    Abstract:

    Kawasaki disease (KD) is an acute, self-limited vasculitis of unknown etiology that primarily affects the coronary artery (CA) and presents during childhood. The characteristic coronary arterial lesion of KD is an aneurysm. Ischemic heart disease derived from a CA aneurysm is experienced approximately two decades after the onset of acute KD. In recent years, the primary issue of concern has been asymptomatic adults with a CA aneurysm caused by undiagnosed KD. We present a case of sudden death as a late KD sequel in a young adult. A postmortem CT scan revealed a coarse calcification of a left anterior descending CA aneurysm, which was confirmed at the time of autopsy. A postmortem CT scan is useful in cases of sudden death where the detection of a calcified CA aneurysm would suggest to the Forensic Pathologist that the deceased suffered from a late sequel of KD. The use of screening postmortem CT scans for young people may detect cases of unsuspected CA aneurysms, raising the possibility of untreated KD.

    Post-traumatic stress disorder (PTSD) is a condition characterised by recurrent intrusive distressing memories of a traumatic event(s) with recurrent dreams and flashbacks. Given the nature of standard Forensic pathology practice which involves on-going assessments of violent crimes and their sequelae with autopsy dissections of victims and in detail study of injuries, it is surprising that more has not been written on this in the literature. Perhaps PTSD should be studied further in a Forensic context with a recognition that years of intimate exposure to violence may lead to accumulated, ongoing and unresolved grief in practitioners.

    Individuals engaged in farming have higher risks of injury and death from trauma than many other workers. Fatalities most often involve tractor-related incident such as roll-overs. Although it is also recognized that farm machinery may result in serious injuries and death, little has been reported on problems associated with hay baling, transport and storage. Case 1: A 43-year-old man trying to dislodge jammed hay in a hay baler had either been pulled, or had fallen, into the baler, where he had been crushed, rotated and then cocooned within a hay bale. The body showed extensive blunt trauma to the head, neck, chest, abdomen, pelvis and limbs, with burning from a fire that subsequently started within the overheated machine. Case 2: A 58-year-old man was crushed between the moving arms of a hay shuttle and a safety fence. Death was attributed to blunt chest and abdominal trauma with crush asphyxia. Case 3: A 56-year-old man fell some distance from the top of stacked hay bales fracturing his neck and causing virtual transection of his cervical spinal cord. These cases demonstrate rare forms of farm deaths that may be associated with the creation (baling), moving (shuttling) and storage of hay bales. All forms of farm machinery should be treated circumspectly, given the possibility that serious injury or death may result from inattention or inappropriate handling. Temporary stacking of hay bales may create high work platforms that risk falls with lethal consequences.

    Pediatric Forensic pathology is an emerging medical subspecialty that spans the area between pediatric and Forensic pathology. Advances in both of these fields have increased the sophistication of diagnoses, with overlap of disorders that might present to either the pediatric or Forensic Pathologist, adding further layers of complexity. Not surprisingly, therefore, there are important ethical and medical controversies in pediatric Forensic pathology that merit careful consideration and attention.

    Twenty years after the Second World War the public were made aware of War Crimes committed by the German Forensic Pathologist, Gerhart Panning (1900-1944). From 1942 till 1944, Panning was professor at the Institute of Forensic Medicine at the University of Bonn. Panning died of tuberculosis on 22 March 1944. After the Second World War, Panning's widow tried to obtain denazification for her husband. There were no particularly serious doubts. In 1965, Konrad Graf von Moltke, the son of Helmuth James Graf von Moltke (1907-1944), presented to the court a private letter from his father to his mother. In this letter, written in 1941, Panning's experiments on Soviet prisoners are described. In the so-called Callsen trial in the court of Darmstadt from 1960 to 1968, the experiments were confirmed by witnesses. In 1941, Panning performed experiments in cooperation with the Sicherheitsstaffel (Nazi special police) in the Ukraine to prove that captured ammunition of the soviet infantry violated international law. For this purpose, different parts of the bodies of living Soviets were used as firing targets. He published the results of these experiments in a scientific journal without any evidence of the origin of these observations. In this article, Panning's life and crimes have been described.

    Medical malpractice charges from 1989 to 2002 were evaluated. A rising number of cases during this period is evident. The charges of practice falling below the standard of care (n = 285) were surveyed to determine who informed the prosecution, which clinical subjects are involved, what kind of charges can be found and whether such allegations can be appropriately assessed by means of a Forensic autopsy. Forensic Pathologists were found to be useful for ascertainment and interpretation of autopsy findings. If special questions arise, an additional expert opinion should be suggested by the Forensic Pathologist. There was no relevant shift in the range of subjects involved compared to former studies. The investigated charges might represent only a small fraction of cases of medical practice falling below the standard of care.

    Medicolegal malpractice assessments have been an important part of the work of Forensic Pathologists. Not only botchers are concerned but also physicians of all clinical subjects, especially malpractice charges claiming a malpractice leading to death. Single long articles and book chapters have been published about malpractice assessments since the end of the 17th century by Forensic Pathologists. After World War II systematic studies of malpractice charges first were published in the 1960s. Meanwhile the attention focuses more and more on the role of Forensic Pathologists to help in prevention of malpractice.

    At the Victorian Institute of Forensic Medicine the Forensic Pathologist provides expert medical advice to the Coroner. Cases of natural death from hemopericardium and hemothorax are reasonably common in the Forensic setting and are readily identifiable on postmortem CT (PMCT) scanning. The management of these cases raises interesting and challenging issues for the Forensic Pathologist. We present three cases in which PMCT angiography was useful in the management of each individual case and illustrate the potential of the technique in medico-legal death investigation.

    Snakebite envenoming (SBE) is a public health issue in developing countries. The estimated annual global incidence of snakebites is about 5.4 million snakebites per year, resulting from 1.8 to 2.7 million cases of SBE and from 81,000 to 138,000 deaths with 400,000 survivors suffering permanent physical and psychological disabilities. There are more than 3000 species of snakes around the world: 600 are venomous and over 200 are considered to be medically important because of their clinical effects. The severity of SBE depends on several factors among which bite localization, snake's size, condition of glands and teeth, bite angle and bite duration, the microflora of the snake's mouth and victim's skin, age of the victim, weight, health status, and victim's activity after a bite. Snake venoms are mixtures of protein families, and each of these families contains many different toxins or toxin isoforms. Based on their effects, snake venoms can be classified as hemotoxic, neurotoxic, or cytotoxic and they can all act together involving multiple tissues and organs. When the bite is fatal, the mechanism of death is primarily related to the paralysis of respiratory muscles, which causes asphyxia and hypoxic-ischemic encephalopathy, but also anaphylactic shock, hemorrhagic shock, cardiomyopathy, acute tubular necrosis (ATN). The purpose of this literature review is to evaluate epidemiological and post-mortem examination findings in fatal SBEs in order to better understand the pathophysiological mechanisms, thus helping Pathologists in defining the correct diagnosis.
