hypereosinophilic dermatitis like Erythema Annulare Centrifugum in a patient with chronic lymphocytic leukaemia
Journal of The European Academy of Dermatology and Venereology, 2005Co-Authors: J Miljkovic, I BartenjevAbstract:A 65-year-old male presented with a history of disseminated Erythema Annulare Centrifugum lesions and a high peripheral blood eosinophilia (55%). Histopathology of a lesion revealed a superficial and deep perivascular and interstitial inflammatory eosinophilic infiltrate. The bone marrow aspirate showed a heavy eosinophilic infiltrate. There was no evidence of any other systemic involvement except for bone marrow eosinophilia. The patient had a 6-year history of chronic lymphocytic leukaemia. The disease is in complete remission for the last 3 years. The clinical and histopathological findings in the reported patient correspond to the diagnosis of hypereosinophilic dermatitis. Topical treatment with corticosteroid creams was successful. The cutaneous lesions gradually resolved in 8 weeks, within 7 months the peripheral blood eosinophilia slowly declined from 55% to 7%.

ISSN 0001-5555 Sir, Figurate Erythemas are classified as Erythema Annulare Centrifugum (EAC), Erythema gyratum repens, Erythema migrans and necrolytic migratory Erythema. Differential diagnoses are mycosis fungoides, urticaria, granuloma Annulare and pseudolymphoma. EAC consists of recurrent and persistent Erythematous eruptions or urticarial papules forming annular or serpentiginous patterns with an advancing macular or raised border and scaling on the inner aspect of the border. EAC was first described by Darier in 1916, and classi fied in 1978 by Ackerman into a superficial and a deep type (1, 2). EAC can be associated with a wide variety of triggers including infections, food and drug ingestion, endocrinological conditions and as a paraneoplastic sign. A diagnosis of EAC should be followed by diagnostic workup because it may result in the discovery of an underlying disease. We describe here a 68yearold man affected by EAC who developed small Bcell nonHodgkin’s lymphoma 7 years later.

Erythema Annulare Centrifugum report of a case with neonatal onset

We report a case of EAC with neonatal onset and a clinical course of over 20 years. The patient presented from the 6th day of life with Erythematous papules that enlarged centrifugally to form figurate, annular or policyclic plaques involving the entire cutaneous surface. The clinical picture had a chronic course, disappearing only during unrelated febrile episodes. Histologic examination revealed a normal epidermis and a dense perivascular lymphohistiocytic infiltrate and numerous eosinophils in the superficial and reticular dermis. Today the patient is 24 years old and his lesions are still unchanged. Recently, clinical evaluation revealed a diffuse sderodermic induration. A second biopsy showed a diffuse thickening of collagen fibers in the dermis. To our knowledge, this is the second report of EAC with neonatal onset and such a long and peculiar clinical course.

Our aim was to comprehensively review reports of various dermatoses simulated by MF. Additionally, 2 cases in which MF simulated diseases not previously documented (psoriasis and Erythema Annulare Centrifugum) are presented. A literature search of all case reports of MF cited in the MEDLINE database from 1966 through 2000 plus those in one of the author's (H. Z.) files was performed. A total of 23 reported cases of dermatoses mimicked by MF were found. With the additional 2 dermatoses now presented, this yields a total of at least 25 diseases that can be simulated by MF. In view of the considerable number of dermatoses simulated by MF, the term the great imitator is appropriate for MF.
