Se comenta su ciclo de vida, y aspectos nomenclaturales. Se hace una revisión histórica de como ha evolucionado el conocimiento de este hongo. Finalmente se analiza en España y Europa la enfermedad que provocaba la ingestión de pan elaborado con harina de centeno contaminada con Claviceps purpurea, conocida como Ergotismo o fuego de San Antonio.A review on the parasitic fungus Claviceps purpurea, known in Spain with the common name of “cornezuelo del centeno” is given. Its life cycle and nomenclatural aspects are commented. A historical review of how it has evolved is provided. Finally, the disease caused by the ingestion of bread made with rye flour contaminated with ergot (known as Ergotism or Saint Anthony’s fire) is presented here

Ergotism associated with HIV antiviral protease inhibitor therapy

CeraldiAbstract:Abstract Ergotism is a rare condition of acute vasospasm found classically in young and middle-aged women taking ergot alkaloid agents to treat migraine headache. We report the case of a young man with human immunodeficiency virus (HIV) positivity and describe the drug interaction between protease inhibitors and ergot alkaloid agents, which most likely predisposed to development of ergot toxicity. The HIV-positive population receiving antiviral therapy may be an underrecognized group at risk for Ergotism through decreased hepatic metabolism of ergot preparations. (J Vasc Surg 2003;37:676-8.)

Ergotamine, typically used to treat migraine, has less than 5% bioavailability due to extensive first-pass metabolism by cytochrome P450 3A4 (CYP3A4). Concurrent intake of ergotamine and strong CYP3A4 inhibitors, such as the HIV protease inhibitors (PIs), can lead to clinical Ergotism. A total of 13 cases of clinical Ergotism in HIV-infected patients has been published since 1997 (most recently reviewed by Frohlich et al).
