Materials and Methods Thorough the 161 lesions of 147 patients are included in this prospective study. During PDVFT, it is determined that eight different Fremitus patterns have broken out according to the colour aliasing during patient’s vocal phonation. It is also grouped as, according to the lesion color aliasing pattern during vibration: (c) for central, (p) for peripheral, (d) for perilesional-desmoplastic, (e) for eccentric, (h) for heterogenous-hierarchic, (s) for septal, (t) for total, (z) for absent-zero. Thereafter, the patterns and histopathological results of the lesions are correlated and analysed statistically. Results Out of 161 lesions, (benign: 133 and malignant, 28 cases) totally 189 patterns are encountered. None of the patterns z, t, s or e was detected in any of the malignant lesions. Specific values for different patterns have been obtained which changing between 29% and 100%. After adding the PDVFT finding, the newly generated BIRADS scheme has the accuracy value as high 91%. Conclusion When it is classified according to the colour aliasing pattern(s) of lesion in PDVFT; the “p” pattern is seen about the same ratio in both malign lesion and benign lesion (nonspecific patterns); “c, d, h” patterns mostly related with malignancy, whereas “e, s, t, z” patterns related with benignity. With adding the PDVFT findings; accuracy of well known BIRADS categorization and lesion demarcation were also improved successfully.

Ultrasound in Obstetrics & Gynecology, 1995Co-Authors: C Sohn, A BaudendistelAbstract:During sonographic examinations using the new color Doppler technique, maximum-entropy method (MEM), it was discovered, by chance, that artefacts which are produced by vibrations of the thorax can be used to differentiate between malignant and benign breast lesions. These artefacts, in the form of small color pixels, can either be brought about by vocal Fremitus known from internal medicine or by humming in a low tone. In the case of malignant tumors, the color pixels are visible in the middle of the sonographically portrayed tumor as well as in the surrounding tissue, whereas in benign tumors the artefacts are only found in the surrounding tissue and are quite clearly not present in the center of the tumor. On the preoperative day, 95 patients with 46 benign and 49 malignant lesions were examined using the method described above. In 91% the differential diagnosis was in accordance with the histological results. In seven cases a benign lesion was classified before the operation as malignant and in two cases malignancy was incorrectly diagnosed as benign. Our explanation of the phenomenon is based on the fact that most benign tumors exhibit a restricted growth and form a clear-cut boundary with the surrounding tissue, so the vibrations are not conducted into the tumor. In contrast, malignant lesions grow by infiltrating the surrounding tissue, allowing the vibrations to be conducted into the tumor, where they can be demonstrated as small color pixels. Geburtshilfe Und Frauenheilkunde, 1994Co-Authors: Ch Sohn, A Baudendistel, M Kaufmann, G BastertAbstract:In the experimental phase of application of a new Non-Doppler technology (MEM system, Acoustic Imaging, Phoenix; Dornier Medizintechnik) we observed, that in patients, who spoke during colour imaging of a breast tumour, artifacts appeared in or around the lesion: the colour artifacts were seen regularly inside the tumour in cases of malignancies, and exactly surrounding benign tumours. Postoperative histological findings served as an objective criterion of classification/differentiation. To examine this phenomenon, we performed a study in 71 patients. These women with a sonographically detectable tumour (37 malignant, 34 benign) were examined on the day before surgery. We observed, that if patients uttered the number "99" with a relatively low voice or alternatively hummed a deep sound, the artifacts could be regularly visualized. In 66/71 patients (93%) status evaluation by artifact generation due to vocal Fremitus examination was correct. In 3 patients the tumour was erroneously described as malignant, histology showing a proliferative mastopathy. In 2 cases the tumour was classified as benign, whereas histology revealed a malignancy, in both patients a large ductal-invasive carcinoma (> 3 cm). This phenomena could, however, not be reproduced with other colour techniques. A possible explanation is: Thoracic vibrations during speech can be registered by the MEM technique. These vibrations are not perpetuated into the benign lesion characterised by a displacing growth, due to which the vibrations are "barred off" at the borders of the tumour. Infiltrating growth typical of a malignancy causes transmission of these vibrations into the center of the tumour.(ABSTRACT TRUNCATED AT 250 WORDS)

Superior canal dehiscence syndrome : Diagnosis with vestibular evoked myogenic potentials and Fremitus nystagmus.

Recently, vestibular evoked myogenic potential (VEMP) measurement was established for the diagnosis of SCDS. In the present study, a case series of patients with SCDS were analyzed, with a focus on VEMP. Four patients with SCDS were prospectively examined with ocular VEMP (oVEMP) and cervical VEMP (cVEMP). The clinical features and the standard audiovestibular test battery results are summarized and analyzed. The diagnostic accuracy of VEMP testing is evaluated. The increased oVEMP amplitudes had a specificity of 100% in this patient population. All patients had normal caloric function and head impulse testing. The Tullio sign was observed in two patients. Three patients had autophony. The air-bone gap was not greater than 10 dB in any of the patients. Two patients had marked Fremitus nystagmus. All patients had a bony dehiscence of the superior semicircular canal on computed tomography imaging. The subjective and clinical features in this case series of SCDS patients were heterogeneous. However, objective oVEMP testing had the highest diagnostic value. Furthermore, we describe a new diagnostic clinical sign: Fremitus nystagmus.

MaxwellAbstract:Einleitung Das Bogengangsdehiszenzsyndrom („superior canal dehiscence syndrome“, SCDS) ist eine relativ seltene neurotologische Erkrankung. Sie zeichnet sich durch ein heterogenes klinisches Bild aus. Kürzlich wurden vestibulär evozierte myogene Potenziale (VEMP) zur Diagnostik des SCDS etabliert. In der vorliegenden Arbeit wurde eine Fallserie von Patienten mit SCDS analysiert, mit einem Fokus auf den VEMP-Befunden. Methoden Vier Patienten mit SCDS wurden prospektiv mittels okulären VEMP (oVEMP) und zervikalen VEMP (cVEMP) untersucht. Dabei wurden die klinischen Befunde sowie die weiteren audiovestibulären Testergebnisse zusammengefasst und analysiert. Die diagnostische Genauigkeit der VEMP-Untersuchungen wurde evaluiert. Ergebnisse Die Erhöhung der oVEMP-Amplituden weist in dieser Fallserie eine Spezifität von 100 % auf. Bei allen Patienten fand sich eine normale kalorische Funktion sowie ein unauffälliger Kopfimpulstest. Zwei Patienten wiesen ein Tullio-Phänomen auf, bei 3 Patienten lag eine Autophonie vor. Die Schallleitungskomponente war bei keinem Patienten >10 dB. Ein Patient zeigte einen deutlichen Fremitusnystagmus. Bei allen Patienten zeigte sich in der Computertomographie eine knöcherne Dehiszenz des oberen Bogengangs. Schlussfolgerung Beim SCDS ergab sich in dieser Fallserie ein heterogenes Bild bei Anamnese und klinischer Untersuchung. Die oVEMP-Untersuchung jedoch erwies sich als der diagnostisch wertvollste Test. Darüber hinaus wird hier ein neues diagnostisches klinisches Zeichen beschrieben, der Fremitusnystagmus.
