We have anecdotally observed, however, that many former Endourology fellows subspecialize within their practices. Recently the fellowship paradigm, accredited by the Endourological Society (EUS), has been modified to allow for emphasis on benign disease/kidney stones or laparoscopy/robotic surgery, which is heavily weighted toward oncology. In this study, we sought to assess the practice patterns of former Endourology fellows to evaluate various fellowship models. Methods: Email addresses for 320 of the 327 Endourology fellowship graduates between 2001 and 2010 identified by the EUS were obtained. These were cross-referenced with the American Urological Association (AUA) member database to maximize the yield of valid addresses. Merging of Endourology and Oncology: Endourooncology

SmithAbstract:There is a growing need to bring together endourological approaches and sound oncological principles to achieve minimally invasive cancer control in urologic malignancies. Reduced treatment morbidity and quality-of-life issues are becoming more valued by both patients and physicians. In this chapter, we present an overview of the most recent advances in endoscopic, laparoscopic, and energy-based ablation techniques for the treatment of upper-tract transitional-cell carcinoma, renal-cell carcinoma, prostate cancer, adrenal tumors, and germ-cell testicular tumors. The merge between Endourology and urologic oncology will undeniably revolutionize our approach to urologic malignancies.

This early version of the manuscript was not in its final form. A further revision has since submitted for re-review and has been accepted for publication. It is currently published in its Instant Online format (the raw, unedited manuscript) and carries a DOI of 10.1089/end.2012.0171. It will be included in the November 2012 issue of Journal of Endourology.

In a retrospective analysis during a 2-year period 236 patients treated with endourological procedures and 71 who underwent ESWL with a Siemens Lithostar††Siemens Medical Systems, Inc., Iselin, New Jersey. were analyzed as to the success rate, effectiveness quotient, complication rate and hospitalization. The mean stone size was 1.12cm. in the Endourology group and 1.03cm. in the ESWL group.Complete removal of all stone fragments was achieved in 93.6% of the patients treated endourologically and without retreatment. In the ESWL group success was obtained in 90.1%, with an 11.2% retreatment rate. The retreatment rate was higher (25.0%) for calculi in the mid ureter. However, the level of practice in public tertiary hospitals in developing countries is abysmally low. Objective : To review the current practice of Endourology in a Nigerian public tertiary hospital and discuss the challenges faced during the study period. Setting : This review was done at the urology unit of the Obafemi Awolowo University Teaching Hospitals Complex [OAUTHC], Ile-Ife, Nigeria. Materials and Methods : A retrospective review of medical notes of all patients who have had diagnostic and therapeutic endourological procedure in our practice between June 2002 and May 2004 was carried out Results : Forty nine medical notes were available for review. A total of 51 procedures were carried out, representing twenty percent of the total urological procedures done by the unit during the study period. The commonest indication for an endourological procedure was bladder outlet obstruction. The commonest procedure performed was urethrocystoscopy. There was one complication and one failed procedure. Conclusion : Although endourological applications have many advantages over open urological procedures, endourological practice is low due to lack of equipment personnel and funding. Advances in endourological practice are desired and this will require improvement in infrastructure, training of personnel and a whole reorientation in our choices of the care we give our patients. Retrograde endopyelotomy using the holmium laser, the use of intraluminal sonography, laparoscopic dismembered pyeloplasty, and the conundrum of lower pole stone management are just a sampling of some excellent presentation topics from the meeting. These continuing advances and innovations will help lead Endourology into the new millennium, offering the promise of improved patient care.

The following recent articles deal with important clinical applications in these fields. The current role of flexible nephroscopy is outlined, the clinical safety of diagnostic ureteroscopy for upper tract transitional cell carcinoma (TCC) is confirmed, and further data supporting the initial investigations into laparoscopic cryoablation of small renal tumors are presented.
