    Abstract:

    Tesis para obtener el grado de Magister Scientiae en Procesos Locales de Innovación y Desarrollo Rural (PLIDER), de la Universidad Nacional de La Plata, en agosto de 2020Este trabajo analiza la forma en que se implementó una política pública en un territorio específico como es la localidad de Ceres, en la Provincia de Santa Fe. Puntualmente se aborda la política que promueve el valor agregado en origen para la cadena cárnica, la cual formo parte del conjunto de programas del gobierno nacional que asume en el año 2003 y proponía un Estado con intervención directa en los procesos económicos y productivos. En ese marco nacional, llegaron al ámbito local iniciativas que iban acompañadas de los recursos financieros necesarios para llevarlas a cabo, y en torno a ellas los actores locales se movilizaron y desplegaron estrategias que incidieron en la implementación de la política pública y sus resultados finales. Para poder acceder a las características que tuvo este proceso, describirlo y explicarlo, se focalizó el estudio en dos emprendimientos vinculados a la transformación de la producción de carne, por considerarlos expresiones visibles de la política analizada. Uno fue el Frigorífico Matadero “Néstor Kirchner”, que formaba parte de un Plan gubernamental denominado Norte Grande. Y el otro fue el Matadero de la Escuela, que tiene su origen en el seno de una institución educativa de formación secundaria agrotécnica, y presenta un recorrido diferente al anterior. Se analizaron los distintos actores involucrados, las articulaciones que establecieron, posicionamientos, y roles que cada uno fue desempeñando en las diferentes etapas por las que atravesaron ambos emprendimientos. También se relevó la oferta programática ofrecida desde el Estado a través de sus instituciones. Se adoptó el enfoque Etnográfico, y se Planteó una investigación cualitativa. Las herramientas utilizadas para la recolección de datos fueron la observación participante, entrevistas semiestructuradas, entrevistas en profundidad, y revisión de fuentes secundarias. Algunas de las conclusiones a las que se pudo llegar con este abordaje, permiten afirmar que, a diferencia de lo que a priori se consideró un fracaso de la política, lo que se produce es una reconfiguración de la misma respecto a los objetivos y formas de implementación Planificadas. Dicha reconfiguración se da en base a dos procesos que son los que determinan la forma presente del emprendimiento. Uno es la apropiación por parte de capitales privados de los beneficios directos de la política pública. Y el otro es la apropiación del gobierno local de la política pública como herramienta de legitimación y construcción de poder. La incidencia de los técnicos locales y de las instituciones en los desarrollos de las iniciativas de valor agregado, quedó comprobada al analizar y poder describir los marcados cambios que se produjeron en ellas a partir de sus actuaciones. Se pudo demostrar también la presencia de un sistema de abastecimiento de carne que viene funcionando desde hace mucho tiempo y que implica acuerdos, vínculos comerciales, fidelidades, y negociaciones explícitas e implícitas, que tienen gran incidencia en la aplicación de la política pública, poniendo en riesgo la iniciativa que esta propone. En el caso del Frigorífico NK, hubo una falta de coordinación entre los Planes de acción previstos para la localidad por parte del Estado nacional, y las instituciones y técnicos que el mismo Estado tiene en el territorio. Se Plantean como desafíos, por un lado, la revisión de las políticas que ponen énfasis en la creación de infraestructura por sobre los procesos sociales, y por otro, apuntar al incremento de la capacidad estatal para intervenir con una mirada integral de los territorios, a través de sus instituciones y técnicos.This paper analyzes the way in which public policy was implemented in a specific territory such as Ceres, a city in Santa Fe province. It precisely addresses the policy that promotes added value in the source of origin for the meat chain, policy that was part of a number of programs by the National Government taking power in 2003, which proposed a State with direct intervention in the economic and productive processes. Within said national framework, there were locally promoted initiatives, combined with the necessary financial resources to carry them out, and around those initiatives the local actors mobilized and deployed strategies that had an impact on the implementation of public policies and their final results. In order to access the most relevant characteristics of this process, describe it and explain it, the study focused on two enterprises connected to the transformation of beef production, as they are considered visible manifestations of the analyzed policy. One is the meat-processing Plant and slaughterhouse “Nestor Kirchner”, which was part of the Government Plan called Great North (Norte Grande). And the other is a School slaughterhouse, which started within a secondary agrotechnical educational center, and which presents a different direction from