Implante secundario en pacientes afáquicos sin soporte capsular: lentes ancladas al iris

Si existeixen restes de càpsula el procés d’elecció per l’implant de la LIO és al sulcus ciliar. No obstant això el suport capsular molts vegades no s’observa. En aquests casos les alternatives quirúrgiques per l’implant de la LIO inclouen l’implant de la LIO a la càmera posterior mitjançant fixació escleral, l’implant de la lent a la càmera anterior amb lents de suport angular o l’implant de les lents de càmera anterior de fixació iridiana. A la tesi doctoral presento els resultats de dos estudis relacionats i complementaris, escrits i realitzats per mi, publicats en les revistes oftalmològiques British Journal of Ophtahlmology i Cornea, sobre la seguretat de l’opció quirúrgica que consisteix en implantar lents de fixació iridiana per corregir l’afàquia en els pacients que no presenten suport capsular. En general els resultats suggereixen que l’implant de les lents de fixació iridiana Artisan (Ophtec BV, Groningen, The Netherlands) en els pacients afàquics són una opció efectiva, previsible i segura a llarg termini, per tant i en base a la nostra experiència es presenten com una opció eficaç que considerem de primera elecció per a tractar aquest tipus de pacients. No obstante, es necesario un control monitorizado del paciente a lo largo de su vida mediante revisiones oftalmológicas periódicas que incluyan una exploración ocular completa, determinación de la presión intraocular y sobretodo un contaje endotelial.Secondary intraocular lens (LIO) implantation is often a challenging surgical procedure, as patients with aphakia usually have a history of eye trauma or complicated intraocular surgery. If capsular support is spared, LIO implantation in the ciliary sulcus represents the elective surgery. However, capsular support is often absent. In these cases, the current available surgical alternatives include: posterior chamber LIO implantation with scleral fixation, anterior chamber angle-supported LIO implantation, or anterior chamber iris-claw LIO implantation. The aim of this PhD work is presenting the results of two related and complimentary studies, performed and written by me, published in the journals British Journal of Ophthalmology and Cornea about the safety of secondary iris-claw LIO implantation in aphakic eyes without capsular support. The global results suggest that iris-claw Artisan lens (Ophtec BV, Groningen, The Netherlands) in aphakic eyes represent an effective, predictable and safe procedure at long-term. Therefore, in our experience we consider iris-claw lens as the first choice to treat these type of patients. Nevertheless, close monitoring of the patient with periodic ophthalmological revisions that include a complete ophthalmologic exam, intraocular pressure measurement and an Endothelium Cell count is mandatory

To validate the usefulness of this system, we applied it to the quantitative analysis of metabolic switching by the change of glucose concentrations in cultured bovine arterial Endothelium Cell, human umbilical vein Endothelium Cell and rat cardiac muscle Cell. In all of these Cell species, a decrease of glucose concentration increased total carbonate that represents the Cellular respiration activity and decreased free lactic acid that represents glycolysis activity. We firstly analyzed a switching of metabolic pathways from glycolysis to respiration with a decrease of glucose quantitatively. This ISFET system can be applicable to many biological investigations by analyzing metabolic activity and give dynamic Cellular information.

Ieej Transactions on Electronics Information and Systems, 2011Co-Authors: Akira Yamada, Satoshi Mohri, Michihiro Nakamura, Keiji NaruseAbstract:Our group previously reported the application of a flow-through type pH/CO2 sensor system designed to evaluate the metabolic activity of cultured Cells. The sensor system consists of two ion-sensitive field effect transistors (ISFETs), an ISFET to measure the total pH change and an ISFET enclosed within a gas-permeable silicone tube to measure the pH change attributable to CO2. In that study, we used the system to quantitatively analyze metabolic switching induced by glucose concentration changes in three cultured Cell types (bovine arterial Endothelium Cell (BAEC), human umbilical vein Endothelium Cell (HUVEC), and rat cardiomuscle Cell (RCMC)), and to measure the production rates of total carbonate and free lactic acid in the cultured Cells. In every Cell type examined, a decrease in the glucose concentration led to an increase in total carbonate, a product of Cellular respiration, and a decrease of free lactic acid, a product of glycolysis. There were very significant differences among the Cell types, however, in the glucose concentrations at the metabolic switching points. We postulated that the Cell has a unique switching point on the metabolic pathway from glycolysis to respiration. In this paper we use our sensor system to evaluate the metabolic switching of human embryonic kidney 293 Cells triggered by glucose concentration changes. The superior metabolic pathway switched from glycolysis to respiration when the glucose concentration decreased to about 2 mM. This result was very similar to that obtained in our earlier experiments on HUVECs, but far different from our results on the other two Cells types, BAECs and RCMCs. This sensor system will be useful for analyzing Cellular metabolism for many applications and will yield novel information on different Cell types.

intraocular architecture of secondary implanted anterior chamber iris claw lenses in aphakic eyes evaluated with anterior segment optical coherence tomography
British Journal of Ophthalmology, 2009Co-Authors: Michael Koss, Thomas KohnenAbstract:Aim: To evaluate the intraocular stability and safety of secondary iris-claw IOLs in aphakic patients. Methods: 18 eyes of 16 patients received iris-claw intraocular lenses (IOLs) to correct for aphakia. Primary outcome measurements included visual acuity (6m Snellen charts), central Endothelium Cell count (cECC) and intraocular position of the IOL assessed with anterior segment optical coherence tomography (OCT). Results: Sixteen patients (mean age 66 years; range 16-84 years, axial length 24.44mm) were re-examined 22 months postoperatively (range 12 - 38months). Preoperative decimal BCVA was 0.51, IOP 15.3mmHg, and central ECC 1816/mm 2 . Postoperative BCVA was 0.68, IOP 13.1mmHg, and central ECC 1626/mm 2 (difference over time 176/mm 2 = 10.5%, p>0.05). The anterior chamber depth was 4 mm for the eyes Conclusion: Secondary anteriorly iris-claw IOLs appear to be a safe choice to correct aphakia with no significant intermediate postoperative central Endothelium Cell loss especially in eyes ≥24mm, as distances of the IOL to the central and peripheral cornea proofed consistent.
