    Abstract:

    Objectives: To verify the incidence of skin diseases in newborns of pregnant women at risk of a public hospital in the city of Santos, Brazil (Hospital Guilherme Alvaro), determining the potential relation between these dermatoses and diseases presented by the mothers. Methods: A total of 1,000 neonates were examined in the first 36 hours of life. The examination was repeated daily in each child up to hospital discharge. The paternal and newborn variables were submitted to analysis to detect statistically significant associations. Results: The most frequent skin disorders were: Mongolian spot (69.8%), Erythema Toxicum Neonatorum (19.7%), Port-wine stain (angiomatous naevi) (13.9%), and hypertrichosis lanuginose (10.3%). Conclusions: Minipuberty had a low incidence in neonates of diabetic mothers. The incidence of Mongolian spot was very low among newborns classified as caucasian.

    Abstract:

    Objetivo: Verificar a incidencia de dermatoses em recem-nascidos em enfermaria para gestantes de risco de um hospital publico da cidade de Santos, S.P. (Hospital Guilherme Alvaro) verificando a possivel relacao com doencas , apresentadas pelas maes. Metodos: O autor examinou 1.000 neonatos nas primeiras 36 horas de vida no periodo de 27 de dezembro de 2000 a 30 de julho de 2001 e esse exame foi repetido diariamente em cada crianca enquanto a mesma permanecesse internada. A pesquisa foi clinica, tendo sido realizada biopsia de pele em apenas uma crianca portadora de leucemia cutanea em funcao da necessidade para o diagnostico. As variaveis paternas e as dos recem¬nascidos foram submetidas a analise atraves do teste X~ e de regressao logistica para se detectar relacoes significantes estatisticamente. Resultados: Diagnosticou-se 31 diferentes dermatoses nos recem-nascidos. Cerca de 87,9 por cento das criancas apresentavam uma ou mais alteracoes dermatologicas sendo que as mais frequentes foram: mancha mongolica (69,8 por cento), eritema toxico neonatal (19,7 por cento), ictericia fisiologica (11,8 por cento), nevo angiomatoso (13,9 por cento), hipertricose lanuginosa (10,3 por cento), hiperplasia de glandula sebacea (7,8 por cento), descamacao fisiologica (7,0 por cento), puberdade em miniatura (4,1 por cento), cutis marmorata (2,2 por cento) foram as mais frequentes. Em todas as outras dermatoses encontradas o percentual de incidencia foi igualou menor que 2,0 por cento. Conclusoes: A puberdade em miniatura teve baixa incidencia entre os neonatos de mae diabetica e a ictericia fisiologica foi mais frequente nas criancas cujas maes faziam uso de droga.     Erythema Toxicum Neonatorum is a common rash of unknown etiology affecting healthy newborn infants. In this study, we postulated that the rash reflects a response to microbial colonization of the skin at birth, and that the hair follicle constitutes an "easily opened door" for microbes into the skin of the newborn. We collected microbial cultures from the skin of 69 healthy, 1-d-old infants with and without Erythema Toxicum to identify the colonizing flora and correlate culture results with clinical findings. We also analyzed biopsies from lesions of Erythema Toxicum with scanning and transmission electron microscopy in the search for microbes. Finally, each infant's body temperature was measured as a sign of acute phase response. We found that 84% of 1-d-old healthy infants, with and without Erythema Toxicum were colonized with coagulase-negative staphylococci. In all lesions of Erythema Toxicum, TEM identified cocci-like bacteria localized in the hair follicle epithelium and into recruited immune cells surrounding the hair follicle; morphology and dimension supported their identification as belonging to the genus Staphylococcus. SEM revealed 10 times more hair structures per skin surface unit in newborns compared with adults. Infants with Erythema Toxicum also had higher body temperature. In Erythema Toxicum, commensal microbes gain entry into the skin tissue, most probably through the hair canal. This triggers the local immune system and a systemic acute phase response, including an increase in body temperature. We speculate that early microbial exposure to the newborn may be important for the maturation of the immune system.

