    Abstract:

    A 29-year-old man presents with intermittent Epigastric Discomfort, without weight loss or evidence of gastrointestinal bleeding. He reports no use of aspirin or nonsteroidal antiinflammatory drugs. A serologic test for Helicobacter pylori is positive, and he receives a 10-day course of omeprazole, amoxicillin, and clarithromycin. Six weeks later, he returns with the same symptoms. How should his case be further evaluated and managed?

  • Polypoid vascular and lymphatic malformation of the duodenum: a case report

    Abstract:

    Polypoid vascular and lymphatic malformation of the small intestine is rare. We report the case of a polypoid vascular and lymphatic malformation of the duodenum in a 58-year-old woman presenting with post-prandial Epigastric Discomfort. She did not have common symptoms of intestinal vascular and lymphatic malformation, such as gastrointestinal bleeding, steatorrhea, and hypoalbuminemia. A 5.0x0.8-cm semipedunculated polypoid lesion was found in the duodenal bulb with protrusion into the prepyloric antrum and was successfully removed using an endoscopic snare.

  • Angiography of visceral aneurysms

    Abstract:

    Visceral aneurysyms are rare vascular lesions. Five patients with such lesions seen in our hospital during the last decade are presented. Four patients presented with an acute intra-abdominal haemorrhage and three required an emergency laparotomy. In none of these patients could al aneurysms be detected at operation. One patient had a 4-month history of non-specific Epigastric Discomfort and a preoperative diagnosis was made with ultrasound. In all patients angiography was imperative for exact depiction of the aneurysms. Various diagnostic and therapeutic aspects are discussed and the persisting value of angiography emhasised.

    Abstract:

    A 65-year-old woman with no significant medical history visited the emergency department complaining of Epigastric Discomfort. A computed tomography of the thorax and abdomen showed the attenuation of the pulmonary artery and a posterior mediastinal hematoma. Angiography showed a tortuous esophageal branch of the left gastric artery and a pseudoaneurysm, and during the later phase, the left lower lobe of the lung was enhanced, and finally, the left pulmonary vein was enhanced. We considered that the patient was exhibiting hypoperfusion of the left pulmonary artery arising from left pulmonary artery hypoplasia, since the left lung was supplying the systemic circulation. Transcatheter arterial embolization was performed. The patient has not experienced any recurrence of a ruptured pseudoaneurysm or Epigastric Discomfort. Here, we report the first documented case of pulmonary artery hypoplasia associated with posterior mediastinal hematoma accompanied by a ruptured aneurysm of the left gastric artery.
