Postoperative pain intensity associated with the use of different nickel-titanium shaping systems during single-appointment Endodontic Retreatment: a randomized clinical trial

METHOD AND MATERIALS Between September 2016 and December 2016, 99 patients with asymptomatic root canal-treated teeth requiring nonsurgical Endodontic Retreatment were randomly divided into three groups (n = 33 per group). After removing previous root canal filling, instrumentation was performed using One Shape, Revo-S, and WaveOne systems in groups 1, 2, and 3, respectively. Postoperative pain intensity was assessed at 6, 12, 18, 24, 48, and 72 hours, 7 days, and 1 month after the Retreatment. Data were analyzed using one-way ANOVA and Mann-Whitney U and Kruskal-Wallis tests (alpha = .01). RESULTS Up to 72 hours, postoperative pain was significantly less in group 1 than in groups 2 and 3 (P < .01). From 72 hours to 7 days, postoperative pain was significantly less in groups 1 and 2 (P > .05), compared to group 3 (P < .01). At 1 month, postoperative pain was not significantly different among all three groups (P > .05). Postoperative pain was the highest with WaveOne group. CONCLUSIONS Since One Shape and Revo-S are both based on the rotational approach and WaveOne on reciprocal approach, less incidence of postoperative pain intensity with One Shape and Revo-S in single-appointment nonsurgical Endodontic Retreatment could be associated with the motion type during root canal shaping

The presence of postoperative pain was assessed 1, 2, 3, and 7 days and 1 month after treatment. All 2-visit treatments were completed 1 week after the initial visit. Data were analyzed using the Mann-Whitney U, Kruskal-Wallis, and Pearson chi-square tests (α = 0.01, 0.05). Foram utilizados 120 pré-molares inferiores humanos, armazenados em solução de formol a 10% e divididos aleatoriamente em 6 grupos. Os dentes foram instrumentados e obturados, sendo em seguida, armazenados em estufa a 370C e umidade durante 60 dias. O retratamento foi realizado de acordo com as seguintes técnicas: limas Flexofile com Xilol; limas Flexofile e Hedstrom com Xilol; Profile GT com Xilol; Flexofile com clorexidina gel; limas Flexofile e Hedstrom com clorexidina gel; Profile GT com clorexidina gel. A extrusão apical de debris foi mensurada no decorrer dos procedimentos e a capacidade de limpeza das paredes por três procedimentos distintos: radiograficamente, através da lupa estereoscópica e do microscópio eletrônico de varredura (M.EV.). Os dentes foram radiografados no sentido orto e mesio-radial, sendo as radiografias analisadas quanto à presença de remanescentes de material obturador por dois examinadores simultaneamente. Em seguida foram fraturados longitudinalmente e as imagens capturadas através da lupa estereoscópica e transferidas ao computador. A quantidade de material obturador residual, foi mensurada utilizando o programa "Imagelab 98" e os resultados analisados estatisticamente. Cinco elementos de cada grupo foram analisados em M.E.V. nos 5 mm apicais. Os resultados indicaram que os grupos que utilizaram a clorexidina gel, com instrumentação manual ou rotatória, apresentaram menor extrusão de debris, sendo similar ao grupo com xilol e instrumentação rotatória, não diferindo estatisticamente entre si. Five elements of each group were observed in the SEM to compare the cleaning of the root canal walls, including the- apical thirds. The groups that used the chlorhexidine gel, with manual or rotary instrumentation, showed smaller debris extrusion, as well as the group with xylene and rotary instrumentation, not differing statistically to each other. Peculiarities of ultrastructure of tooth root canals treated with resorcinol formaldehyde resin, evaluation of influence of Endodontic Retreatment procedures and resistance to fracture. Study in vitro

In Lithuania this method was widely applied almost until 1994. Consequently, dentists are still struggling to resolve Endodontic Retreatment problems related to the use of this material in clinical practice. The present study gives an in-depth analysis of the overall impact of the combination of both machine-driven and ultrasound instruments on root dentin. In addition, the reliability of diagnostic methods (i.e. dyeing with methylene blue and inspection under microscope), which are commonly used in dental clinical practice in order to detect dentin cracks in teeth previously treated with RF paste when their colour differs from the neutral colour of the tooth tissue, have been tested in the study. Moreover, a study of resistance to fracture was carried out not only to evaluate the impact of load on root dentin after the Endodontic Retreatment but also to assess the level of the impact that dentin surface defects resulting from the Retreatment can have on the tooth strength. The results of the present study in vitro are difficult to be interpreted and directly related to the real clinical situation. Still, the results allow to make certain assumptions when it comes to the prognosis of Endodontic Retreatment of such teeth

Survival study on teeth after successful Endodontic surgical Retreatment: influence of crown height, root length, crown-to-root ratio and tooth type

Methodology: A single operator performed Endodontic microsurgery interventions between 2008 and 2018 on teeth with refractory apical periodontitis. The present analysis selected the teeth classified as “complete periapical healing” according to the scale suggested by Molven. The postoperative periapical radiographs and those taken at the last recall visit were analysed by two independent calibrated examiners, who measured crown height and root length in a blind manner. The crown-to-root ratio was calculated as the ratio of the two variables. The level of inter- and intra-operator agreement was tested with Bland–Altman plots with 95% limits of agreement. An independent statistician conducted a survival analysis using Kaplan–Meier plots and a log-rank test (α = 0.05) to assess the significance of the differences among the subgroups defined by the following criteria: (a) crown height median; (b) root length median; (c) crown-to-root ratio 1; (d) crown-to-root ratio median; (e) single-rooted teeth vs. multi-rooted teeth. Results: At the end of the analysis, 42 patients were evaluated, each one contributing to the study with a single tooth. The mean follow-up period was 4.2 ± 2.4 years.