    Erythema Toxicum Neonatorum is a common, inflammatory skin reaction in healthy newborn infants characterized by an accumulation of activated immune cells in the lesions. Its etiology and physiologic significance are still unclear. The purpose of this study was to extend the search for possible inflammatory mediators of the rash. We performed immunohistochemistry on punch biopsy cryosections from lesions of four, 1-day-old infants and from four matched controls without rash, using antibodies against the water channel proteins aquaporin-1 (AQP1) and aquaporin-3 (AQP3), psoriasin, and the nitric oxide synthase (NOS) enzymes, neuronal NOS (nNOS), inducible NOS (iNOS), and endothelial NOS (eNOS). All sections from the lesions showed a dense, nodular cellular infiltrate located near the hair follicle. The vessels in the dermis showed a high incidence of AQP1 and eNOS. Strong staining for AQP1, AQP3, and psoriasin, as well as nNOS, iNOS, and eNOS were seen in the entire epidermal layer. The infiltrate in the dermis contained numerous cells expressing AQP1, AQP3, nNOS, iNOS, and eNOS. Double immunofluorescence staining showed that AQP3 was located in CD1a-expressing Langerhans cells and other dendritic cells in the dermis, as well as in CD14-expressing macrophages, CD15-expressing neutrophils, and EG2-expressing eosinophils surrounding the hair follicle. Our findings show that AQP1 and AQP3, psoriasin, and NOSs are involved in the activation of the skin immune system at birth.

Some dermatological finding was observed in 95.8% of neonates; of these, 88.6% had transient neonatal skin conditions, 42.6% had congenital birthmarks, 26.8% had some benign neonatal pustulosis, 2% had lesions secondary to trauma (including scratches), 0.5% had skin malformations, and 0.1% had an infectious disease. The most prevalent dermatological findings were: lanugo, which was observed in 38.9% of the newborns, sebaceous hyperplasia (35%), dermal melanocytosis (24.61%), skin desquamation (23.3%), Erythema Toxicum Neonatorum (23%), salmon patch (20.4%), skin Erythema (19%), genital hyperpigmentation (18.4%), eyelid edema (17.4%), milia (17.3%), genital hypertrophy (12%), and skin xerosis (10.9%). Conclusions: Dermatological findings are frequent during the first days of life and some of them characterize the newborn's skin. Mixed‐race newborns and those whose mothers had some gestational risk factor had more dermatological findings. Resumo: Objetivo: Verificar a prevalência dos achados dermatológicos nos primeiros dias de vida e analisar se há associação com características neonatais, gestacionais e sazonalidade. Métodos: Recém-nascidos de três maternidades de uma capital brasileira foram selecionados aleatoriamente para serem submetidos ao exame dermatológico realizado por dermatologistas. Resultados: Foram selecionados aleatoriamente 2839 neonatos com até 72 horas de vida, 309 foram excluídos por terem sido admitidos em Unidade de Tratamento Intensivo. Dos 2530 neonatos examinados 49,6% eram da raça branca e 50,5% do sexo masculino. Foi observado algum achado dermatológico em 95,8% dos recém-nascidos; destes, 88,6% tinham lesões cutâneas transitórias neonatal, 42,6% marca de nascimento, 26,8% tinham pustulose benigna neonatal, 2% lesões secundárias ao trauma, 0,5% malformação cutânea e 0,1% doença infecciosa. O achado dermatológico mais frequente foi o lanugo, que foi observado em 38,9% dos neonatos, seguido pela hiperplasia de glândulas sebáceas (35%), melanocitose dérmica (24,6%), descamação da pele (23,3%), eritema tóxico neonatal (23%), mancha salmão (20,4%), eritema da pele (19%), hiperpigmentação da genitália (18,4%), edema palpebral (17,4%), cistos de mília (17,3%), hipertrofia da genitália (12%) e xerose cutânea (10,9%). Conclusões: Os achados dermatológicos são frequentemente identificados nos primeiros dias de vida e muitos deles caracterizam a pele do recém-nascido. Os neonatos pardos e aqueles cujas mães apresentavam algum fator de risco gestacional tiveram mais achados dermatológicos. A idade gestacional, a etnia do neonato, o gênero, o índice de Apgar, o tipo de parto e a sazonalidade influenciaram na presença de manifestações cutâneas específicas.
