Fascioliasis risk factors and space time clusters in domestic ruminants in bangladesh

The total number of domestic ruminants in each district was used to calculate Fascioliasis cases per ten thousand animals at risk per district, and this was used for cluster and hotspot analysis. Clustering was assessed with Moran’s spatial autocorrelation statistic, hotspots with the local indicator of spatial association (LISA) statistic and space-time clusters with the scan statistic (Poisson model). The association between district Fascioliasis prevalence and climate (temperature, precipitation), elevation, land cover and water bodies was investigated using a spatial regression model. A total of 1,723,971 cases of Fascioliasis were reported in the three-year study period in cattle (1,164,560), goats (424,314), buffalo (88,924) and sheep (46,173). A total of nine hotspots were identified; one of these persisted in each of the three years. Only two local clusters were found. Five space-time clusters located within 22 districts were also identified. Annual risk maps of Fascioliasis cases correlated with the hotspots and clusters detected. Cultivated and managed (P < 0.001) and artificial surface (P = 0.04) land cover areas, and elevation (P = 0.003) were positively and negatively associated with Fascioliasis in Bangladesh, respectively. Results indicate that due to land use characteristics some areas of Bangladesh are at greater risk of Fascioliasis. The potential risk factors, hot spots and clusters identified in this study can be used to guide science-based treatment and control decisions for Fascioliasis in Bangladesh and in other similar geo-climatic zones throughout the world.

Biliary Fascioliasis: sonographic appearance patterns.

The study included 27 patients with Fascioliasis and abdominal sonographic findings in biliary system. All diagnoses were confirmed via serologic enzyme-linked immunosorbent assay, and 5 patients underwent cholecystectomy. Sonographic findings in the biliary system were defined as primary and secondary. Primary findings included spontaneously moving echogenic structures, linear echoes, curvilinear echoes, oval-shaped echogenic structure, matted echogenic particle, echogenic particle adherent to the gallbladder wall, motionless freely floating round echogenic foci, and leaf-like echogenic structures. Secondary findings were dilatation or wall thickening of the biliary system. In the liver, multiple confluent subcapsular nodules were also noted on sonography and CT in 14 of 27 patients. Sonography can detect biliary system lesions in Fascioliasis and can aid diagnosis of the disease. However, the radiologist should be familiar with the different sonographic appearances of biliary Fascioliasis. (c) 2008 Wiley Periodicals, Inc.

We also investigated whether there are significant differences in MDA levels and antioxidant enzymatic activities between acute and chronic Fascioliasis. Methods Forty Fascioliasis patients who were diagnosed by ES-ELISA positivity were included in this study. The patients were classified as 18 with acute and 22 with chronic Fascioliasis. Results In patients with Fascioliasis, levels of MDA were statistically higher and erythrocyte SOD and GPx activities were statistically lower than in healthy controls. MDA levels were found to be higher in patients with acute Fascioliasis than in patients with chronic Fascioliasis although MDA levels were significantly higher in patients with chronic Fascioliasis than in controls. There was no statistically significant difference between the two groups for the antioxidant enzyme activities. Conclusion The results of this study may indicate that Fascioliasis produces specific effects on the antioxidant defense mechanisms due to its inflammatory character. Our results also allow us to suggest that oxidative stress has an important role in the pathogenesis of Fascioliasis and the persistence of this oxidative stress can be one of the underlying factors in the pathogenesis of the chronic disease.

Fascioliasis in eosinophilic patients in the Isparta region of Turkey.
Infection, 2003Co-Authors: Mustafa Demirci, Metin Korkmaz, Selçuk Kaya, Aydinten KumanAbstract:The aim of this study was to investigate Fascioliasis in a group of eosinophilic and non-eosinophilic patients in the Isparta region of Turkey. All cases were examined for antibodies against Fasciola hepatica by the modified ES-ELISA method. Seropositive patients with Fascioliasis were investigated by radiological and laboratory evaluations. Of the 756 eosinophilic patients, 6.1% were found to be positive for Fascioliasis by ES-ELISA, while the rate was only 0.9% among the non-eosinophilic group (statistically significant, p < 0.001). Among the seropositives, 19 cases were evaluated further. Ultrasonographic findings were found in 14 of the 19 cases. In six of nine cases, computed tomography findings were related to Fascioliasis. F. hepatica eggs were found in ine of the 19 cases. Fascioliasis was demonstrated in the Isparta region of Turkey for the first time. The higher prevalence of Fascioliasis among patients with eosinophilia indicates the importance of this sign for this infection.
