    Abstract Objectives The study sought to characterize the efficacy of stellate ganglion block (SGB) as a treatment for Electrical Storm (ES). Background The efficacy of percutaneous SGB for managing ES is not well understood. Methods The authors conducted published data searches using PubMed/Medline and Google Scholar for mixed combinations of terms including stellate ganglion block, *ganglion block(ade), sympathetic block(ade), and arrhythmia, ventricular arrhythmia (VA), or tachycardia, ventricular fibrillation, Electrical Storm. Inclusion criteria were presentation with guideline-defined ES and treatment with SGB. Exclusion criteria were presentation with any supraventricular arrhythmia, VA without ES, or surgical sympathectomy. Studies lacking basic demographic data, arrhythmia description, and outcomes were excluded. Results Of 3,374 publications reviewed, 38 patients from 23 studies met study criteria (52.0 ± 19.1 years of age, 11 women, 17 with ischemic cardiomyopathy). Antiarrhythmics were used in all patients. Mean left ventricular ejection fraction was 31 ± 10%. ES was triggered by acute myocardial infarction in 15 patients and QT prolongation in 7 patients. The most common local anesthetic used for SGB was bupivacaine (0.25% to 0.50%). SGB resulted in a significant decrease in VA burden (12.40 ± 8.80 episodes/day vs. 1.04 ± 2.12 episodes/day; p  Conclusions SGB is an effective acute treatment for ES. However, larger prospective randomized studies are needed to better understand the role of SGB in ES and other VAs.

    Background— The occurrence of ventricular tachyarrhythmias seems to follow circadian, daily, and seasonal distributions. Our aim is to identify potential temporal patterns of Electrical Storm (ES), in which a cluster of ventricular tachycardias or ventricular fibrillation, negatively affects short- and long-term survival. Methods and Results— The TEMPEST study (Circannual Pattern and Temperature-Related Incidence of Electrical Storm) is a patient-level, pooled analysis of previously published data sets. Study selection criteria included diagnosis of ES, absence of acute coronary syndrome as the arrhythmic trigger, and ≥10 patients included. At the end of the selection and collection processes, 5 centers had the data set from their article pooled into the present registry. Temperature data and sunrise and sunset hours were retrieved from Weather Underground, the largest weather database available online. Total sample included 246 patients presenting with ES (221 men; age: 65±9 years). Each ES episode included a median of 7 ventricular tachycardia/ventricular fibrillation episodes. Fifty-nine percent of patients experienced ES during daytime hours ( P P P =0.003). Conclusions— ES incidence is not homogenous over time but seems to have a clustered pattern, with a higher incidence during daytime hours and working days.     Abstract:

    lectrical Storm, characterized by clustering of ventricular achyarrhythmic episodes, is associated with an ominous rognosis. Serial antiarrhythmic therapy, beta-blockade, atheter ablation, and left stellate ganglion blockade have all een used for acute management in this setting. Thoracic pidural anesthesia is a therapeutic option that selectively tarets nerve fibers that innervate the myocardium. Thoracic pidural anesthesia has been used for management of refracory angina. We postulated that thoracic epidural anesthesia ight be useful for management of Electrical Storm by reducng the cardiac sympathetic drive. We report the case of a atient who presented with Electrical Storm. All conventional ethods for management of arrhythmia control were ineffecive. Thoracic epidural anesthesia effectively managed ventriclar arrhythmias in this patient.
