    Abstract:

    Secular trends in non-occupational post-exposure prophylaxis (NPEP) use have not been well-characterized. We performed a retrospective longitudinal study of 894 electronic medical records of NPEP users, mostly men who have sex with men, at a Boston community Health Center who presented between July, 1997 and August, 2013. NPEP use and condomless sexual exposures increased over time; 19.4% had multiple NPEP courses. Having an HIV-infected partner was associated with increased odds of regimen completion, and three-drug regimens were associated with decreased odds of completion. Targeted adherence and risk-reduction counseling are warranted for select NPEP users at this Center.

    Abstract:

    The ADVANCE (Accelerating Data Value Across a National Community Health Center Network) clinical data research network (CDRN) is led by the OCHIN Community Health Information Network in partnership with Health Choice Network and Fenway Health. The ADVANCE CDRN will 'horizontally' integrate outpatient electronic Health record data for over one million federally qualified Health Center patients, and 'vertically' integrate hospital, Health plan, and community data for these patients, often under-represented in research studies. Patient investigators, community investigators, and academic investigators with diverse expertise will work together to meet project goals related to data integration, patient engagement and recruitment, and the development of streamlined regulatory policies. By enhancing the data and research infrastructure of participating organizations, the ADVANCE CDRN will serve as a 'community laboratory' for including disadvantaged and vulnerable patients in patient-Centered outcomes research that is aligned with the priorities of patients, clinics, and communities in our network.

    Abstract:

    Individual or multiple resistance to clindamycin, tetracycline, erythromycin, levofloxacin, or mupirocin was detected in a large proportion of methicillin-resistant Staphylococcus aureus pulsed-field type USA300 isolates collected at an ambulatory Health Center in Boston. The clindamycin, tetracycline, and mupirocin resistance genes identified in these isolates are commonly associated with plasmids.

    Abstract Objective This study was designed for internal quality improvement to evaluate whether South Cimahi Public Health Center is suitable to become Inpatient Health Center. Method The qualitative descriptive study was chosen for this study. Data collection techniques used in-depth interviews, focus group discussions (FGD) and document studies. This research was analized by SWOT's method (Strengths, Weaknesses, Opportunities, Threats), IFE (Internal Factor Evaluation) and EFE (External Factor Evaluation) values. Results The result showed that South Cimahi Public Health Center having an IFE's value: 3.6; and EFE's value: 3.63. It means South Cimahi Public Health Center has high strength dan less weaknesses. As the result, the South Cimahi Public Health Center couid be recommended to become an inpatient Health Center. Based on these findings, it's recomended that the government of Cimahi to upgrade the facilities of South Cimahi Public Health Center to providea better medical support services in order to improve patient satisfaction.

    Abstract Objective Physicians face barriers to incorporating recommended contraceptive and preconception Health services, including reproductive life plans (RLPs), into primary care. With promising findings from early studies of RLPs, we examined the impact of a novel reproductive Health self-assessment tool (RH-SAT) on reproductive Health counseling. Methods We created the RH-SAT for an urban community Health Center population and trained providers on preconception and contraceptive guidelines. Semi-structured interviews were conducted to assess perceptions of the tool with 22 patients and with all 15 providers at the clinic. Transcripts were thematically analyzed using a grounded theoretical approach. Results Patients and providers reported the RH-SAT presented new and thought-provoking material that promoted patient participation and facilitated counseling. Conclusion This RH-SAT is acceptable and useful to patients and providers in an underserved urban Health Center. In accordance with Medical Communication Alignment Theory (MCAT), increased patient participation in reproductive Health discussions may alert providers to patient interest in these topics. Practice implications This study provides preliminary evidence that the RH-SAT can help overcome barriers to reproductive Health counseling in primary care. Providers may wish to incorporate tools into their practice to improve communication with patients about their reproductive Health goals.
