Methods The quality control circle group was set up, with the theme of " improving the compliance of off-bed activities at the early stage after the Funnel Chest operation" .The relevant factors were analyzed and countermeasures were formulated and implemented as well.Standardized processes were set up on the basis of the combination of the confirmed effects and the clinical practices for the continuous improvement.     Abstract:

Results The compliance of off-bed activities at the early stage after the Funnel Chest operation was improved from 13.3% to 56.7%(target value 53.5%)(P<0.05). In addition, constipation and complication was both improved to some extent after the implementation of quality control circle, the comprehensive scores of QC ability of the team rose from (19.50±4.116)to(2.42±4.158)(P<0.05). Conclusions The implementation of QCC improves the compliance of off-bed activities at the early stage after the Funnel Chest operation, reduces constipation and promotes the patients' rehabilitation.Meanwhile it improves the quality of nursing service, which not only enhances nursing quality and benefits in the construction of nursing team.     Abstract:

    Objective To explore the effects of parent synchronous health education-based continuing nursing among Funnel Chest children after Nuss procedure. Methods From June 2017 to December 2018, this study selected 80 Funnel Chest children with Nuss procedure and their parents in the First Affiliated Hospital of Zhengzhou University as subjects. All of subjects were divided into control group and observation group with the method of random table, 40 children and 40 parents in each group. Control group carried out routine nursing, while observation group implemented parent synchronous health education-based continuing nursing. The study compared the disease knowledge, care ability, mental status of parents and thoracic correction effects of children of two groups after observation group. Results A total of six months after intervention, the disease knowledge and care ability of parents in observation group were higher than those in control group with statistical differences (P<0.05) . The scores of anxiety and depression of parents in observation group were lower than those in control group with statistical differences (P<0.05) . A total of six months after surgery, the excellent rates of thoracic correction effects of children in observation group were higher than those in control group with a statistical difference (P<0.05) . Conclusions Parent synchronous health education-based continuing nursing can effectively improve disease cognition, care ability, mental status of parents and thoracic correction effects of children.     Abstract:

    Purpose. We examined the surgical indications for Funnel Chest, taking psychological factors into consideration. Methods. We assessed 36 young people with Funnel Chest who were seen as outpatients, including 31 boys and 5 girls aged from 1 to 22 years old. Respondents were asked whether they suffered psychological distress, and if they wanted surgery. The severity of the deformity was evaluated using the Vertebral Index (VI) and the Frontosagittal Index (FSI) calculated from Chest roentgenograms. Results. The VI in 11 patients without distress (23.7 ± 4.1) was lower than that in 25 patients with distress (32.8 ± 8.2), and the FSI in the patients without distress (33.5 ± 5.3) was higher than that in the patients with distress (23.6 ± 8.6). The VI in 19 patients who did not want surgery (26.9 ± 7.9) was lower than that in 17 patients who did (33.5 ± 7.5), and the FSI in the patients who did not want surgery (30.4 ± 8.1) was higher than that in the patients who did (22.4 ± 8.1). The distressed patients suffered many psychological problems, such as being the object of bullying. Conclusion. The severity of the deformity affected the patient's psychological state. We consider that a VI ≫28 or an FSI ≪28 are indications for surgery, based on the mean VI + SD and the mean FSI-SD of patients not suffering distress.

    Abstract:

    Funnel Chest is the most common Chest deformity, occurring in 0.06–0.3% of the general population. When it occurs concomitantly with esophageal cancer, it hinders intrathoracic surgery that is necessary for treatment. Although there are a few reports of esophagectomy performed in patients with Funnel Chest, simultaneous treatment of esophageal cancer and Funnel Chest with Funnel Chest surgery (Nuss method) and esophagectomy has not been reported. We report the first case of advanced esophageal cancer complicated by severe Funnel Chest that was treated using the Nuss method and radical thoracoscopic esophagectomy. A 59-year-old man was diagnosed with advanced thoracic esophageal cancer and severe Funnel Chest. Because his sternum was almost attached to the vertebral bone, thereby creating a narrow space in the mediastinum, esophageal surgery was expected to be complicated. After the patient underwent neoadjuvant chemotherapy, we used the Nuss method to reconstruct the Chest and widen the mediastinum. Subsequently, radical thoracoscopic esophagectomy was performed with the patient in the left decubitus position without any difficulty, and the postoperative course was uneventful. Simultaneous Funnel Chest surgery (Nuss method) and thoracoscopic esophagectomy with the patient in the left decubitus position are recommended for esophageal cancer patients with severe Funnel Chest.
