    Abstract:

    Background:  We previously reported that the fasting serum insulin level was significantly lower in Japanese patients than in Korean and Chinese patients, and showed evidence that a Difference in the dietary component would be one of the most influential factors for the Ethnic Difference. However, it is well known that type 2 diabetes mellitus (T2DM) results from the interaction between genetic predispositions and environmental risk factors. Therefore, we investigated Ethnic Differences by focusing on gene polymorphism, possibly related to T2DM in Japanese, Korean, and Chinese subjects. Methods:  Data sources included MEDLINE and EMBASE between January 2001 and October 2008. We conducted a search for articles containing minor allele frequency (MAF) in the gene polymorphisms of peroxisome proliferator-activated receptor-γ (PPARG), inward-rectifying potassium channel Kir6.2 (KCNJ11), Calpain 10 (CAPN10), and transcription factor 7-like 2 (TCF7L2). The pooled odds ratio was calculated by using a fixed-effects model with the Mantel–Haenszel method after confirming statistical evidence of homogeneity across the Ethnicities using the Breslow–Day test. Results:  The Breslow–Day test revealed that there were no statistically significant Differences between Ethnicities in pooled odds ratios for the gene polymorphisms in PPARG (P = 0.828), KCNJ11 (P = 0.194), CAPN10 (P = 0.090), and TCF7L2 (P = 0.376). Also, pooled odds ratios of each gene polymorphism in East Asians were 0.645 for PPARG (P = 0.000), 1.168 for KCNJ11 (P = 0.000), 0.967 for CAPN10 (P = 0.759), and 1.386 for TCF7L2 (P = 0.000). Conclusion:  The results of this study and our previous studies suggest that behavioral and environmental risk factors have a more significant impact on Ethnic Difference in East Asian patients with T2DM compared with genetic predispositions.

Methods Data sources included MEDLINE and EMBASE between 2001 and 2007. We conducted a search for articles containing mean or geometric mean values of FSI in East Asian subjects with NGT, IGR, or type 2 diabetes (T2DM). The Monte Carlo method was used for simulation of the mean and standard deviation of individual measures in each Ethnic group; calculation of the median ratio and 95% confidence interval of individual measures between Ethnic groups. Results Twenty-two articles fully met our pre-determined criteria and were included in the meta-analysis. Results of the meta-analysis revealed that FSI level is significantly lower in Japanese subjects with NGT, IGR, or T2DM than in Korean and Chinese subjects. Conclusions Ethnic Difference in FSI level between East Asians was observed in not only T2DM patients but also subjects with NGT or IGR. The lower FSI level in Japanese subjects was accompanied by lower triglyceride level. These results suggest that Ethnic Difference in dietary habit was one of the most influential factors for the Ethnic Difference in FSI.

Results of the meta-analysis revealed that FSI level is significantly lower in Japanese patients than in Korean and Chinese patients. Conclusions Results from our review of Ethnic Differences in dietary habit in the inter-East Asian population suggested that Difference in dietary component was one of the most influential factors for the Ethnic Difference.

    Introduction There are a large number of global clinical trials ongoing for patients with non-small cell lung cancer (NSCLC). Ethnic Difference in toxicity has not been adequately studied. Methods We performed a systematic search in PubMed for randomized phase II and III trials of NSCLC from January 2000 to December 2009, examining Ethnic Difference in hematological toxicity due to cytotoxic chemotherapy. Ethnicity was classified into Asian and non-Asian. We chose three treatment regimens used for NSCLC globally: cisplatin plus gemcitabine (CG), cisplatin plus vinorelbine (CV), and carboplatin plus paclitaxel (CP). We applied sensitivity analysis to examine unreported Ethnic Differences in hematological toxicities by changing the percentage of Asian patients from 0 to 18% in trials reported from the United States and Europe. Results We identified 12 phase II trials and 38 phase III trials of NSCLC with a total of 11,271 patients. Among these, 14 trials had reported Ethnic origins. Grade 3/4 toxicities were more frequently observed in the Asian studies. On the basis of sensitivity analysis, odds ratio of grade 3/4 neutropenia was significantly higher in Asian patients than non-Asian, when treated with CG (OR = 1.55–3.45, p p p p p p p p = 0.097–0.323), or CP (OR = 1.21–1.39, p = 0.114–0.152). Conclusions Severe hematological toxicity was frequently observed in Asian patients compared with non-Asian (mostly whites) in the treatment of chemotherapy for NSCLC.

    Proceedings: AACR 103rd Annual Meeting 2012‐‐ Mar 31‐Apr 4, 2012; Chicago, IL Purpose: The aim of this study was to investigate the pharmacogenetic determinants of sunitinib-related toxicity and Ethnic Difference in metastatic renal cell carcinoma (mRCC) among Korean patients. Experimental Design: A pharmacogenetic study was performed in 65 patients with mRCC treated with sunitinib (50mg orally once daily for 4 weeks-on/2 weeks-off). Detailed data regarding the toxicity of sunitinib, including thrombocytopenia, neutropenia, anemia, and hand-foot syndrome (HFS) were prospectively collected in a clinical trial program (n=38) or general oncology practice (n=27) using sunitinib. A total of 12 genetic polymorphisms in 7 candidate genes (CYP1A1, ABCG2, ABCB1, PDGFRα, VEGFR2, RET, and FLT3) were analyzed for an association with treatment-related toxicity using Pearson-χ2 test. Results: Common grade 3 or 4 treatment-related toxicities were thrombocytopenia (36.9%), neutropenia (18.4%), anemia (7.7%), and HFS (12.3%). Patients carrying an ABCG2 421 A/A genotype developed significantly more grade 3 or 4 thrombocytopenia, neutropenia, and HFS: odds ratio [OR] 10.15; P=0.03, thrombocytopenia, OR 17.33; P=0.003, neutropenia, OR 33.60; and P=0.005, HFS. Conclusion: c.421C>A genetic polymorphism in the ABCG2 gene may be most associated with the risk of sunitinib-related toxicity in mRCC patients.