the previous case. Different actors involved, established articulations, positioning, and the roles that each one carried out in each stage they experienced in both undertakings were analyzed. The programmatic offer provided by the State through its institutions was also relieved. An Ethnographic approach was adopted, and a qualitative research was considered. The techniques used in the data collection were participant observation, semi-structured interviews, in-depth interviews, and revision of secondary sources. Some of the findings reached through this approach, allow us to establish that -unlike what had been considered a failure of the policy beforehand-, what occurs is the reconfiguration of this regarding the objectives and ways of Planned implementation. Said reconfiguration is based on two processes that determine the way present in the venture. One is the appropriation of direct benefits of public policy by private capital. And the other is the appropriation of public policy as a tool of legitimation and construction of power by the local Government. The influence of local technicians and institutions on the development of the initiatives of added value was evident when the significant outcome achieved after their intervention was analyzed and described. It could also be demonstrated the presence of a meat supply system that has been working for a long time and which implicates agreements, commercial connections, loyalties, and explicit and implicit negotiations, which have a huge impact on the implementation of public policy, jeopardizing its own initiative. In the case of the meat-processing Plant NK, there was a lack of coordination between the action Plans that were expected for the city from the National State, and the institutions and technicians that the State itself has in the territory. There are challenges set out. On the one hand, the revision of the policies that emphasize the creation of infrastructure over social processes; and on the other hand, aiming to the increase of the State ability to take part with a comprehensive look at the territories, through its institutions and technicians.EEA RafaelaFil: Lovaisa, Marcelo.     Background: The twelve Integrated Care Program pilot projects (ICPs) created by the Government Plan ‘Integrated Care for Better Health’ aim to achieve four outcome types (the Quadruple Aim) for people with chronic diseases in Belgium: improved population health, improved patient and provider experiences and improved cost efficiency. The aim of this article is to present the development of a mixed methods realist evaluation of this large-scale, whole system change programme. Methods: A scientific team was commissioned to co-design and implement an evaluation protocol in close collaboration with the Government, the ICPs and several other involved stakeholders. Results: A protocol for a mixed methods realist evaluation was developed to gain insights into the mechanisms that foster successful results in ICPs. The qualitative evaluation proposed will be based on the document analysis of yearly ICP progress reports, selected case studies and focus group interviews with stakeholders. Processes and outcomes of all the projects will be monitored using indicators based on administrative data on population health and the quality and costs of care. A yearly survey will be organized to collect data on patient-reported outcomes and experiences and on provider-reported measures of inter-professional collaboration and proper wellbeing. Using both quantitative and qualitative data, we will develop theories about the mechanisms and the associated contextual factors that lead to integrated care and the Quadruple Aim outcomes. Discussion: The objective of this study is to deliver policy recommendations on strategies and best practices to improve care integration in Belgium and to implement a sustainable monitoring system that serves both policy makers and the stakeholders within the ICPs. Some challenges due to the large scale of the project and the multiple stakeholders involved may impede the successful implementation of this proposal

The qualitative evaluation proposed will be based on the document analysis of yearly ICP progress reports, selected case studies and focus group interviews with stakeholders. Processes and outcomes of all the projects will be monitored using indicators based on administrative data on population health and the quality and costs of care. A yearly survey will be organized to collect data on patient-reported outcomes and experiences and on provider-reported measures of inter-professional collaboration and proper wellbeing. Using both quantitative and qualitative data, we will develop theories about the mechanisms and the associated contextual factors that lead to integrated care and the Quadruple Aim outcomes. DISCUSSION: The objective of this study is to deliver policy recommendations on strategies and best practices to improve care integration in Belgium and to implement a sustainable monitoring system that serves both policy makers and the stakeholders within the ICPs.
