Reclaiming the Autopsy as the Practice of Medicine: A Pathway to Remediation of the Forensic Pathology Workforce Shortage?

Weedn, M J MenendezAbstract:The historically constricted Forensic Pathology workforce pipeline is facing an existential crisis. Pathology residents are exposed to Forensic Pathology through the American Council of Graduate Medical Education autopsy requirement. In 1950, autopsies were conducted in one half of the patients dying in American hospitals and 90% in teaching hospitals, but they have dwindled to fewer than 5%. Elimination of funding for autopsies is a major contributor to the lack of support for autopsies in departments of Pathology. Funding may require reclaiming the autopsy as the practice of medicine. Funding of autopsies would rekindle interest in hospital autopsies and strengthen the Forensic Pathology workforce pipeline.

WeednAbstract:Forensic Pathology Reviews is a new (April 2004) and ambitious series published by Humana Press and edited by Michael Tsokos, MD of the Institute of Legal Medicine at the University of Hamburg. It is a part of an explosive growth in Forensic publications that is difficult not to link to the recent interest from the spate of television shows or Forensics. However, the introduction to the series correctly refers to a significant expansion of scientific progress in the Forensic sciences and states: “The Forensics Pathology Reviews knowledge on special topics in the field, focusing closely on the dynamic and rapidly growing evolution of medical science and law.” The introduction further states that the series is intended to take a problem-oriented approach and provide comprehensive and well documented reviews of the international literature with insights into new diagnostic techniques.

A comprehensive analysis of Forensic science training in Forensic Pathology fellowship programs.

WeednAbstract:The purpose of this study is to assess the current nature and extent of Forensic science training in the nation's 43 Accreditation Council for Graduate Medical Education (ACGME)-accredited Forensic Pathology fellowship programs. This manuscript describes the results of a survey of training program directors performed as a project for the College of American Pathologists (CAP) Forensic Identity Committee. The results show a considerable diversity in the Forensic sciences training in Forensic Pathology programs, ranging from one to ten weeks, with an average of 4.1 weeks. However, almost one half of programs provide no training in at least one of the surveyed disciplines. Only 58.1% of responding programs meet the ACGME requirements for Forensic science training, and a similar percentage (56.0%) currently provides the duration of Forensic science training recommended by the National Association of Medical Examiners (NAME). The deficit in the NAME training recommendations, surprisingly, was strictly in the field of toxicology.

In China, Forensic Pathology is one of the required major subspecialties for undergraduates majoring in Forensic medicine, in contrast to Forensic education in Western countries where Forensic Pathology is often optional. The enduring predicament is that the professional qualities and abilities of Forensic students from different institutions vary due to the lack of an efficient Forensic pedagogical model. The purpose of this article is to describe the new pedagogical model of Forensic Pathology at Zhongshan School of Medicine, Sun Yat-sen University, which is characterised by: (a) imparting a broad view of Forensic Pathology and basic knowledge of duties and tasks in future careers to students; (b) educating students in primary skills on legal and medical issues, as well as advanced Forensic pathological techniques; (c) providing students with resources to broaden their professional minds, and opportunities to improve their professional qualities and abilities; and (d) mentoring students on occupational preparation and further Forensic education. In the past few years, this model has resulted in numerous notable Forensic students accomplishing achievements in Forensic practice and Forensic scientific research. We therefore expect this pedagogical model to establish the foundation for Forensic pathological education and other subspecialties of Forensic medicine in China and abroad.

Selecting Forensic Pathology as a career: a survey of the past with an eye on the future.

