Protective effects of Artemisia campestris extract against gastric acid reflux-induced esophageal Mucosa injuries

In the present investigation, we studied the putative protective effect of Artemisia campestris aqueous extract (ACAE) against gastro-esophageal reflux (GER)-induced esophagitis in rats. The experimental ophagitis was induced by the ligation of the pylorus as well as the junction between the forestomach and the corpus. We firstly found that ACAE administration at 100, 200 and 400 mg/kg, b.w., p.o . significantly protected GER-induced macroscopic and histological injuries in the Esophagus tissue. Our extract also counteracted GER-induced Esophagus lipoperoxidation, restored the depletion of antioxidant enzyme activities such as superoxide dismutase (SOD), catalase (CAT) and glutathione peroxidase (GPx) as well as thiol groups levels. Furthermore, we showed that acute GER provoked an increase in Esophagus Mucosa hydrogen peroxide (H 2 O 2 ), free iron and calcium levels, whereas ACAE treatment reversed all GER-induced intracellular mediators' disturbances. In conclusion, we suggested that ACAE had potent protective effects against esophagitis due, in part, to its antioxidant properties as well as its opposite effect on some intracellular mediators.

We showed also that ER increased esophageal calcium, hydrogen peroxide (H2O2) and free iron levels while MBSAE treatment protected against intracellular mediators deregulation.Conclusion Our data suggest that MBSAE exerted a potential protective effect against ER-induced damage in rat Esophagus, at least in part, due to its antioxidant properties.

circumferential endoscopic Mucosal resection in barrett s Esophagus with high grade intraepithelial neoplasia or Mucosal cancer preliminary results in 21 patients

However, the remaining Barrett's Esophagus epithelium remains at risk of developing further lesions. The aim of this study was to evaluate the efficacy of circumferential endoscopic mucosectomy (circumferential EMR)s in removing not only the index lesion (high-grade intraepithelial neoplasia (HGIN) or Mucosal cancer), but also the remaining Barrett's Esophagus epithelium. PATIENTS AND METHODS: A total of 21 patients were included in the study (11 men, 10 women), who had Barrett's Esophagus and either HGIN (n = 12) or Mucosal cancer (n = 9). Of the patients, 17/21 were at high surgical risk and five had refused surgery. On the basis of preprocedure endosonography their lesions were classified as T1N0 (n = 19) or T0N0 (n = 2). The lesions and the Barrett's Esophagus epithelium were removed by polypectomy after subMucosal injection of 10-15 ml of saline; a double-channel endoscope was used in 15/21 cases. Circumferential EMR was performed in two sessions, the lesion and the surrounding half of the circumferential Barrett's Esophagus Mucosa being removed in the first session. In order to prevent the formation of esophageal stenosis, the second half of the Barrett's Esophagus Mucosa was resected 1 month later. RESULTS: Complications occurred in 4/21 patients (19 %), consisting of bleeding which was successfully managed by endoscopic hemostasis in all cases. No strictures were observed during follow-up (mean duration 18 months) and endoscopic resection was considered complete in 18/21 patients (86 %). For three patients, histological examination showed incomplete removal of tumor: one of these underwent surgery; two received chemoradiotherapy, and showed no evidence of residual tumor at 18 months' and 24 months' follow-up, respectively. Two patients in whom resection was initially classified as complete later presented with local recurrence and were treated again by EMR. Barrett's Esophagus Mucosa was completely replaced by squamous cell epithelium in 15/20 patients (75 %). CONCLUSIONS: Circumferential EMR is a noninvasive treatment of Barrett's Esophagus with HGIN or Mucosal cancer, with a low complication rate and good short-term clinical efficacy. Further studies should focus on long-term results and on technical improvements.

Morphological state of the mucous membrane of the Esophagus of patients with post-resection manifestations of reflux esophagitis depending on the method of the formation of mechanical Esophagus-gastric anastomosis

The aim of the study – to evaluate the pathogistological changes of the Esophagus Mucosa associated with gastro-esophageal reflux in patients with esophageal cancer and gastroesophageal, junction cancer after surgical treatment, depending on the technique of forming the Esophagus-gastric anastomosis. The study included 30 patients who developed a mechanical invagination of the Esophagus-gastric anastomosis developed by the Ukrainian patent (study group) and 30 patients who formed the end-to-end mechanical circulatory Esophagus-gastric anastomosis (comparison group). At 12 months of observation all patients were given fibroesophagogastroduodenoscopy. Endoscopic diagnosis of esophagitis was performed according to a modified Los Angeles classification. To assess the morphological state of the esophageal Mucosa, the esophageal Mucosal biopsy was performed on the site of anastomosis and morphologically evaluated the severity of the reflux esophagitis. Reflux-associated changes in squamous epithelium of the Esophagus were evaluated according to the consensus recommendations of the Esohisto Project. The differences in the results obtained were considered statistically significant at p<0.05, which ensures a 95% level of probability. It has been established that the frequency with which the microscopic signs of reflux esophagitis are fixed are almost 2 times lower in the group of patients who were formed mechanical invagination Esophagus-gastric anastomosis in comparison with the mechanical circular (46.7% vs. 83.3%, p<0.05), since invagination simulates the reproduction of antireflux properties of the lost gastrointestinal transition. Endoscopically diagnosed cases of reflux esophagitis are additionally supplemented by microscopically detected from 5.9% of subjects in the study group to 28.6% of subjects (p<0.05) in the comparison group, which indicates a higher sensitivity histological diagnosis. The signs that are consistently associated with post-resection reflux esophagitis include hyperplasia of the basal layer at both the frequency (86.7% vs. 100%) and the severity of the severity (6.7% vs. 23.3%, p<0.05), as well as moderate prolongation of papillae (30.0% vs. 66,6%, p<0.01), according to which the best results were obtained in the group of patients that formed the invagination mechanical Esophagus-gastric anastomosis. According to the Esohisto Project criteria, the frequency of both “mild” and “severe” esophagitis in the group of patients that formed the invagination mechanical Esophagus-gastric anastomosis was lower compared to the group with mechanical circulatory Esophagus-gastric anastomosis (36.7% and 10.0% vs. 63.3% and 20.0%, p<0.01, respectively).

The Radiation Field Feature of ~(188)Re Esophageal Stent

Methods: Measured the β-ray and bremssfrahlung dose of every selected point on the bionics esophageal stent and then drew out computer software by mathematics formula. Results: The radiation field of 188Re esophageal stent had its own feature: the max range of β-ray was 11 mm, 90% dose construction field was within 1. 5 mm, 95% dose range was 2. 5 mm. About 4. 21% of total energy of γ-ray and bremssfrahlung was out of 6. 5 mm. The absorption dose of every direction in same point of esophageal model was similar. (P 0. 05). Conclusion: β-ray was the major radiation while γ-ray and bremssfrahlung were 4. 21% among the radiation field. The max dose construction field was within 0. 5 ~ 1. 5 mm, just as the depth of Esophagus Mucosa. So 188Re was a good choice of palliative introcavitary radiotherapy of esophageal carcinoma.
