients. The study durations were each 12 weeks. At the follow-up end points, the pooled results showed that the change from baseline for the silodosin group was significantly higher than the placebo group for the IPSS, QoL score and Qmax(mean difference (MD)=−2.78, P<0.00001; MD=−0.42, P=0.004; MD=1.17, P<0.00001,respectively) and patients felt more satisfied with QoL related to urinary symptoms in the silodosin group than the placebo group. Ejaculation Disorder was the most commonly reported adverse effect. The pooled results also showed that the silodosin group was superior to the 0.2 mg tamsulosin group with respect to the IPSS and QoL score (IPSS: MD=−1.14, P=0.02; QoL score: MD=−0.26, P=0.02) and inferior to the 0.2 mg tamsulosin group with respect to Qmax (MD=−0.85, P=0.01). In contrast, there was no significant difference in the incidence of Ejaculation Disorder and dizziness between the silodosin and 0.2 mg tamsulosin groups. The current meta-analysis suggested that silodosin is an effective therapy for LUTS in men with BPH and is not inferior to 0.2 mg tamsulosin.

DESIGN: Double-blind, parallel-group, placebo-controlled, multicentre, prospective randomized study. SETTING: Outpatient care in 28 centres across Canada. PARTICIPANTS: Men aged 45 to 80, in good health, with moderate BPH and no evidence of prostate cancer. A total of 613 men were entered into the study; 472 completed the 2 years of treatment. INTERVENTION: After 1 month of receiving a placebo (run-in period), patients were given either finasteride (5 mg/d) or a placebo for 2 years. OUTCOME MEASURES: Efficacy: changes from baseline in BPH symptom scores, maximum urinary flow rates and prostate volume. Safety: onset, course and resolution of all adverse events during the treatment period. RESULTS: In the efficacy analyses the mean BPH symptom scores decreased 2.1 points (from 15.8 to 13.7) in the finasteride group, as compared with a decrease of 0.7 points (from 16.6 to 15.9) in the placebo group (P < or = 0.01). The maximum urinary flow rate increased by a mean of 1.4 mL/s (from 11.1 to 12.5 mL/s) in the finasteride group, as compared with an increase of 0.3 mL/s (from 10.9 to 11.2 mL/s) in the placebo group (p < or = 0.01). The mean prostate volume decreased by 21% (from a mean volume of 44.1 cm3 at baseline) in the treatment group; it increased by 8.4% (from a mean volume of 45.8 cm3 at baseline) in the placebo group (p < or = 0.01). In the safety analysis, the proportion of patients who experienced any adverse event was similar in the two groups (81.0% in the treatment group and 81.2% in the placebo group). However, the incidence of adverse events related to sexual dysfunction were significantly higher in the finasteride group than in the placebo group (Ejaculation Disorder 7.7% v. 1.7% and impotence 15.8% v. 6.3%; p < or = 0.01 for both parameters). CONCLUSION: Finasteride is a well-tolerated and effective alternative to watchful waiting in the treatment of moderate BPH.

Ejaculation Disorder Is Associated With Increased Efficacy of Silodosin for Benign Prostatic Hyperplasia
Urology, 2010Co-Authors: Yukio Homma, Kazuki Kawabe, Masayuki Takeda, Masaki YoshidaAbstract:Objectives To provide clinical evidence that Ejaculation Disorder caused by selective alpha 1A -blockers may be associated with larger symptomatic improvements in patients with benign prostatic hyperplasia. Methods Post hoc analyses of data from a randomized, double-blind, placebo-controlled clinical trial of silodosin in Japan were performed. Subjects were randomized into 3 treatments: silodosin (4 mg twice daily), tamsulosin (0.2 mg once daily), or placebo. For statistical analysis, subjects receiving silodosin were stratified by the presence or absence of Ejaculation Disorder. Efficacy was assessed using total International Prostate Symptom Score (IPSS), IPSS subscores, 25% reduction in total IPSS, and quality of life score. Safety was assessed by frequency of adverse drug reactions (ADRs) and silodosin discontinuation. Results The silodosin subgroup with Ejaculation Disorder (SIL+EjD) showed larger change in total IPSS than the silodosin subgroup without Ejaculation Disorder (SIL-EjD) (difference: −4.36 [95% CI: −6.44 to −2.27]) and the placebo group (difference: −6.29 [95% CI: −8.44-−4.14]). Remarkable improvement was observed at all time points. The success rate in SIL+EjD was higher than in SIL-EjD and placebo when measured using a 25% reduction in the total IPSS category. There were no significant differences in ADR rates other than Ejaculation Disorder. Discontinuation rates between SIL+EjD and SIL-EjD were similar. Conclusions Our results suggest that Ejaculation Disorder caused by selective alpha 1A -blockers is associated with very large improvements in lower urinary tract symptoms. Patients with Ejaculation Disorder may have larger symptomatic improvements without incremental risk for adverse events.