Quinton, Lakshmanan Sathyavagiswaran, Suzanne UtleyAbstract:Background Each year there are about 30 to 40 physicians who train and become board-certified in the specialty area of Forensic Pathology, compared with hundreds or thousands in other disciplines. There are not enough board-certified Forensic pathologists to cover national need. The National Association of Medical Examiners' (NAME) Forensic Pathology Training Committee conducted a survey of its members to determine which factors influenced them to select Forensic Pathology as a career, and to offer suggestions about possible recruitment methods in the future. Methods Two of the authors developed a 13-question survey form that included questions designed to determine the demographics of the responders, education level at which interest emerged, influential factors in the selection of Forensic Pathology, exposure to the subject matter of Forensic Pathology in medical school and residency, opinions about the best educational level for recruitment targeting, and faculty reactions to selection of Forensic Pathology as a career choice. Comments and suggestions were also solicited. The survey was sent by email to the 552 physician NAME members who have email addresses on file at the NAME Home Office. Results One hundred sixty-one surveys were returned for a response rate of 29%. Most responders were full-time, board-certified Forensic pathologists who had been practicing for an average of 18 years. The most influential factors in developing interest were exposure to Forensic Pathology in residency training and the influence of a professor or mentor. Medical school was the favored education level to target recruitment. Less than half had a Forensic pathologist as an autopsy instructor in anatomic Pathology residency. The number of responders who were encouraged by faculty to pursue Forensic Pathology was about the same as the number who were either discouraged or who perceived no particular positive or negative reinforcement. The typical scenario for Forensic Pathology exposure during anatomic Pathology residency was a 4-week rotation at an off-site location from the medical school or hospital, with a mentor that had an adjunct, assistant, associate, or clinical faculty appointment. Conclusions If the past predicts the future, it will be important to ensure that Pathology residents have a planned and positive exposure to Forensic Pathology and that Forensic pathologist mentors are available to training programs. There are a variety of other methods that might be used for recruitment which include more emphasis on medical students, a more academic approach, and affiliation, emphasizing the scientific nature of the work, integrating Forensic Pathology more into the ongoing medical school curriculum, improving the anatomic Pathology residency autopsy experience, and avoiding possible turnoffs that can be caused by presentation of sensational or unpleasant cases that are not representative of routine daily work. Improved remuneration and building esteem by peers were also cited as critical factors, as was recruitment of more physicians into Pathology in general. The Committee intends to develop a plan for recruitment and retention in the field of Forensic Pathology. Based on the survey data, this will require a conjoined effort with the American Association of Medical Colleges, the Accreditation Council on Graduate Medical Education, the Association of Pathology Chairman, and other entities to enable a planned and multifaceted approach to recruitment and retention in the field.

Veterinary Forensic Pathology: The Search for Truth

Mcdonough, Beverly McewenAbstract:Veterinary Forensic Pathology is emerging as a distinct discipline, and this special issue is a major step forward in establishing the scientific basis of the discipline. A Forensic necropsy uses the same skill set needed for investigations of natural disease, but the analytical framework and purpose of Forensic Pathology differ significantly. The requirement of legal credibility and all that it entails distinguishes the Forensic from routine diagnostic cases. Despite the extraordinary depth and breadth of knowledge afforded by their training, almost 75% of veterinary pathologists report that their training has not adequately prepared them to handle Forensic cases. Many veterinary pathologists, however, are interested and willing to develop expertise in the discipline. Lessons learned from tragic examples of wrongful convictions in medical Forensic Pathology indicate that a solid foundation for the evolving discipline of veterinary Forensic Pathology requires a commitment to education, training, and certification. The overarching theme of this issue is that the Forensic necropsy is just one aspect in the investigation of a case of suspected animal abuse or neglect. As veterinary pathologists, we must be aware of the roles filled by other veterinary Forensic experts involved in these cases and how our findings are an integral part of an investigation. We hope that the outcome of this special issue of the journal is that veterinary pathologists begin to familiarize themselves with not only Forensic Pathology but also all aspects of veterinary Forensic science.

Forensic Pathology of companion animal abuse and neglect

These cases require modification of postmortem procedures and written reports, as the questions asked by courts typically differ from those asked in routine diagnostic cases. Here we review the practice of veterinary Forensic Pathology as it applies to cases of companion AAN, as well as the fundamental principles of Forensic Pathology, the components of a Forensic necropsy, and the goals of the necropsy in cases of blunt-force trauma, projectile wounds, and starvation. Future directions and endeavors in veterinary Forensic Pathology are broached.
